Background: Based on the results of observations made in September 2022-December 2022 in the medical record storage room of the Tk. IV Dr. R. Ismoyo Kendari, there were 427 inpatient medical records and 254 (59%) inpatient medical records that suffered damage caused by physical, chemical and biological factors. The purpose of this study was to determine the causes and types of damage to the medical records of inpatients due to physical, chemical and biological factors in the medical record storage room of the Tk Army Hospital. IVDr. R. Ismoyo Kendari.Methods: This study used a qualitative analytic descriptive design, especially the cross sectional type. The sample in this study consisted of 4 medical record officers, namely 1 Head of Medical Records, 1 Secretary of Medical Records, 1 registration officer and 1 filling officer in the medical record storage room of the Tk Army Hospital. IV Dr.R.Ismoyo Kendari and 254 medical records of inpatients in September 2022 - December 2022 who were damaged. The research instrument used observation, checklists, and interview guidelines. Results: The results of this study indicate that physical factors, chemical factors, and biological factors cause damage caused by negligence of officers, exposure to sunlight, dust, ink, rust, mold, and insects. Conclusion:The conclusion of this study found forms of damage such as torn, detached/missing medical record forms, and yellow to brown in color as a result of physical factors, chemical factors, and biological factors.

Background: Medical records at Udayana Denpasar Level II Hospital that exceed capacity are caused by double medical records, misfiles occur during the implementation of retrieval, the retention process has not run optimally and the increasing number of patient visits. The purpose of this study was to determine the impact of medical record storage overload on the response time of medical record provision. Methods: The research design used is a quantitative method with a cross sectional approach. The data analysis used is the Chi Squere test. With a total sample of 16 medical record officers and medical record documents contained in 30 medical record storage shelves where each shelf consists of 5 sections.   Results: The results of the Chi Squere test show that the p value is 0.002, the value is less than 0.05. So it can be concluded that there is an impact of overload of medical record storage on the response time of providing medical records, thus impacting the quality of hospital services, the services provided become long because they wait for medical records to be delivered to the polyclinic. Based on these results, it is necessary to transfer media by scanning medical records then stored in sharing folders and uploaded to the interface system. This media transfer is expected to provide a solution in dealing with the overload of medical record storage so that it can be more efficient in providing medical records.       Conclusion: The conclusion in this study is that there is an impact of overloaded medical record storage on the response time of medical record provision, which has an impact on the quality of hospital services, the services provided will be long because they wait for medical records to be delivered to the polyclinic, so that doctors experience problems in patient examinations because they cannot see the patient's previous examination history.

Background: Many factors cause stunting, one of which is health problems for young women (mothers-to-be) who are about to become pregnant. In several studies, smoking behavior and exposure to cigarette smoke also have an impact on pregnancy and fetal disorders which result in stunted babies being born. Before becoming pregnant, young women must really prepare their health. This preparation can start when you reach puberty until you are ready to become a prospective bride. The female bride-to-be (catin) is a woman of childbearing age who was in a healthy condition before becoming pregnant so that she can give birth to a normal and healthy baby and the male prospective bride-to-be will be introduced to the reproductive health problems of herself and the partner she will marry. Purpose: The aim of this activity is to help cadres in recruiting catin. Method: This service activity uses counseling and discussion methods for 30 cadres of prospective brides and grooms consisting of the Traditional Village Council, Village Kelian, Village Perbekel, Village PLKB, Village TPPS, and Village Midwife Element TPK. Results: There was an increase in post test scores after being given the material. The mean pre-test score was 68 and rose to 70. There was agreement in selecting potential brides, especially Hindu brides. This flow will be disseminated to each village, and will be socialized at every meeting with teenagers. So that teenagers who will later get married go through this flow. Conclusion: The importance of health screening for prospective brides and grooms to prevent stunting in children in the future. The catin flow is a tool to make it easier to disseminate information about how prospective brides and grooms, especially Hindus, report to Kelian Desa about their wedding plans.

