Based on the 2023 Indonesian Health Survey, the prevalence of overweight among adults reached 23.4% nationally and 24.5% in East Java Province. This condition highlights the need for food innovation, particularly high-fiber snacks suitable for overweight adults. This study aimed to develop cereal based on okara (soy pulp) flour and mung bean flour as an alternative high-fiber food product. Organoleptic testing was conducted on 30 semi-trained panelists evaluating color, aroma, texture, and taste using hedonic and hedonic quality tests. Data were analyzed using the Kruskal–Wallis test followed by the Mann–Whitney test, while nutrient content analysis was analyzed using One Way ANOVA followed by Duncan’s post hoc test. The results showed that in the hedonic test, color, texture, and taste parameters were significantly different (p<0.05), while aroma was not significantly different (p>0.05). In the hedonic quality test, color and taste showed significant differences (p<0.05), whereas aroma and texture did not (p>0.05). Overall, formulation 922 was the most preferred by panelists. The nutritional content of the cereal ranged from 407–410 kcal for energy, 76.44–77.53% carbohydrates, 8.91–9.66% protein, 6.80–7.30% fat, 5.20–5.51% moisture, 1.26–1.41% ash, and 9.72–10.90% total dietary fiber. The developed cereal is classified as a high-fiber food and has potential as a healthy snack for overweight adults.

The optimal therapeutic impact of local vaginal drug delivery systems is strongly influenced by the physical characteristics balance of Solid Vaginal Suppositories. A comprehensive review regarding the comparison of mechanical profiles, specifically melting time and crushing strength parameters, from various base classifications constitutes the primary objective of this literature research. The implementation of a Literature Review study design was executed through the extraction of empirical data from twelve experimental journals published within the last ten years. Excessively rapid phase transformation characteristics at physiological basal temperatures and low compression resistance were consistently demonstrated by lipophilic bases such as Oleum Cacao. The risk of structural deformation during the distribution process is highly susceptible to unmodified lipid preparations. High surface elasticity accompanied by a delay in molecular hydration duration reaching 120 minutes was recorded in the utilization of Glycerinated Gelatin Base. Structural rigidity exceeding 4 kgF and disintegration time efficiency under 60 minutes were optimally demonstrated by Polyethylene Glycol (PEG) Base. An enhancement in mechanical resistance against external shocks during the storage period is offered by the thorough modification of the synthetic polymer ratios. Therefore, the determination of the PEG base as the most optimal material is recommended to maintain the quality stability of pharmaceutical products. Compendial regulation standards regarding the physical strength testing of pharmaceutical preparations must be obeyed by every institution to ensure long-term treatment effectiveness. Thus, the alignment between active substance release duration and physical preparation endurance can be realized for absolute patient comfort.

Critically ill patients are individuals with unstable health conditions who require close monitoring of hemodynamic parameters. The high mortality rate among critically ill patients treated in intensive care units reaches 36.5% of 33,148 patients. Hemodynamic instability in critically ill patients can be influenced by underlying diseases, medical interventions, as well as physical and psychological stress responses during treatment. Therefore, in addition to pharmacological therapy, non-pharmacological interventions such as Asmaul Husna dhikr therapy are needed, as they can provide a relaxation effect and help stabilize hemodynamic parameters in critically ill patients. To determine the outcomes of the implementation of Asmaul Husna dhikr therapy. This study used a descriptive case study method involving two critically ill patients with unstable non-invasive hemodynamic parameters. Changes in non-invasive hemodynamic parameters were observed in both respondents after the implementation of Asmaul Husna dhikr therapy. In respondent 1, the hemodynamic status was initially categorized as increased and became stable after the intervention. Similarly, in respondent 2, the hemodynamic status changed from increased before the intervention to stable after the intervention. The implementation of Asmaul Husna dhikr therapy can be used as a complementary therapy to help stabilize hemodynamic parameters in critically ill patients.

