This study analyzes failures in the bending machine at PT. XYZ and determines maintenance priorities to reduce downtime and improve production efficiency. The company often faces repeated breakdowns, especially in hydraulic and control components, which negatively impact productivity. To address these issues, the research applies Failure Mode and Effect Analysis (FMEA) and Fault Tree Analysis (FTA). The study employs a descriptive qualitative approach using downtime and repair data from September 2024 to February 2025. FMEA was conducted to identify failure modes, effects, and causes, and to calculate the Risk Priority Number (RPN) as a basis for prioritization. FTA was then applied to trace root causes by mapping logical relationships among contributing factors leading to the top event. Recommendations were formulated with the 5W+1H method to propose preventive maintenance actions. The results indicate that the hydraulic valve is the most critical component, with an RPN value of 504 due to oil contamination. The main causes include damaged filters, improper oil usage, and lack of a cooling system. The hydraulic cylinder seal and back gauge were also found to contribute significantly to machine failures. FTA analysis revealed root causes such as inadequate maintenance procedures, unsuitable materials, and insufficient inspections. The proposed improvements involve regular replacement of oil filters, structured lubrication schedules, installation of oil coolers, and technician training to strengthen compliance with standard procedures. Overall, the integration of FMEA and FTA provides a systematic approach to identify critical components and root causes, enabling PT. XYZ to implement preventive strategies that minimize failures, reduce downtime, and improve bending machine performance sustainably.

This study aims to evaluate the Resource Development and Installation (RDI) technique to reduce symptoms of Posttraumatic Stress Disorder (PTSD) in female victims of domestic violence. Domestic violence (which falls under the category of intimate partner violence/IPV) is a global public health problem that contributes significantly to women's mental health disorders, including post-traumatic stress disorder (PTSD). Recent systematic evidence shows a strong association between various forms of IPV and adverse mental health outcomes in women, with large variations in prevalence across contexts. Resource Development and Installation (RDI) is a psychological stabilization procedure within EMDR aimed at generating positive resources in individuals exposed to traumatic experiences. The research method used in this study was multiple baseline with a single case (single case research), namely research conducted on subjects with the aim of determining the magnitude of the effect of treatment given repeatedly over a certain period. This study involved three adult female participants who experienced trauma symptoms due to violence perpetrated by an intimate partner in a domestic context. The data analysis technique used in this study was visual inspection by comparing changes in trauma scores in respondents between the baseline and intervention phases. Trauma symptoms were measured using the Harvard Trauma Questionnaire (HTQ). There was a decrease in HTQ scores from an average of 2.9 (presence of PTSD symptoms) before the intervention to 2.1 (minimal). Resources Development and Installation were effective in reducing trauma symptoms in female victims of violence. This was evident in the decrease in trauma symptom scores between before and after the provision of Resources Development and Installation to female victims of violence.

This research aims to analyze the forms of legal protection for consumers in the case of illegal cosmetic circulation in the Traditional Market of Moluo Village, Kwandang District, North Gorontalo Regency, and to examine the roles of the Health Department and the Gorontalo Food and Drug Supervisory Agency (BBPOM) in supervising and guiding business actors. The research method used is empirical legal research with a non-doctrinal qualitative approach, thru interviews with sellers, consumers, and supervisory authorities, as well as field observations and document studies. The research results show that the majority of vendors in the market already understand the importance of a distribution permit from BPOM and choose to sell legal products, although in practice illegal cosmetics are still found due to high consumer demand for instant results. The main obstacle lies in consumers who are often aware of the dangers of illegal cosmetics but still purchase them. BPOM Gorontalo implements legal protection thru two channels: preventive measures such as socialization, education, and routine inspections, and repressive measures such as legal action in accordance with Article 435 of Law Number 17 of 2023 concerning Health. The implications of this research confirm the need for more comprehensive legal protection strategies with cross-agency collaboration, increased public legal literacy, and consumer empowerment to make them more critical in product selection.