Background Recently, the development of technology is also becoming more advanced. Today's technology is able to bring about a new era where everything becomes fast-paced practicality. The negative impacts of excessive gadget use include gadget addiction, sleep disturbances, eye fatigue, poor posture, impaired close vision, and decreased cognitive abilities. The purpose of this community service is to provide counseling on the impact of gadget use on children at the reading house of the library framework in North Aceh district. Method The implementation of community service is face-to-face, using the counseling method. This activity was carried out on Tuesday, 31 October 2023 at the Rangkang Pustaka Reading House in Nisam village, North Aceh Regency. The counseling was conducted for children aged 7 to 11 years. who are at the elementary school,with a total of 37 participants. The results showed that from a total of 37 children, it was found that 13 already had sufficient understanding of the impact of gadget use, while 24 still did not have knowledge and understanding of the adverse effects that arise from the use of gadgets. In conclusion, gadgets can reduce the ability to think and analyze things, and can have long-term and permanent consequences if not addressed early on. This activity is very helpful for children to become independent and reduce smartphone addiction.

Background: Type 2 diabetes mellitus can affect all aspects of the patient's life so that it can be life threatening if not treated properly. The problems experienced by Type 2 Diabetes Mellitus patients can be minimized by having sufficient knowledge and ability to control their disease with self-care. Self Care describes patient behavior carried out consciously and independently with the aim of optimizing and increasing independence in terms of health. Development of an empowerment program for Type 2 Diabetes Mellitus patients through a cadre approach. Purpose: the aim of this activity is to increase cadres' ability to manage self-care for type 2 diabetes mellitus patients. Method: The method of community service activities is cadre empowerment. Activities were carried out in Sembung Village, Mengwi, Badung with 50 health cadres as partners. Result: The result of this activity is that there has been an increase in understanding by 60% and cadres have increased the ability of type 2 Diabetes Mellitus patients to carry out independent care. Conclusion: The conclusion is that health cadres are able to act as motivators and mentors in carrying out comprehensive self-care for Type 2 Diabetes Mellitus patients.

ABSTRACT The high number of cases of death due to cervical cancer in Indonesia is exacerbated by the arrival of sufferers to health services at an advanced stage of > 70%. Efforts can be made to reduce risk factors for cervical cancer through primary prevention efforts. The aim of community service is to increase teenagers' knowledge and attitudes about cervical cancer, with educational methods through audiovisual media. The level of knowledge before education was mostly sufficient 72 people (72%) and the attitude category was sufficient 70 (70%). After education there was a good increase in knowledge from 28 (26%) to 91 people (91.0%), while attitudes also existed an increase from 5 people with good attitudes to 87 people (87%). The Wilcoxon test results obtained a significant value (p) of 0.000, it can be concluded that cervical cancer prevention education is effective through audio-visual for young women. .It is hoped that health workers will increase educational activities using audio-visual methods about cervical cancer. The public, especially young women, are expected to seek information from print and electronic media about cervical cancer.

Background Stunting is one of the nation's problems that must be addressed immediately. According to the Ministry of Health's Basic Health Research (Riskesdas) data, the national stunting rate has decreased from 37.2% in 2013 to 30.8% in 2018. According to the Indonesian Toddler Nutrition Status Survey in 2019, this rate decreased to 27.7%. Stunting reduction has been declared a national priority program. This refers to the Sustainable Development Goals (SDGs) agenda which consists of 17 goals agreed upon by UN members, where the problem of stunting is one of the targets that must be considered. Purpose This activity aims to help improve the health status of the community, increase community knowledge, awareness and concern about stunting and increase community creativity in stunting prevention efforts through assistance in making nutritious food. Methods Face-to-face and counseling and conducting physical examinations of toddlers in Ulee Blang Manee Village. Results In Ulee Blang Mane Village there are 17 toddlers who are stunted. While the geographical conditions of Ulee Blang Mane village are still relatively close to the city center and also have the potential to grow the community's economy. This potential should be unique to the community in increasing family nutrition consumption so that it can reduce the problem of stunting. the results of the activity show that the Ulee Blang Mane village community has an increased understanding of how to prepare nutritious food to prevent stunting. Conclusion Appropriate and targeted socialization related to feeding toddlers is important to do and a more in-depth study is needed related to feeding by mothers or families including obstacles in its implementation.