Congestive heart failure (CHF) is a cardiovascular disease that frequently causes shortness of breath (dyspnea) and negatively impacts patients’ quality of life. The management of dyspnea involves not only pharmacological approaches but also nonpharmacological interventions, one of which is handheld fan therapy, which works by stimulating sensory receptors in the facial area. This study aims to describe the effect of handheld fan therapy on reducing shortness of breath in patients with CHF. This study employed a descriptive design using a case report method on a single 63-year-old female patient diagnosed with CHF. The intervention was conducted over four days, involving the use of a handheld fan for 10 minutes, held at a distance of 15–30 cm from the face and directed toward the nose and mouth. Measurements were taken before and after the intervention using respiratory rate and oxygen saturation as parameters. The results showed a gradual decrease in respiratory rate from 24 breaths/minute to 18 breaths/minute, as well as an increase in oxygen saturation from 91% to 96% during the intervention period. These findings indicate an improvement in breathing patterns and increased respiratory efficiency in the patient. Handheld fan therapy is effective as a nonpharmacological intervention for reducing shortness of breath and improving oxygenation in patients with CHF. This intervention is safe, easy to use, and can be implemented as a complementary therapy in nursing practice.

Calcium deficiency among Indonesian adolescents remains a nutritional concern that necessitates the development of food products utilizing alternative calcium sources. Eggshell powder has potential as a calcium fortification ingredient due to its high calcium content and abundance as food waste. This study aimed to analyze the effect of eggshell powder substitution on the organoleptic characteristics and acceptability of oatmeal cookies among adolescents aged 10–15 years. This experimental study used a Completely Randomized Design with four substitution levels: 0% (F0), 5% (F1), 6% (F2), and 7% (F3). Organoleptic quality testing was conducted by three trained panelists, while hedonic testing was performed by 30 adolescent panelists on selected formulas (F0, F2, F3), analyzed using the Friedman test (α = 0.05). Organoleptic quality results showed that F2 and F3 had characteristics closest to the control, while F1 showed higher negative attribute intensity in taste and texture parameters. Hedonic test results showed no significant differences between formulas across all parameters (p > 0.05), with mean scores ranging from 3.87 to 4.63 out of a scale of 5 (very much liked). Formula F3 is recommended as the best formula as it demonstrated acceptability comparable to the control while containing the highest calcium substitution level.

Effervescent granules are a pharmaceutical dosage form that offer advantages in terms of ease of use, improved palatability, and rapid solubility in water. This study aimed to formulate and evaluate the physical properties of vitamin C Effervescent granules using the dry granulation method. The main materials used included ascorbic acid as the active substance, citric acid and tartaric acid as acid sources, and sodium bicarbonate as the base source. Evaluation was carried out on organoleptic properties, flow properties, pH, angle of repose, moisture content, density, compressibility index, and dispersion time. The results showed that the granules met the requirements for organoleptic properties, pH (4–5), angle of repose (38.41°), compressibility index (12.3%), and dissolution time (49.62 seconds). However, the flow property (29.11 seconds) and moisture content (6.47%) did not meet the established standards. These findings indicate that the formulated granules met most of the evaluation parameters but still require optimization, particularly in the drying process and flow properties, to obtain a more stable and high-quality dosage form.

This review article evaluates the physical characteristics and stability of Oil in Water (O/W) and Water in Oil (W/O) creams through a comparative literature study published between 2021 and 2026. The evaluation focuses on important parameters, including viscosity, spreadability, adhesion, pH, and accelerated stability testing. The findings reveal that O/W creams generally have lower viscosity and greater spreadability, making them more suitable for daily cosmetic and therapeutic applications due to their lighter texture and better aesthetic acceptance. In contrast, W/O creams demonstrate higher physical stability, stronger adhesion, and superior occlusive properties, which are beneficial for protecting sensitive active ingredients and maintaining skin hydration over longer periods. The differences in performance are strongly influenced by the optimization of emulsifier Hydrophilic-Lipophilic Balance (HLB) values and the ratio between oil and water phases. Both cream types show good biocompatibility with skin pH ranging from 4.5 to 6.5, indicating their safety and effectiveness for topical pharmaceutical and cosmetic formulations.

