Pressure ulcers are a common complication in patients with impaired consciousness due to prolonged immobilization. This condition can increase morbidity and prolong the length of stay, thus requiring effective preventive interventions such as progressive mobilization. To determine the outcomes of implementing progressive mobilization on the incidence of pressure ulcers in patients with impaired consciousness at the Anggrek 2 High Care Unit (HCU) of Dr. Moewardi General Hospital. This study employed a case study design involving two participants with impaired consciousness. The intervention, consisting of progressive mobilization, was conducted over 3 days with monitoring of skin condition and pressure ulcer risk. Findings: Following the implementation of progressive mobilization, both participants showed improvement in skin condition and a reduction in the risk of pressure ulcers, as evidenced by the absence of progression to more severe wounds and an increase in skin integrity. The application of progressive mobilization is effective in preventing and reducing the risk of pressure ulcers in patients with impaired consciousness. This intervention can serve as a safe and planned nursing intervention in the intensive care unit.

Diabetic foot ulcer is a disease of the feet of DM sufferers with the characteristics of sensory, motor, autonomic neuropathy as well as macrovascular and microvascular disorders. Diabetic foot injuries are one of the complications that DM sufferers fear because they can result in amputation. Modern Dressing wound treatment with the Moist Wound Healing method is effective in healing ulcer wounds in DM patients. The wound healing method aims to maintain the moisture of the wound by using a moisture-retaining bandage so that wound healing and tissue growth can occur naturally. To determine nursing care in dm patients with leg injuries the application of Modern Dressing wound care using metcovazin and foam in diabetic ulcer patients. This study presents a descriptive study describing the treatment of ulcer wounds in DM patients with Modern Dressing wound treatment using a case study approach. Based on the results of the application of Evidence Based Practice (EBP) dapart, it is concluded that the application of Modern Dressing wound care is more effective than conventional wound treatment. as evidenced by the change in the decreased score in Mrs. N with 3 intervention processes, namely by decreasing the initial score of 30 down to 22 after Modern Dressing wound treatment. Modern dressing is more effective for diabetic foot ulcer care because it accelerates new tissue growth, reduces pain, and lowers the risk of infection.

Background: Stroke is a medical condition characterized by a disruption in blood flow to the brain, most commonly resulting in ischemic stroke, which accounts for 70%–85% of all stroke cases. Prolonged immobility in stroke patients increases the risk of pressure ulcers. Effective prevention strategies Include regular repositioning and meticulous skin care. Effleurage massage, when combined with Virgin Coconut Oil (VCO), enhances blood circulation and skin hydration, thereby potentially reducing the risk of impaired skin integrity. Objective: To analyze the management of a non-hemorrhagic stroke patient Through the application of Effleurage Massage therapy using Virgin Coconut Oil (VCO) to reduce the risk of skin integrity impairment. Methods: This case study involved a non-hemorrhagic stroke patient who was observed during hospital visits over a three-day period. Effleurage massage using VCO was applied twice daily—once in the morning and once at night. Skin integrity was assessed before and after the intervention using the Braden Scale Results: A decrease in the risk of skin integrity impairment was observed following the intervention, as evidenced by improved Braden Scale scores. Conclusion: Effleurage Massage using Virgin Coconut Oil may serve as an effective complementary therapy to reduce the risk of skin integrity Impairment in non-hemorrhagic stroke patients.