The background of this research is based on Cervical cancer, a disease that poses a serious threat to women's health in Indonesia. Early detection through prevention programs such as IVA (Visual Inspection with Acetic Acid) is a crucial step in reducing the number of cervical cancer cases. In this effort, an effective communication strategy from the Pasar Minggu District Health Center plays a very important role. This study focuses on the prevention program and communication strategies implemented at the Pasar Minggu District Health Center. This study uses a qualitative approach with a descriptive study method. Data collection techniques were carried out through in-depth interviews with Health Center officers, observations of socialization activities, and documentation. The results show that the communication strategies implemented by the Pasar Minggu District Health Center include informative, educational, and persuasive communication strategies. The Health Center utilizes various communication channels such as social media and direct counseling. Obstacles faced include low levels of public health literacy, stigma against IVA examinations, and limited human resources.

PT. YGI, an automotive manufacturing company, uses the Press Injection GB-36 machine in its production process. However, this machine frequently experiences failures, causing high downtime and reduced production efficiency. This study aims to identify the types of failures, determine the most critical components, and propose a maintenance system using the Reliability Centered Maintenance (RCM) approach. The method used is descriptive qualitative, involving FMEA (Failure Mode and Effect Analysis), Logic Tree Analysis (LTA), and maintenance action selection. Data were collected through observations, historical failure documentation, and interviews. The analysis results indicate that components such as the injection nozzle, heating, and clamping are the most critical, contributing over 80% of total failures based on Pareto analysis and the highest RPN values in FMEA. Proposed maintenance actions include Condition Directed and Time Directed approaches. Additionally, the maintenance system is supplemented with standard operating procedures (SOP) and routine inspection schedules to improve machine reliability and reduce production downtime. This study is expected to enhance the efficiency and productivity of the Press Injection GB-36 machine at PT. YGI through the appropriate implementation of RCM.

This research aims to provide an in-depth explanation of the extension procedure for the Equipment Safety Certificate of the Tug Boat TB. Mitra Jaya XVI. This process is conducted at the Class I Port Authority and Harbormaster's Office (KSOP) Dumai and managed by PT. Berlian Ocean Shipping Dumai as the shipping agent. The equipment safety certificate is a vital document ensuring the seaworthiness of the vessel's equipment and compliance with maritime regulations. The research methodology utilized is a combination of Field Research—involving direct observation at the site—and Library Research—for gathering relevant regulatory and theoretical data. The findings indicate that the process for managing the certificate extension has generally complied with the established Standard Operating Procedures (SOP). Nevertheless, applicants still face significant obstacles during the application process. These constraints include delays in document inspection by related parties and technical issues within the online SIMKAPEL system (Information Management System for Harbormaster and Ship Services), which frequently experiences disruption. These system outages repeatedly lead to the postponement of certificate issuance. The conclusion of this study emphasizes that while formal procedures are followed, there is a necessity for optimizing the ship agency services and internal supervision. It is crucial to implement improvements to the technical system of SIMKAPEL and enhance internal coordination between surveyors and administrative staff at the KSOP to reduce waiting times, ensuring that certificates are issued promptly for the smooth operation of the vessel.

Bell's Palsy is a sudden onset of peripheral facial nerve paralysis that is generally unilateral. This condition is characterized by facial muscle weakness, asymmetry, difficulty closing the eyes, and facial expression disorders. Associated risk factors include exposure to cold temperatures and the habit of chewing ice cubes, which can trigger vasospasm of the blood vessels and inflammation of the facial nerve.  Physiotherapy management is important to accelerate recovery, prevent complications such as synkinesis and contractures, and improve the patient's quality of life. This study aims to describe the implementation of physiotherapy in cases of Bell's Palsy at PKU Muhammadiyah Selogiri Hospital and evaluate the effectiveness of physiotherapy intervention on the recovery of facial function in patients. A case report was conducted on a 41-year-old female patient diagnosed with right-sided Bell's Palsy. The examination included anamnesis, static and dynamic inspection, palpation, basic movement examination, sensory examination, and assessment using the House-Brackmann Facial Grading System (HBGS). Physiotherapy interventions were given three times a week, including Infra Red (IR), Transcutaneous Electrical Nerve Stimulation (TENS), light massage, facial exercises (raising eyebrows, closing eyes, blowing, smiling), and education on preventing exposure to cold. Evaluation was performed using Manual Muscle Testing (MMT) and Ugo Fisch. The implementation of physiotherapy with a combination of IR, TENS, massage, and facial exercises is effective in gradually improving the facial function of patients with Bell's Palsy. Physiotherapy also plays a role in educating patients about the prevention of recurrence related to environmental risk factors and lifestyle.