Urban development has resulted in an increase in impermeable surfaces, increasing the risk of runoff inundating the road network. This situation occurs in the drainage system on Jl. Letjend S. Parman, Jl. Emo Kurniaatmaja, and Jl. Pulau Banda, Pasirkareumbi Village, Subang Regency. This study aims to evaluate the hydraulic performance of existing channels and develop redesign guidelines to reduce the risk of local flooding. The methods used include hydrological analysis of daily maximum rainfall data from the Subang rainfall station, rainfall frequency analysis using the Generalized Extreme Value (GEV) distribution, Intensity-Duration-Frequency (IDF) curves, and hydrological and hydraulic simulations using the Storm Water Management Model (SWMM). Model parameters include sub catchment area, rainfall intensity, time of concentration, infiltration using the Curve Number method, channel dimensions, cross-sectional shape, channel material, and the Manning coefficient. The results indicate that the GEV distribution provides a good fit for determining design rainfall. Based on the typology of a metropolitan city and a catchment area of ​​26.04 ha, the modeling period is 2 to 5 years. The SWMM simulation results indicate that some channel segments have very high flow velocities, potentially causing scour, while other segments have very low velocities, potentially causing sedimentation. This confirms the need for evaluation and restructuring of channel dimensions and slopes to improve the hydraulic performance of road drainage and reduce localized flooding.

This community service activity aims to improve the effectiveness of clean water distribution management through the implementation of a Water Level Control (WLC) system as an automatic control for water pumps and three-phase motors at a regional drinking water supply company in Lamongan. The methods employed include field observation, problem identification, system design and implementation, as well as periodic evaluation of system performance. The activity focuses on the water treatment unit by conducting control and monitoring of operational parameters such as frequency (Hz), pump pressure, and flow stability to ensure optimal water distribution according to consumer needs. The results indicate that the implementation of the WLC system enhances operational efficiency, minimizes human error, and supports more structured and sustainable equipment maintenance. In addition, this activity contributes to improving the knowledge and technical skills of personnel in operating, monitoring, and maintaining automated control systems. In managing automation-based technology in a professional and independent manner, human resource capacity is strengthened, while the quality of clean water services is also improved through this community service program.

This meta-review evaluates the effectiveness of diverse mental health programs targeting Syrian refugees in Turkey over the period 2015-2023 through a systematic synthesis of 47 studies involving a total of 12,847 participants. Meta-regression results demonstrate a statistically significant reduction in Post-Traumatic Stress Disorder symptoms, with a large effect size (d = -0.86, 95% CI [-1.02, -0.70], p < .001), as well as a significant reduction in depressive symptoms (d = -0.72, 95% CI [-0.88, -0.56], p < .001) following intervention. Among the therapeutic approaches examined, Cognitive Behavioral Therapy exhibited the strongest therapeutic effect (β = 0.45, p < .001), followed by Eye Movement Desensitization and Reprocessing (β = 0.38, p < .001). Further moderator analysis revealed that group-based interventions were more effective than individual approaches (OR = 1.86, 95% CI [1.42, 2.30]). At the same time, program duration of 12-16 weeks showed a strong correlation with optimal clinical outcomes (r = 0.67, p < .001). These findings extend the results of Acarturk et al. (2022) regarding the effectiveness of CBT and reinforce the argument advanced by Shehadeh et al. (2016) concerning the superiority of group-based approaches, while also indicating a longer optimal intervention duration than previously reported. In contrast to the meta-analysis by Antuña-Camblor & Hernández (2025), which emphasized EMDR, this study identifies CBT as superior to EMDR among Syrian refugees in Turkey. The novelty of this study lies in its comprehensive analysis of program moderator factors and the identification of the most effective intervention duration parameters for the target population.

Chronic kidney disease (CKD) is a global health problem with continuously increasing prevalence and mortality. This condition is characterized by a progressive and irreversible decline in kidney function, which is clinically assessed using the estimated glomerular filtration rate (eGFR) as one of the main parameters for staging and monitoring disease progression. Red blood cell distribution width (RDW) is a hematological parameter that reflects the degree of variation in erythrocyte size. In patients with CKD, elevated RDW levels are often associated with oxidative stress and chronic inflammation, which may accelerate kidney damage and increase mortality risk. To analyze the correlation between RDW and eGFR values in patients with chronic kidney disease at ODSK Hospital, North Sulawesi Province. This study used an analytical observational design with a cross‑sectional approach. It was conducted retrospectively using secondary data from the medical records of CKD patients at ODSK Hospital, North Sulawesi Province, from February 2024 to August 2025. This study used a total sampling method with 189 CKD patients who met the inclusion and exclusion criteria.  Based on Spearman-rank correlation analysis, the p‑value obtained was 0.129 (>0.05). There was no significant relationship between RDW and eGFR values in CKD patients at ODSK Hospital, North Sulawesi.Province.