Stroke patients often experience mobility impairments, which increase the risk of developing pressure ulcers (decubitus). Pressure ulcers are a common complication for patients with limited mobility, especially in the ICU. One intervention that can be used to reduce the risk of pressure ulcers is back massage using olive oil. This study aims to analyze the effect of back massage using olive oil on reducing the risk of pressure ulcers in stroke patients in the ICU at Sultan Agung Hospital. This study used a pre-experimental design with a one-group pretest-posttest approach, involving 17 respondents who received back massage using olive oil. The results showed that most respondents were aged 56-65 years (35.3%), the majority were male (70.6%), with comorbidities such as diabetes mellitus (52.9%), and the length of hospitalization was 3-5 days. Before the intervention, most respondents were categorized as being at moderate risk based on the Braden scale; however, after the intervention, the majority were no longer at risk of pressure ulcers. The Wilcoxon test showed a p-value of 0.001, indicating that back massage using olive oil had a significant effect on reducing the risk of pressure ulcers. In conclusion, back massage using olive oil is effective in reducing the risk of pressure ulcers in stroke patients in the ICU. Therefore, this technique can be used as an alternative intervention to prevent the occurrence of pressure ulcers in stroke patients.

Diabetes mellitus is a chronic disease of insulin metabolism which is characterized by glucose levels exceeding normal values.  Diabetic ulcers are one of the causes of the risk of infection if not treated properly and correctly.  Factors that cause diabetic ulcers are poor blood circulation, age, gender, lack of physical activity, and an increase in diabetes mellitus. This application aims to determine the effect of foot exercises on the risk level of diabetic ulcers in people with diabetes mellitus. Descriptive by conducting an application to 2 respondents 3 times in 1 week with an interval of 2 days.  Respondents measured the highest systolic blood pressure between the right and left brachials and the right and left ankles which was then divided to obtain the Ankle Brachial Index (ABI) value before and after carrying out diabetic foot exercises. After implementing it 3 times, the result was that the ABI value for both respondents increased, namely by the difference between Mrs.  Sa 0.25 mmHg and Mrs.  Si 0.30 mmHg. It was found that there was an increase in the Ankle Brachial Index (ABI) value before and after carrying out diabetic foot exercises in both respondents.

Bed rest is one of the factors that can cause skin integrity disorders. Bed rest for a long time can cause physiological changes in the patient's body system, one of which can cause the appearance of decubitus in the integument system. The treatment technique that can be given is by giving Effleurage Massage with Virgin Coconut Oil. To find out the results of the implementation of Effleurage Massage with Virgin Coconut Oil on the Risk of Decubitus in Bed Rest Patients in the Kartasura Health Center Working Area. This research used a descriptive design on 2 (two) male respondents aged 63 and 78 years which was conducted from 4 June - 6 June 2024 in Singopuran Village, Kartasura, Sukoharjo with each respondent being given massage intervention for 3 consecutive days. join in. The instrument used is the Braden Scale to measure the level of skin integrity score. There was an increase in the Braden scale score between before and after application to Mr. S and Mr. N. The application of Effleurage Massage with Virgin Coconut Oil is effective in increasing the skin integrity score in bed rest sufferers who are at risk of pressure ulcers in Singopuran Village, Kartasura, Sukoharjo.

Background: Impaired mobility in stroke patients often causes excessive pressure on the skin, increasing the risk of pressure ulcers. If left untreated, decubitus wounds can get worse and lead to infection. Based on evidence, regular right-left tilt mobilization is a potential intervention to prevent pressure ulcers. Objective: Analyze the application of right-slant-left-slant mobilization to prevent pressure ulcers in stroke patients at Ibu Fatmawati Soekarno Regional Hospital, Surakarta City. Method: Descriptive research design with a case study approach, carried out every 3 hours on 2 respondents to explore the application of right oblique-left oblique mobilization in preventing pressure ulcers in stroke patients. Results: Before implementing the right oblique-left oblique mobilization every 3 hours the respondent was at moderate risk with a score of Tn .S 15, Ny.S 16 and pressure ulcers can be prevented. Conclusion: There is an effect of tilting mobilization to the right and left which is indicated by a decrease in the risk of developing pressure ulcers at the Ibu Fatmawati Soekarno Regional Hospital, Surakarta City.