Quality control is a crucial aspect in various fields, such as education, the food industry, and manufacturing, as it functions to maintain the consistency of service or product quality. Inaccuracy in managing quality can cause financial losses, damage an institution's reputation, and reduce stakeholder trust. Based on this urgency, this study reviews recent literature to answer the main question of how quality control strategies can be effectively applied in diverse organizations. The review was conducted through a qualitative literature review approach by exploring scientific publications via Google Scholar and the Publish or Perish (PoP) application. Article selection was based on the relevance of keywords related to quality control and was limited to publications from 2020–2025. Out of 24 articles found, filtering was done until 5 of the most relevant articles remained for in-depth analysis. The analysis reveals three main findings. In higher education, a strong internal quality system and a sustainable quality culture are needed. The food industry emphasizes the standardization of raw materials and optimization of production processes. Meanwhile, the manufacturing sector makes extensive use of statistical techniques and tiered inspections. Overall, the effectiveness of quality control requires a combination of technical, managerial, and organizational culture aspects. Future research is recommended to explore the integration of digital technology and sustainability principles in quality control practices.

Procedures for Issuing Port Health Quarantine Clearance (PHQC) and Ship Health Book for the MT. Venus Ship at Kuala Tanjung Port by PT. Seroja Jaya Agensi. Every ship entering the port must be in good health, free from disease outbreaks and pests. Issuance of ship health documents is the responsibility of the Health Quarantine Agency (BKK). PT. Seroja Jaya Agensi, as the agency company, acts on behalf of the ship owner in processing ship health documents at Kuala Tanjung Port. This study aims to determine the procedures for issuing Port Health Quarantine Clearance (PHQC) and Ship Health Book for the MT. Venus Ship. The procedure begins with the ship agent submitting an application to the BKK, followed by a sanitation inspection by health officers, issuance of a Certificate of Free Practice (CoP), and recording or issuing the ship health book. These documents are the primary requirements for the ship's clearance process. The research methods used were field research and library research. The results showed that the document issuance procedure involved close coordination between the ship's agent and relevant agencies, and required careful attention to the completeness and validity of the ship's documents.

This research aims to analyze the impact of implementing the International Safety Management (ISM) Code internal audit on the operational smoothness of the vessel MV. Martha Golden, managed by PT. Multi Jaya Samudera Belawan. The implementation of the ISM Code is a mandatory requirement regulated by Law No. 17 of 2008 concerning Shipping, designed to ensure safe ship operation and the prevention of marine pollution. The research methodology employed is field research (field research) through observation and the collection of direct operational data, complemented by a literature review (library research) to examine related regulations and theory. The findings demonstrate that the consistent execution of the ISM Code internal audit has a significant positive influence on improving the vessel's safety and operational integrity. This audit, carried out by the Designated Person Ashore (DPA), encompasses scheduling, conducting thorough inspections onboard, and reporting findings. Consistent auditing minimizes the risk of accidents at sea, enhances the compliance of all crew members with the applicable Safety Management Sistem (SMS), and ensures that the vessel operates safely and efficiently according to international standards. The study concludes that discipline in performing internal audits and the crew's adherence to SMS procedures are the primary keys to maintaining operational integrity. It is recommended that the company increase the frequency and quality of internal audits and crew training to ensure the vessel's safety and operational efficiency are consistently maintained.