This study investigates the levels of asprosin and irisin in patients with type 2 diabetes mellitus (T2DM) and diabetic nephropathy (DN), and evaluates their relationships with insulin resistance, glycemic control, and renal function. Additionally, it explores their diagnostic performance as potential non-invasive biomarkers for the early detection of DN using ROC curve analysis. A controlled prospective study was conducted involving 130 participants, categorized into healthy controls, T2DM patients, and T2DM with DN. Serum asprosin, irisin, and insulin levels were measured using ELISA, while biochemical and renal parameters such as fasting blood glucose, HbA1c, creatinine, urea, and eGFR were assessed using standard spectrophotometric techniques. ROC analysis was employed to assess the diagnostic accuracy of asprosin and irisin. Asprosin levels were significantly elevated, while irisin levels were markedly reduced in DN patients compared to T2DM and control groups (p < 0.001). Moreover, DN patients exhibited higher levels of fasting glucose, HbA1c, HOMA-IR, creatinine, and urea, with lower eGFR, indicating substantial renal dysfunction. ROC analysis revealed that asprosin had an AUC of 0.910 (95% CI: 0.839–0.981), with 90.0% sensitivity and 88.9% specificity at a cutoff value of 11.27 ng/mL. Irisin showed an AUC of 0.886 (95% CI: 0.812–0.960), with 95.0% sensitivity and 71.1% specificity at a 158.25 ng/mL cutoff. Asprosin and irisin are strongly associated with insulin resistance and renal impairment in patients with T2DM and DN. Their high diagnostic performance supports their utility as promising non-invasive biomarkers for the early detection and monitoring of diabetic nephropathy progression.

Arabica coffee (Coffea arabica) contains various bioactive compounds such as caffeine, polyphenols, flavonoids, and alkaloids that have the potential to produce diverse pharmacological effects on both the autonomic and central nervous systems. The variation in its chemical composition is influenced by factors such as cultivar, post-harvest processing, and preparation methods, resulting in biological responses that are dose-dependent and not always linear. This study aimed to evaluate the pharmacological activity of arabica coffee simplicia powder at three concentrations (1%, 2%, and 4%) by observing parasympathomimetic, parasympatholytic, sympathomimetic, sympatholytic, analeptic, central nervous system stimulation and depression, as well as muscle relaxation parameters. The research was conducted using a laboratory experimental design with test animals administered orally according to the designated concentrations. The results showed that some parameters increased at higher concentrations, such as parasympathomimetic activity (51.85%) and central nervous system stimulation (63.74%), while other parameters demonstrated decreasing or biphasic patterns, such as sympatholytic activity, which was high at 1% and 4% but low at 2%, and analeptic activity, which dropped sharply at 2% and increased again at 4%. These response patterns align with literature indicating that caffeine and polyphenols such as chlorogenic acid may interact synergistically or antagonistically, generating complex pharmacological effects through mechanisms involving adenosine modulation, excitatory neurotransmitters, the enteric nervous system, as well as antioxidant and anti-inflammatory pathways. Overall, this study demonstrates that arabica coffee simplicia exhibits multipotential pharmacological activity, with responses strongly influenced by concentration and the composition of its bioactive compounds, thereby providing a basis for further research on the phytopharmaceutical potential of arabica coffee.

A high-fat diet can cause hyperlipidemia, which is one of the main risk factors for atherosclerosis. Papaya flowers (Carica papaya L.) contain antioxidants that have the potential to inhibit the development of atherosclerotic lesions. This study aims to determine the effect of papaya flower extract on the histopathological features of the aorta of Wistar rats induced by a high-fat diet. The study design is a posttest only-trial group design. The study sample consisted of 25 male Wistar rats divided into 5 groups with treatment for 21 days. Group A received no treatment. Group B was given high-fat diet consisting of 3 mL of pork fat and 2 g of duck egg yolk. Group C was given high-fat diet and simvastatin 0.18 mg/200 gBW. Groups D and E were given high-fat diet and papaya flower extract 125 mg/kgBW and 250 mg/kgBW, respectively. The parameter used is the histopathological description of the aorta in the form of foam cell formation. The results showed that there were few foam cell in group A. Group B had many foam cells in the tunica intima and media of aorta. Group C, D and E showed a reduction in the number of foam cells. The administration of papaya flower extract (Carica papaya L.) was effective in inhibiting foam cell formation in the aorta of Wistar rats induced by a high-fat diet, with a better effect seen at a dose of 250 mg/kgBW, which was almost comparable to simvastatin.