Background: Children with disabilities, especially mentally retarded, have difficulty distinguishing between appropriate and inappropriate touches, which will make it difficult for disabled children to identify actions that lead to sexual harassment directed at them. Intellectual disability or mental disability is a condition where a person's intelligence development experiences obstacles so that they do not reach the optimal stage of development. Digital pop up books are very easy to understand in the learning process and are an effective medium in providing sexual education for mentally retarded children. Purpose: The aim of this community service is to increase awareness of mentally retarded teenagers about preventing sexual violence through digital pop up book media. Method: The stages include planning and preparation, implementation of service activities with workshops, Popi Dita fungame, barcode story corner hut and evaluation. The activity was held on Wednesday 7 June 2023 at SLB Samudra Lavender Bangkalan. Measuring the success of this digital pop up book media uses a pre-test and post-test with a questionnaire containing 16 short questions. Result: The results showed an increase in student knowledge scores from 4.95 to 7.36. The paired t-test obtained a p- value of 0.019 which shows that sexual violence prevention interventions can increase the knowledge or awareness of mentally retarded children regarding the prevention of sexual violence.    Conclusion: This activity was successfully carried out with high participation and enthusiasm from students in taking part in this activity. It is hoped that this pop up book can be a useful educational tool for schools in an effort to minimize the number of victims of sexual violence.

Background: Osteoporosis is a common disease that needs to be treated by a health professional promptly. To address this issue, osteoporosis prevention exercise instructors were trained and certified as part of empowerment initiatives. Purpose: The aims of this program was to create competent, nationally certified instructors. Method: The Indonesian Healthy Bones Community Association (PERWATUSI), a non-profit organization whose objective is to ensure that Indonesians have healthy and strong bones, is responsible for carrying out this empowerment program. The event took place on July 19–20, 2023, in a hall at Bali International University. Results: A total of 59 instructors were selected from various sports communities and trained to provide the public on how to prevent early osteoporosis, conduct suitable exercise program, and emphasize safety training. Before the training started, candidates had to have the necessary educational background, professional qualifications, training in osteoporosis preventive activity, and a commitment to continuing learning. All of participants (100%) met the qualification. Conclusion: These instructors are crucial to the fight against osteoporosis. Those who have earned certification are anticipated to be committed to encouraging healthy lives and lowering the risk of bone fractures.

Background: Diabetes Mellitus (DM) is a disease that is increasing every year in countries around the world. Reports from various regions in Indonesia show that the incidence and complications of DM are widespread, making it a national problem that requires more attention. A frequently encountered complication of DM is diabetic foot, which can manifest as ulcers, infection and gangrene. Actions to treat diabetic foot include preventive measures which include foot care education, diabetic shoes and foot exercises. Foot exercises are exercises carried out by DM sufferers and non-sufferers to prevent injuries and help improve blood circulation in the legs. Apart from their role in providing health education, nurses as members of the medical team can also play a role in teaching foot exercises so that DM patients are able to do foot exercises independently. Objective: To increase the knowledge and skills of DM sufferers by implementing foot exercises in an effort to prevent diabetes complications in the feet (diabetic foot). Method: Foot exercise activities are carried out on DM sufferers who do not have wounds on the feet. The methods used were lectures, questions and answers, and direct demonstrations using newspapers. Results: The results of the activities obtained were an increase in knowledge before and after providing counseling and demonstrations of foot exercises for DM sufferers. Conclusion: After carrying out this community service, the community's ability to do foot exercises has increased and can be done independently, so it is hoped that this can prevent complications in DM sufferers.