Refusal of Visual Inspection with Acetic Acid (VIA) examination remains a major challenge in cervical cancer early detection, particularly among women of reproductive age. This study aimed to analyze factors influencing refusal of VIA examination among women of reproductive age in Beringin Jaya Village, Oba Tengah District. A quantitative analytic study with a cross-sectional design was conducted involving 74 respondents. Data were collected using a structured questionnaire assessing educational level, knowledge, information, and the role of health workers. Data analysis included univariate analysis, bivariate analysis using the Chi-Square test, and multivariate analysis using ordinal logistic regression.The results showed that most respondents refused or hesitated to undergo VIA examination. Bivariate analysis demonstrated that educational level, information, and the role of health workers were associated with refusal of VIA examination, while knowledge level showed a tendency toward association. However, multivariate analysis revealed that only the role of health workers had a statistically significant effect on refusal of VIA examination. Educational level, knowledge, and information were not significant predictors after adjustment.These findings indicate that service-related factors, particularly the role of health workers, are more influential than individual factors in shaping decisions regarding VIA examination. Strengthening health workers’ roles through effective communication and interpersonal counseling is essential to improve VIA examination uptake.

The 5-star hotel is full of beauty, futuristic design, and perfect service. One of the components that guests complain about at the Tentrem Hotel Yogyakarta is housekeeping negligence. The research method is qualitative, with data collection techniques including interviews, observations, and documentation. The results of the study show that the forms of negligence that occur in housekeeping services at Hotel Tentrem Yogyakarta are 3 (three) actions: first, negligence in cleaning guest rooms, errors in placing guests' personal belongings, and delays in providing additional requests (amenity delay). Housekeeping negligence affects guest satisfaction at Hotel Tentrem Yogyakarta by reducing the reliability and tangibles dimensions of service quality. Guests feel the hotel cannot be trusted to provide a comfortable stay experience, leading to a decrease in the empathy and assurance dimensions. Guests feel their privacy is violated, which may even lead to suspicion toward hotel staff. Efforts to improve the negligence occurring in housekeeping services at Hotel Tentrem Yogyakarta involve three actions: implementing an inspection checklist and supervisor verification before guests check-in, ensuring staff are well-trained, educated, and professional in their work, and adopting a digital logistics system to record guest requests and prioritize deliveries.

Amphibian Cessna 172 SP is an aircraft designed to operate in aquatic environments and is frequently exposed to seawater, which is highly corrosive. Such exposure poses a serious issue of corrosion affecting both the structure and components of the aircraft, particularly the floats, lower fuselage, and propeller. Corrosion not only reduces the quality and integrity of the material but also potentially compromises flight safety and increases maintenance costs. This study aims to analyze the causes of corrosion on the Seaplane Amphibian Cessna 172 SP using the fishbone diagram to identify the main contributing factors, including material, method, environment, manpower, and machine. In addition, the 5W+1H approach is employed to examine the problem in greater detail and develop preventive strategies. The results of the analysis indicate that the dominant factors causing corrosion are the high intensity of direct contact with seawater, suboptimal cleaning processes due to limited manpower, and the use of cleaning agents not fully compatible with aircraft materials. Mitigation efforts include implementing more effective cleaning procedures, applying anti-corrosion chemicals such as AeroShell Fluid 41, Bonderite Turco S.S.2, Ardrox AV-15, CorrosionX, Toolmates Dry Film Lubricant 6075, and Ultra Tef-Gel, as well as conducting scheduled inspections at specific intervals. This research is expected to provide more appropriate preventive strategies to extend the service life of the aircraft structure while maintaining both operational safety and efficiency.

Preventing accidents during drilling and well service operations is a constant and primary concern for every company or contractor operating in the upstream business. Accident analyses often identify a number of causative factors and/or anomalies, where equipment-related items play a significant role. This article concentrates on the rig inspection process as a consistent method for detecting equipment-related anomalies before operations commence. These inspections allow necessary repairs to be made in a timely manner, thereby limiting the occurrence of accidents after the rig becomes operational. When thorough and systematic rig inspections are performed, the percentage of accidents where equipment failure is the primary cause remains very low. However, the factor of equipment condition contributing to accidents still persists in a significant number of cases. Drilling rig mast inspection is a crucial component in maintaining the safety and efficiency of drilling operations. This inspection process aims to detect structural anomalies and equipment conditions that could affect rig performance. In this study, we analyze the inspection methods applied to rig masts, including visual inspection, non-destructive testing, and condition monitoring. The results of these inspections are expected to identify potential risks and enable necessary repairs before operations begin. Through the implementation of systematic and standardized inspection procedures, companies can reduce the likelihood of accidents and equipment failures, thereby enhancing workplace safety. This study also discusses the importance of training for inspection personnel to ensure they possess the necessary knowledge and skills to detect potential problems that may arise. Thus, drilling rig mast inspection contributes not only to safety but also to overall operational efficiency.