Decubitus wounds are wounds caused by prolonged pressure on a prominent surface area of ​​the bone. Nurses play an important role in preventing the incidence of decubitus, especially by carrying out an early assessment of the risk of decubitus, namely by using the Braden Scale. Objective: to determine the risk of decubitus injuries using the Braden Scale. Method: This type of research is quantitative research with a descriptive research design. The sample in this study was 20 people using a non-probability accidental sampling. Inclusion criteria for this research were patients treated in hospital. Islam Malahayati Medan for more than 2 days of treatment, young elderly, middle elderly and old elderly patients. Data analysis in this study used SPSS 25 for Windows for descriptive statistics. Results: There were 7 people (35%) in the no risk category, 6 people (30%) in the mild risk category, 2 people (10%) in the moderate risk category and 4 people (20%) in the high risk category and 1 person (5%) severe risk category. Suggestion: Hospitals need to include the Braden Scale assessment format in the inpatient assessment format to detect the risk of decubitus injuries and carry out assessments on patients every day while the patient is hospitalized.

The background of this research is based on a brain disease, namely stroke. Stroke results in much higher mortality and morbidity. Viewed from the aspects of risk of death, high medical costs, decreased quality of life and increased prevalence, stroke has a broad impact. Apart from that, decubitus problems also arise in stroke sufferers due to prolonged pressure, the parts that experience bruising are the hips, buttocks, foot joints and heels. Based on a preliminary study conducted at the Tangerang Regency General Hospital, there were 89 patients with a diagnosis of stroke starting from 2022. Meanwhile, in the Cempaka room, it was found that during the assessment there was 1 case with a diagnosis of stroke on January 2 2024. The results of this research are effectiveness. right and left tilt positions in reducing pressure sores or decubitus in patients with stroke. The conclusion of this study is that the right and left side positions are non-pharmacological techniques for reducing pressure ulcers or decubitus and can be used in patients who experience problems with prolonged bed rest. The suggestion from this research is that intervention in right and left tilt positions can be carried out according to the 2021 PPNI SOP.

Bed rest is a condition when a person cannot move freely due to conditions that interfere with movement (activity). Pressure sores are one of the biggest impacts of bed rest. One of the interventions to prevent pressure sores is giving Virgin Coconut Oil (VCO) through massage. Aims; Knowing the results of the implementation of giving Virgin Coconut Oil (VCO) through Massage on the prevention of pressure sores on bed rest patients in the ICU Room of PKU Muhammadiyah Karanganyar Hospital. Method; This type of research is descriptive in the form of a case study. The approach used is the nursing care approach: planning, implementation, and evaluation. Respondents in this study amounted to two patients with a risk of pressure sores. The instrument uses an observation sheet and a Braden scale measurement questionnaire. Results; The results of the implementation showed that the Braden scale rose for both respondents for the first respondent, the Braden scale became 11, and for the second respondent, it became 15. Conclusion; There are differences in the Braden Scale in patients at risk of pressure sores after giving Virgin Coconut Oil (VCO) through Massage at PKU Muhammadiyah Karanganyar Hospital.

Pressure  ulcer is local injury of skin of subtexture it, generally at bump of bone, the effect of pressure or pressure combination with friction or seraping. To preventing the risk of pressure ulcer at total bedrest patient need to treatmen of skin by Nigella Sativa Oil. The purpose of this case study is to be analyze of application by prevention of pressure ulcer pressure sores method at total bedrest patient. The type of this research is descriptive with method of the case study. The subject of this research use two patient who is sustain of total bedrest more than 3 eith enteria for assesment result by Norton 14 scale less than 14 which it is categorized risk of everage pressure ulcer. The result of this research can be conclude at subject I from 10 Norton scale to 13 scale, at subject II from 13 Norton scale to 16 scale. So, at the time from it review with high severe of injury to use the Design from every the category that obtains the result 0 which it means not happens pressure ulcer injury by both of subject. The nurse must be applies by Nigella Sativa Oil therapy at total bedrest patient to can prevent of pressure ulcer risk.