Hypertension is a condition where there is an increase in systolic blood pressure of 140 mmHg or more and diastolic blood pressure of 90 mmHg or more. Hypertension can be caused by poor diet and lack of physical activity, The purpose of this study was to determine whether there is an influence of low impact aerobic gymnastics on changes in blood pressure in the elderly at Damai Ranomuut Nursing Home in Manado. This study was conducted with a pre post test design where the sample was observed first before being given treatment (pre test) and after being given treatment (post test) the sample was observed again. Respondents were 12 people, data collection was carried out using observation sheets. Furthermore, the data was analysed with the Wilcoxon test with a significance level (α) of 0,05. The results of the study using the T-test statistical test obtained pre-post results with a ρ value = 0,000 where the ρ value < α = 0,05. The results of this study indicate that there is an influence of low impact aerobic gymnastics on changes in blood pressure in the elderly at Damai Ranomuut Nursing Home. The conclusion of this study is that there is an influence of low impact aerobic gymnastics on changes in blood pressure in the elderly at Damai Ranomuut Nursing Home in Manado. Suggestions for the elderly at Damai Ranomuut Nursing Home to increase knowledge and awareness about the importance of maintaining health in order to avoid high blood pressure disease.

Mangroves have high ecological and economic value for local communities, as spawning grounds, nursery grounds, feeding grounds for marine life, and act as protection against abrasion. Protected mangrove forests provide many choices for coastal communities to meet economic needs. However, the activities and exploitation of humans that increase from time to time can threaten, destroy and even reduce the stability of the life, quantity and quality of mangrove forests. This study aims to determine the physical and chemical parameters of the environment, the species composition and structure of the mangrove forest community in Paradiso Beach, Oesapa Barat Village, Kelapa Lima District, Kupang City. This research is quantitative descriptive. This research is located at Paradiso Beach, Oesapa Barat Village, Kelapa Lima District, Kupang City. Methods of data collection by making observations and recording the results of observations and interviews in the field, analysis in the laboratory and documentation. Data analysis begins with data collection, data reduction, data presentation and drawing conclusions. The results showed that station I had parameters of temperature (270C), pH (8.08), salinity (20.5‰), DO (6.42mg/l) and station II had parameters of temperature (280C), pH (7.99), salinity (21.7‰), DO (7.43 mg/l), the parameters of the two points of this station meet the environmental quality standards for mangrove forest communities. The composition of mangrove forest types at the two station points only consisted of 3 families with 3 mangrove species, namely Sonnetaria alba, Rhizophora apiculata, and Avicennia alba. Community structures of the mangrove forest at station I and station II were categorized as damaged with rare criteria at the tree level, sapling level and seedling level. Importance Value Index (INP) of station I (258%) and station II (222.1%), this shows that Sonneratia alba has a higher level of spatial tenure in mangrove forest communities, diversity index (H') at station I (0.64) and station II (0.71), this shows that the mangrove forest community has environmental stability. and the uniformity index (J') of station I (0.68) and station II (0.76), this shows an unbalanced distribution pattern between tree species in mangrove forest communities, and dominance index (D) of station I (0,61) and Station II (0.51), this shows the concentration and distribution of the dominant Sonneratia alba species in the mangrove forest community of Paradiso Beach, West Oesapa Village, Kelapa Lima District, Kupang City.

Community service teaches to lecturers and trainers at the Training and Education Center of Muhammadiyah University of Malang (Pusdiklat UMM) plays a crucial role in enhancing the qualifications of lecturers and administrative staff to meet the established standards of human resources both nationally and internationally, in line with the dynamics of global human resource competencies. In this context, Smart Indonesia Academy, a management company, provides competency assessor training at Pusdiklat UMM as a community empowerment effort. This research adopts an analytical descriptive method to collect, analyze, and interpret data. The research findings indicate the smooth implementation of community service activities by Smart Indonesia Academy at the Pusdiklat UMM office. In conclusion, this study highlights the complexity and challenges of implementing competency-based training and education approaches in the workplace and higher education, emphasizing the importance of in-depth understanding and problem-solving efforts to enhance implementation effectiveness.

Medan Tuntungan Sub-district is one of the strategic areas in Medan City that has great potential in the development of the tourism sector. This research aims to identify and analyze the characteristics of tourism architecture and the attractiveness of tourist destinations in the area. The method used is descriptive qualitative with data collection techniques through literature study and observation of several leading tourist attractions, including Kenanga Garden, Royal Sumatra Golf Course, Rajasyah Outbound & Edu Center, and Graha Maria Annai Velangkanni. The results showed that each destination has its own charm in terms of attractions, accessibility, amenities, and ancillary facilities. The existence of adequate facilities and a natural atmosphere are the added value of these destinations. However, some locations still need improvement in terms of infrastructure and promotion. Therefore, collaboration between the government, tourism managers, and the community is needed to support the development of sustainable tourism in Medan Tuntungan Sub-district.