Sindrom metabolik merupakan isu kesehatan yang semakin menonjol di Indonesia, dengan prevalensi mencapai 21,66% dan angka lebih tinggi pada populasi urban. Literasi gizi, sebagai kapasitas individu untuk memahami dan menerapkan informasi nutrisi, dipandang berperan penting dalam mengarahkan perilaku makan serta memengaruhi status metabolik. Namun, pada komunitas adat seperti masyarakat Badui, struktur sosial-budaya dan pola hidup tradisional dapat menciptakan dinamika yang berbeda dibandingkan populasi umum. Penelitian ini bertujuan menganalisis hubungan antara tingkat literasi gizi dan parameter kesehatan metabolik pada masyarakat Badui Luar. Studi observasional dengan desain potong lintang ini melibatkan 41 responden dewasa. Literasi gizi diukur menggunakan kuesioner terstandar, sedangkan parameter metabolik meliputi lingkar perut, tekanan darah, kadar glukosa darah puasa, trigliserida, dan HDL yang diperiksa menggunakan metode klinis sederhana. Analisis korelasi Pearson digunakan untuk menilai hubungan antarvariabel. Hasil penelitian menunjukkan bahwa literasi gizi berada pada kategori cukup baik (123,73 ± 7,89), namun tidak berhubungan signifikan dengan seluruh parameter metabolik (r –0,218 hingga 0,241; p>0,05). Pola ini mengindikasikan bahwa faktor budaya, aktivitas fisik agraris, serta pola konsumsi tradisional kemungkinan berperan lebih dominan dalam membentuk profil metabolik masyarakat Badui dibandingkan literasi gizi formal.

This study aims to evaluate the relationship between skinfold calliper measurements and handgrip strength with segmental fat and muscle composition among adults in Kota Bambu, providing evidence for simple and applicable community-based screening tools. A cross-sectional design was applied to 135 participants aged 18–96 years. Skinfold thickness was assessed at four anatomical sites (biceps, triceps, suprailiac, scapular), handgrip strength was measured using a digital dynamometer, and segmental body composition was obtained via bioelectrical impedance analysis. Findings demonstrated a moderate positive correlation between handgrip strength and arm skeletal muscle mass (r = 0.371–0.407; p < 0.01), indicating that handgrip performance reflects segmental muscle contractility and functional reserve. Skinfold measurements showed moderate-to-strong positive correlations with both local and central subcutaneous fat distribution (r = 0.562–0.635; p < 0.01), confirming their sensitivity in estimating segmental adipose accumulation. These results highlight that calliper and handgrip strength can serve as practical, low-cost preliminary screening indicators for mapping muscle and fat distribution in urban communities, although they are not substitutes for comprehensive body composition assessment. Further longitudinal studies integrating advanced physiological and functional parameters are recommended to enhance predictive validity and clinical applicability.

The extract of Carica papaya L. leaves contains phytochemical compounds such as flavonoids, alkaloids, phenolics, and saponins that play a role in biological activities, including antioxidants, anti-inflammatory, analgesic, and modulation of the nervous system. This study aims to evaluate the effects of fresh papaya leaf extract on the central nervous system (CNS) and autonomic nervous system based on varying extract concentrations. The study used test animals with treatment groups consisting of negative control, positive control, and extracts at concentrations of 4%, 8%, and 16%. Parameters observed included parasympathomimetic (PSM), parasympatholytic (PSL), sympathomimetic (SM), sympatholytic (SL), analeptic effects (ANA), CNS stimulation (SSSP), CNS depression (DSSP), and muscle relaxation (RO). The results showed that the 16% concentration produced the highest PSM activity (46.82%) and dominant CNS stimulation (38.59%), while DSSP and RO decreased at higher concentrations. This phenomenon indicates that increasing the extract concentration does not always enhance CNS depression, but rather leads to stimulation. These findings support the potential of papaya leaf extract as a neuroprotective and multifunctional agent.
