Hospital medical record is an important component in the implementation of hospital management activities, hospital medical records must be able to provide complete information about the medical and health services process at the hospital, both in the past, present and future estimates of what will happen. The purpose of the medical record is to support the achievement of orderly administration in order to improve health services. Without the support of a good medical record management system, then the orderly administration will not succeed. Medical records arevery supportive not only to organize important instrument processes in handling medical legal problems that may occur in hospitals less successfully in aneffort to improve the quality of service as expected. The most important thing in the medical record is the numbering of medical record documents. One type of medical record numbering used is the Numbering System Unit (UNS) is a numbering system where the system provides one number to outpatients, inpatient and emergency department. Each visiting patient receives one number when the patient first arrives to the hospital and is used forever on subsequent visits. The patient's medical record number is unlikely to be used for the number of another patient, since the numbering is already organized and will be continuous with the filling of the medical record document and in the process of handling the patient. Based on research conducted in Main Clinic of Inpatient PKU Muhammadiyah Kutoarjo in giving medical record number Unit Numbering System (UNS) have been run well and effective start from place of registration of patient, filling of medical record by doctor, giving medicine to patient, with ease in searching, storing, and maintaining patient's medical record but still experiencing the problem of duplicate nu mbering at the time of patient registration where the patient got double number, although not nearly every day found the existence of double patient medical record number.

Load imbalance in three-phase distribution transformers can increase neutral current and power losses, thereby reducing the efficiency and reliability of electric power distribution systems. This study aims to analyze the level of load imbalance, power losses caused by neutral current, and the effectiveness of manual load balancing using the two-time-point method, namely peak load time (PLT) and off-peak load time (OPLT). The research was conducted on the PNIAI007 distribution transformer at PT PLN (Persero) ULP Enarotali using a quantitative descriptive approach with a case study design. Data were obtained through measurements of phase currents and neutral current, which were then analyzed to compare conditions before and after load balancing. The results show that the load imbalance level was significantly reduced from more than 29% to 2.47% during daytime operation and 1.21% during nighttime operation. The neutral current decreased from 37.81 A to 14.58 A during the daytime and from 51.76 A to 19.83 A at night. In addition, power losses due to neutral current were reduced by more than 85%. These results indicate that the two-time-point load balancing method is effective in improving the efficiency and operational reliability of distribution transformers.

This study aims to evaluate the effectiveness of digital transformation in the 5.0 era in transforming latent cash waqf assets into productive capital capable of sustainably driving the community’s economy. The study employed a qualitative approach using a literature review method. Data were obtained through literature searches and official documents from the Indonesian Waqf Board, then analyzed descriptively to obtain comprehensive conclusions regarding the effectiveness of digital systems in cash waqf management. The results indicate that financial technology (fintech) such as QRIS, crowdfunding, Islamic banking applications, and digital wallets (e-wallets) are capable of overcoming conventional bureaucratic barriers through the concept of “micro waqf,” which is accessible to all levels of society. Based on the Technology Acceptance Model (TAM) theory, ease of digital access has been proven to increase public interest in waqf because it reduces administrative barriers and concerns regarding transaction amounts. In addition, real-time data-based reporting systems also improve transparency and accountability in waqf management. Therefore, the digitalization of cash waqf in the 5.0 era serves as a form of social engineering in collecting collective funds to sustainably support sharia-based productive sectors.

Sunlight visible color of white light is composed of light color% mixture of light colors red (R), % lightcolor green (G) and % color blue light (B) is called a basic light color mixture (R+G+ B). Sunlight shining on the surface of the solar cell PV cells convert solar cell produces a voltage at the output referred voltage Voc. In this research, the irradiation treatment surface of each of three samples of the same type solar cells with light colors red, green and blue alternately. Samples used solar cells have the characteristics of the open circuit voltage Voc parameters = 6 volts dc reference voltage Voc expressed as (ref). Each of the three R-G-B light color lamp is connected to a variable dc voltage source in volts. Electrical power R-G-B lights shining on the solar cell is set as large, on PL(R) = PL(G) = PL(B) = 2.27 watts. Radiation treatment each R-G-B light onto the surface of the three samples of solar cells with a distance = 7 cm, the measurement results obtained voltage Voc(R) [average] = 2.91 volts, Voc(G) [average] = 2 , 40 volts, and Voc(B) [average] = 3.02 volts at the output of the solar cell. The effectiveness of PV cells (photovoltaic) is obtained from the comparison of Voc(R) [average-2], Voc(G) [average] and Voc(B) [average] against a reference voltage Voc = 6 volts. Solar cell conversion results obtained irradiating light of each color of R-G-B generate % red light (R) = 34.94 %, % green light (G) = 28.8 % and the % of blue light (B) = 36.2 %. The effectiveness of PV cells to convert solar cell material believed color red, green and blue is expressed by the parameter PV(R), PV(G) and PV(B). Thus obtained PV(R) = 34.9 %, and PV(G) = 28.8 % and further PV(B) = 36.2 %.