This study aims to analyze the influence of per capita income, education level, the productive-age population, and the human development index on poverty levels in Indonesian provinces. The data used in this study are secondary data obtained from the Indonesian Central Bureau of Statistics and the World Bank. The analysis method used is multiple linear regression. The results of the study show that per capita income, education level, and the human development index have a significant influence on poverty levels in Indonesian provinces. However, the number of productive-age population does not have a significant influence on poverty. These findings have important policy implications in efforts to reduce poverty levels in Indonesian provinces.

Chest X-ray report generation has become an important topic in vision-to-language research. However, fully generative models often create fluent clinical reports that may contain unsupported or inaccurate statements, leading to hallucination problems and reducing reliability. This study investigates evidence-grounded report generation using the Open-i (IU X-ray) dataset with two main goals: generating coherent radiology reports from X-ray images and minimizing unsupported clinical entities through evidence retrieval. Four experimental models were evaluated: a baseline image-to-report model (E1), an alignment-enhanced model using the InfoNCE objective (E2), a retrieval-grounded model that incorporates Top-K evidence sentences with citation markers such as [E#] (E3), and a reranking model that selects the most evidence-supported output (E4). Experimental results on the Open-i test dataset show that grounding methods significantly reduce hallucination rates and improve entity overlap performance. The reranking approach achieves the best grounding quality, although stronger grounding slightly lowers text-overlap scores and increases inference time. Overall, retrieval-based grounding with explicit citations and reranking offers an effective approach for improving factual consistency and reducing unsupported information in automated radiology report generation.

Brushing teeth properly and correctly is an important factor for maintaining oral health. The success of maintaining oral and dental health is also influenced by the pattern of brushing teeth. The pattern of brushing teeth includes the technique of brushing teeth, the time and frequency of brushing teeth. The problem in this study is poor oral and dental hygiene in elementary school children (46.4%). Dental and oral hygiene is an important factor for dental and oral health to be free from disease, therefore dental and oral hygiene must be maintained and maintained in order to create optimal health. Objective: Knowing the relationship of brushing patterns with dental and oral hygiene in elementary school children. Method:. Collecting data on brushing patterns by filling out a checklist on the observation sheet and interview sheet. Collecting dental and oral hygiene data using an examination sheet. Respondents in this study amounted to 110 students, who were taken by simple random sampling. The data analysis technique used was the Chi-square test. Results: The pattern of brushing teeth in the poor category is 89 respondents (80.9%), dental and oral hygiene is in the bad category, namely 51 respondents (46.4%). The results of the Chi-square test are P value 0.002.Conclusion: That there is a relationship between the pattern of brushing teeth with dental and oral hygiene in elementary school children.

Abortion is the termination of a pregnancy before 20 weeks of age and can also occur at 12 weeks of gestation or when the fetus weighs less than 500 grams. According to the World Health Organization (WHO), the incidence of abortion in Indonesia is the highest among other Southeast Asian countries, namely 2 million out of 4.2 million people. Incomplete abortion, that is, only part of the conception is removed, what remains is the decidua or placenta. Initially there is bleeding in the decidua basalis, followed by necrosis of the surrounding tissue, then part or all of the products of conception are released. Other complications of incomplete abortion are infection, perforation, and shock. The purpose of this study was to provide nursing care for patients with incomplete abortion in the couch room at Aisyiyah Pariaman Hospital. The research used a descriptive approach in the form of a case study to find out the problem of nursing care in patients with incomplete abortions in the couch room of Aisyiyah Pariaman Hospital with one patient. This research was conducted on 20-24 February 2023, The results of the study after the study found 3 diagnoses, namely: Anxiety related to situational crises,Riskfor hypovolemia related to active fluid lossand Acute pain related to physiological injury,. During the 5 days of the study the patient went home with thesuggestion that the patient could carry out home care and control according to the recommendations from the hospital.
