Background: Comprehensive midwifery care is essential for early detection of complications and reducing maternal and infant mortality rates. In Sleman Regency, despite high access to health services, continuous monitoring remains crucial for ensuring maternal and fetal safety. Objective: To provide comprehensive midwifery care for Mrs. M at Klinik Pratama Shaqi, Sleman, from the third trimester of pregnancy through the postpartum and neonatal periods. Methods: This is a descriptive qualitative study using a case study approach based on Varney's seven-step midwifery management and SOAP documentation. Findings: During pregnancy (37 weeks 6 days), the mother experienced physiological complaints of back pain and shortness of breath, managed through posture education and pregnancy exercises. Labor proceeded normally and spontaneously on October 6, 2024, utilizing "gentle birth" practices such as endorphin massage to reduce pain. A second-degree perineal tear occurred and was treated according to procedure. The baby was born healthy, weighing 3100 grams, with successful Early Initiation of Breastfeeding (EIB). During the postpartum and neonatal periods, uterine involution was normal, breast milk production was adequate, and the infant's growth met standards with no danger signs. Implications: Continuous assistance improves the mother's physical and psychological readiness and ensures a healthy neonatal transition.

Low back pain is one of the most common complaints experienced by pregnant women, particularly in the third trimester, as a result of various physiological, hormonal, and biomechanical changes that occur during pregnancy. Changes in posture, weight gain, and shifts in the center of gravity contribute to the development of this condition. This study aims to examine various non-pharmacological interventions that can be used to reduce low back pain in pregnant women and to analyze the role of midwives in implementing these interventions in midwifery practice. The method employed is a narrative literature review synthesizing ten verified and relevant studies, particularly those discussing prenatal yoga and pregnancy exercise as forms of non-pharmacological intervention. The results indicate that the implementation of prenatal yoga and pregnancy exercise is effective in reducing the intensity of low back pain, improving muscle flexibility and strength, and providing physical and psychological relaxation for pregnant women. In addition, these interventions contribute to increased comfort and improved quality of life during pregnancy. The findings of this study support the importance of evidence-based midwifery practice and highlight the active role of midwives in providing education, assistance, and empowerment for pregnant women through the application of safe and effective non-pharmacological interventions.

Significant physical and psychological changes occur throughout pregnancy, especially in the third trimester (weeks 27–40).  Lower back pain, frequently caused by poor posture, changes in the curvature of the spine, and increasing body weight that shifts the center of gravity forward, is a common issue during this time.  Between 30% and 78% of pregnant women report experiencing lower back discomfort.  Maintaining physical health, endurance, and mental preparedness for childbirth are key objectives of prenatal exercise.  This study aimed to examine the effect of pregnancy exercise on lower back discomfort in third-trimester pregnant women at the Likupang Community Health Center.  The study used a one-group pretest-posttest design and was pre-experimental in nature, with the Likupang Community Health Center serving as the research site.  Fifteen pregnant women from the Likupang Community Health Center were chosen as the study sample.  The results indicated that pregnancy exercise had a significant impact on reducing back pain (p-value 0. 000), showing a clear effect between pregnancy exercise and the reduction of lower back pain (p-value 0. 000), as confirmed by the bivariate statistical test.  The findings suggest that pregnancy exercise can effectively alleviate lower back discomfort in third-trimester pregnant women.

Maternal health is a measure of the level of social welfare. Maternal death is the death of a woman during pregnancy, childbirth or within 42 days after the end of pregnancy. Perineal rupture is the second most common cause of postpartum bleeding. Tearing may coincide with uterine atony. Based on data from the World Health Organization (WHO), 2.7 million cases of perineal rupture occurred in mothers giving birth. This figure is estimated to reach 6.3 million in 2050. One way to prevent perineal rupture is to maintain the elasticity of the perineum. Increasing the elasticity of the perineum is done by means of pregnancy exercises. The aim of this study was to determine the relationship between pregnancy exercise and perineal rupture in postpartum mothers at Anemia Of Pregnant Women At BPM Helena Astuti Sinaga. The research design is an analytical study with a cross-sectional approach. The population in this study was all mothers giving birth at Anemia Of Pregnant Women At BPM Helena Astuti Sinaga in November-March 2023, namely 764 people with a research sample of 88 people. The sampling technique used random sampling and research analysis used the chi square test. The research results showed that the majority of pregnant women who took part in pregnancy exercise at BPM were 47 (53.4%) and those who did not do pregnancy exercise were 41 (46.6%). Inpartum mothers who experienced perineal rupture were 42 (47.3%) and those who did not experience perineal rupture were 46 (52.3%). Based on the results of statistical tests between pregnancy exercise and perineal rupture, the p value = 0.003 (p < 0.05). The conclusion of this study is that there is a significant relationship between pregnancy exercise and perineal rupture during birth at BPM Helena Astuti Sinaga. So it was suggested to Anemia Of Pregnant Women At BPM Helena Astuti Sinaga to improve services, especially for pregnant women, to do pregnancy exercises.

The sampling technique uses the method proportional samplingas many as 101 people. Data analysis uses univariate and bivariate analysis with tests chi square and multivariate with logistic regression. The results of research using logistic regression are known valuesp-value Logistic regression of educational variables 0.808>0.05, knowledge 0.021<0.05, family support 0.000<0.05 and motivation 0.000<0.05. It is recommended that pregnant women actively participate in outreach programs to increase their knowledge and carry out pregnancy checks and participate in pregnancy exercises.

Back pain during pregnancy peaks at week 24 to week 28, when abdominal enlargement reaches its maximum. Pregnancy and childbirth are natural processes that can cause pain. However, many women feel the pain more severely than they should because they are affected by panic and stress. This is known as the fear-tension-pain concept, where fear triggers tension or panic which causes muscles to stiffen and ultimately increases pain. Overcoming this discomfort requires non-conventional therapy, one of which is pregnant gymnastics. Based on this, the community service team conducted the implementation of pregnant exercises for third-trimester pregnant women with the aim that pregnant women can do pregnant exercises independently at home and reduce complaints of back pain they experience. This training method is carried out using lectures and demonstrations of pregnant exercises. Implementation was carried out 1x / once a week for 1 month at Puskesmas Lembang Lego Village, Lembang Kec. Lembang, Polman Regency in January 2021 which 20 pregnant women attended. The care method used is qualitative with case research (CSR) by collecting pre- test and post-test data with pain scale observation using Numeric Rating Scale (NRS) measurement. Physical training of pregnant women to prepare for their labor through pregnancy exercises is considered effective as a preventive measure for pregnant women in preparing for their labor. It can reduce the intensity of back pain in Trimester III pregnancy.