The rapid development of information technology in the digital era has transformed the way organizations conduct their business activities. Information systems are now viewed not merely as operational support tools but as strategic elements capable of providing added value, increasing efficiency, and strengthening a company's competitiveness. In the context of PT XYZ, a company engaged in inspection and certification services, the use of information technology has become a crucial part of daily work processes. However, the implementation of information systems at the branch level still faces several obstacles. Data and process integration is not yet fully optimized, while alignment between the systems used and business needs has also not been achieved. This condition has the potential to hinder the company from providing fast, accurate, and innovative services. Therefore, appropriate and sustainable strategic planning of information systems is necessary. This study uses the Ward and Peppard framework enriched with Value Chain analysis to identify value-added activities, SWOT to identify strengths, weaknesses, opportunities, and threats, and PEST to map external factors affecting the business. In addition, the McFarlan Strategic Grid is used to prioritize the application portfolio that aligns with the company's long-term goals. The results of this research are expected to be able to produce an information system strategy design that not only supports operational efficiency, but also encourages service innovation and increases the competitiveness of PT XYZ in the inspection and certification services industry.

In the automotive manufacturing industry, efficiency in quality control is a crucial factor to ensure consistent product quality. Conventional Quality Assurance (QA) processes using manual record-keeping often face challenges such as delayed reporting, human errors, and difficulty in tracking historical data. This study aims to design and implement a QA performance dashboard based on digital forms at PT Dharma Polimetal, Tbk, to enhance efficiency in production quality control. The research methodology includes direct field observation, collection of production and QA data, mapping of QA process flows, interactive dashboard interface design, and system trial implementation. The designed dashboard focuses on four main aspects: QA Incoming, QC Line, QC Gate, and Customer Handling, each containing measurable performance indicators and quality parameters. Initial implementation results indicate significant improvements in QA process monitoring, faster reporting of inspection results, and easier real-time data access for both production teams and management. The system enables early detection of potential quality issues, supports rapid decision-making, and facilitates internal and external audits. Moreover, the use of digital forms within the dashboard enhances data accuracy, minimizes human error, and creates structured historical records for long-term analysis. This study provides a tangible contribution to the digitalization of QA systems, strengthening sustainable quality control practices in the automotive industry, thereby ensuring consistent productivity and product quality.

Shrimp is a food ingredient that has perishable characteristics, so it requires proper handling to maintain product quality and safety. This study aims to understand the application of the quality system in frozen shrimp processing based on the principle of Hazard Analysis Critical Control Point (HACCP) at PT Dachan Mustika Aurora Tarakan. The research was carried out during the period from July to December 2024 with a qualitative descriptive approach. The results of the study show that the frozen shrimp processing system has followed the principles and stages of HACCP in general, starting from the receipt of raw materials, temperature checks, organoleptic tests, laboratory tests, processing with temperature control, washing, sorting, packaging, to storage in cold storage. However, there are still critical points at the Metal Detector washing and inspection stages, with the potential for significant danger in the form of metal chip contamination. The quality control process is carried out in layers to ensure that the final product meets national and international standards. The implementation of HACCP has consistently proven to be effective in improving food safety, maintaining the quality of frozen shrimp, and strengthening the competitiveness of products in the export market. These findings provide recommendations for the seafood processing industry to strengthen surveillance systems at critical points and improve personnel training to minimize the risk of contamination and ensure the sustainability of product quality.

