Organizational commitment is essential for maintaining hospital performance, service quality, and healthcare continuity. Preliminary data from Brebes Regional General Hospital shows that 35% of medical doctors face issues with low organizational commitment, which may negatively impact the institution's effectiveness. This study examines how meritocracy strengthens the effects of transformational leadership and work motivation on medical personnel's organizational commitment. A quantitative approach with causal research design was used to analyze the relationships among these variables. The sample included 72 doctors, selected through purposive sampling. Data were collected via structured questionnaires and analyzed using Moderated Regression Analysis (MRA) to assess both direct and moderating effects. The findings show that transformational leadership, work motivation, and meritocracy together significantly influence organizational commitment. Transformational leadership and work motivation each have a positive impact on organizational commitment. Additionally, meritocracy serves as a moderating factor, enhancing the connection between leadership and commitment, as well as work motivation and commitment. These results suggest that meritocratic systems based on competence, performance, and achievement improve leadership effectiveness and motivation, fostering greater commitment among medical staff. The study suggests that hospital management should implement meritocratic principles consistently in leadership and human resource practices, alongside developing transformational leadership and motivation strategies, to maintain high levels of organizational commitment among medical personnel.

Sleep disturbances are a common complaint among perimenopausal women due to hormonal, psychological, and physiological changes, which impact their health and quality of life. Safe and easy-to-apply non-pharmacological approaches, such as lemongrass aromatherapy (Cymbopogon citratus), have the potential to be a supportive alternative in overcoming sleep disturbances during this phase. However, empirical evidence regarding the effect of lemongrass aromatherapy on the sleep quality of perimenopausal women is still limited, especially in the context of primary health care in Indonesia. This study aims to analyze the effect of lemongrass aromatherapy on the sleep quality of perimenopausal women. This study used a quantitative approach with a one-group pretest–posttest pre-experimental design. The sample consisted of 30 perimenopausal women selected using total sampling technique in the working area of Pustu Paring Lahung UPT Tumpung Laung. The intervention in the form of lemongrass aromatherapy through inhalation was carried out regularly before bedtime. Sleep quality was measured before and after the intervention using a standardized questionnaire. Data analysis was performed descriptively and bivariately using Fisher's Exact Test. Descriptively, there was an improvement in sleep quality after the administration of lemongrass aromatherapy, marked by a decrease in the proportion of poor sleep quality and an increase in good sleep quality. However, the statistical test results showed that the relationship between sleep quality before and after treatment was not statistically significant (p = 0.515). Lemongrass aromatherapy showed a tendency toward clinically significant improvement in sleep quality, but this was not statistically significant. Further research with a stronger design and longer intervention duration is needed to confirm the effectiveness of lemongrass aromatherapy on the sleep quality of perimenopausal women.

This study aims to evaluate the implementation of Sidoarjo Regent Regulation Number 69 of 2017 concerning the Civil Servant Code of Ethics in the Family Planning Sector at the Sidoarjo Regency Women's Empowerment, Child Protection, and Family Planning Office. A qualitative approach with a case study was used to explore the implementation of the policy through interviews, observation, and documentation. The analysis was conducted using William Dunn's six policy evaluation indicators, namely effectiveness, efficiency, adequacy, equity, responsiveness, and accuracy. The results show that the policy has provided clear behavioral guidelines and is applied evenly in the work environment. However, the effectiveness and efficiency of implementation are not optimal due to disciplinary violations, weak supervision, and unstructured communication between superiors. The aspects of adequacy and accuracy are considered relevant to the needs of the organization, but have not been able to fully overcome obstacles such as high workloads and low internalization of ethical values among employees. In general, this policy contributes to shaping the professionalism of civil servants, but still requires strengthening through continuous supervision and more systematic coaching. The originality of the study lies in the use of Dunn's evaluation model in the context of the implementation of the civil servant code of ethics at the regional level, as well as identifying gaps between normative policies and field practices. These findings confirm that the success of a code of ethics depends heavily on organizational communication, work culture, and consistency of oversight.