This study aims to analyze the effect of transformer oil purification on oil breakdown voltage and evaluate its benefits in maintaining transformer performance and reliability. Oil purification is a crucial preventive maintenance step to preserve insulation quality and prevent operational failures caused by reduced dielectric properties. The study was conducted through several stages, including data collection, oil purification, measurement of breakdown voltage before and after purification, and evaluation of results. Data collection involved measuring the oil’s electrical properties according to SPLN 49-1:1982 and observing results using a Break Down Voltage (BDV) test. Purification of a Trafindo 400 kVA transformer was performed through visual inspection, connecting the inlet and outlet hoses to the purification machine, and circulating the oil until the breakdown voltage met the required standards. Results indicated that the oil breakdown voltage before purification was below standard due to reduced insulation quality caused by water contamination, charcoal particles, thermal degradation, and dissolved gases that weakened dielectric properties. Additional factors such as electrical stress, mechanical stress, and excessive loading also contributed to insulation deterioration. After purification, the oil breakdown voltage increased significantly to meet the standard of >30 kV/2.5 mm, demonstrating that purification effectively restores the oil’s insulating capacity and supports optimal transformer performance. Analysis confirms that the transformer oil remains suitable for use, and routine annual purification is recommended to maintain reliability, efficiency, and operational performance. This study highlights oil purification as an effective preventive measure for transformer stability, extending operational life, and reducing the risk of insulation failure. The findings provide valuable guidance for transformer maintenance in the electricity industry, ensuring safe and optimal long-term operation.

This study aims to analyze and improve the quality control of packaging printing production at PT XYZ by applying the Lean Six Sigma methodology. Lean Six Sigma combines waste reduction principles with statistical quality control to achieve operational excellence. The research identifies major production challenges, including high defect rates, overproduction, and ineffective inventory management, all of which contribute to increased costs and reduced efficiency. The DMAIC framework—Define, Measure, Analyze, Improve, and Control—was employed to systematically address these issues. During the Define and Measure phases, types of waste were identified and defect rates were calculated using Defects Per Million Opportunities (DPMO). In the Analyze phase, root causes were examined using a Fishbone (Ishikawa) diagram, highlighting factors related to manpower, materials, machines, and methods. The analysis revealed a sigma level of 3.2, indicating a moderate level of process quality and substantial room for improvement. To address the identified issues, several improvement strategies were proposed. These include implementing a Just-In-Time (JIT) system to reduce inventory waste, adopting the First-In-First-Out (FIFO) inventory method for better material rotation, and conducting targeted technical training for operators to minimize human error. In the Control phase, the study suggests process standardization, routine inspections, and continuous monitoring as key practices to ensure that improvements are sustained over time. The findings and recommendations from this study are expected to contribute to better quality control, reduced operational waste, and enhanced productivity in the packaging printing process. Ultimately, this approach aims to strengthen PT XYZ’s competitiveness in the printing industry by fostering a culture of continuous improvement and quality excellence.

This study analyzes the effectiveness of implementing predictive maintenance (PdM) on the final drive components of the Komatsu PC200-8 unit at PT. Antareja Mahada Makmur, Site PT. Multi Harapan Utama, East Kalimantan, in an effort to reduce downtime and operational losses. Before the implementation of PdM in 2022, there were 12 repair cases for the final drive with a total downtime of 772.1 hours, repair costs amounting to IDR 310.6 million, rental income loss of IDR 208.03 million, and total losses of IDR 518.63 million. In 2023, during the PdM transition phase, the number of cases decreased to 4, with a total loss of IDR 252.05 million, although downtime remained high (714.6 hours) due to the limited scope of PdM implementation on certain units and components. In 2024, with full PdM implementation, the number of repair cases decreased to 5, with total downtime of only 96 hours and losses of IDR 45.75 million. The cost of PdM implementation for the year was only IDR 21.9 million. As of July 2025, no further damage to the final drive has been recorded, demonstrating a significant improvement in equipment reliability. The reduction in total losses from 2022 to 2024 amounted to IDR 472.88 million, indicating PdM’s effectiveness in avoiding significant costs through condition monitoring methods such as oil analysis, magnetic plug rating, thermal inspection, and oil leak testing (floating seal). The findings of this study confirm that PdM is effective in reducing downtime, repair costs, and enhancing asset management in the mining sector. It also improves equipment reliability and overall operational efficiency, proving PdM to be a successful strategy in reducing losses, increasing productivity, and supporting the sustainability of company operations.
