Childbirth is a series of natural processes that trigger various changes in the mother's body so that she can deliver her baby through the birth canal. Perineal stiffness can actually be anticipated and prevented early in pregnancy, one way is through physical exercises such as pregnancy yoga. Pregnancy yoga has many benefits, especially in alleviating complaints during pregnancy, reducing anxiety, improving sleep quality, and preparing the physical condition before childbirth by strengthening and increasing the elasticity of the pelvic floor muscles, so that the birth process becomes easier. This study aims to analyze the effect of prenatal gentle yoga on the incidence of perineal tears in mothers who will give birth in the work area of the Tiley Health Center on Morotai Island. This study used a quasi-experimental method with a posttest control design. The sample used in the study consisted of mothers giving birth who met certain inclusion criteria, with a total of 36 pregnant women. The instruments used in this study were the Kegel Exercise SOP observation sheet and the assessment of the level of perineal tears. Data were analyzed using the Mann-Whitney statistical test. The results showed a distribution of respondents regarding Kegel exercises: 18 respondents (50%) performed Kegel exercises and 18 respondents (50%) did not. The highest rates of perineal rupture were grade I and grade II, with 13 respondents (36.1%) each. The resulting p-value was 0.0001 (p < 0.05), indicating a significant effect of Kegel exercises on the rate of perineal rupture in primiparous women.

The postpartum period is a crucial phase for mothers to recover after childbirth. One of the common problems is perineal wounds, caused either by episiotomy or spontaneous rupture, which require proper care to prevent infection and accelerate healing. This case study aimed to provide midwifery care for Mrs. M, 28 years old, P3A0H3, with a second-degree perineal wound at PMB Bd. Silvi Ayu, S.Keb. The SOAP approach was applied through assessment, diagnosis, intervention, implementation, and evaluation over five days (June 12–16, 2025). Interventions included education on perineal hygiene, encouragement of nutritious food intake, light mobilization, and perineal wound care using boiled binahong leaves. The results showed decreased pain, reduced edema, a dry wound, and complete healing on the fifth day, with the REEDA score decreasing from 11 to 0. The discussion emphasized that flavonoids, saponins, and ascorbic acid in binahong leaves contribute to tissue regeneration and faster healing. Limitations of this study included the short monitoring period, limited sample, and reliance on maternal compliance. In conclusion, binahong leaf decoction was found to be effective, practical, and able to enhance maternal independence in wound care. This study is expected to serve as a reference for midwifery practice and as a basis for further research with a wider scope.

The postpartum period is a critical phase with a high risk of complications, particularly perineal wound infection, especially in mothers with premature rupture of membranes (PROM). PROM increases the risk of microbial invasion into the birth canal, making infection prevention essential. Vulva hygiene is a non-pharmacological intervention that can help reduce this risk. This study aimed to evaluate the application of vulva hygiene in reducing the risk of perineal wound infection in postpartum mothers with PROM. This study used a case study design involving four postpartum mothers with PROM treated in the Wijaya Kusuma Ward of RSUD Limpung Batang in 2025. Data were collected through comprehensive nursing assessments, observation of perineal wound conditions, and evaluation of infection risk signs before and after the intervention. The results showed that regular implementation of vulva hygiene led to a reduction in infection risk signs, decreased perineal pain, and improved genital hygiene in all patients. Nursing education and assistance played a crucial role in the success of this intervention. It can be concluded that vulva hygiene is an effective nursing intervention in preventing perineal wound infection in postpartum mothers with PROM.

Anxiety in laboring mothers is a psychological factor that can influence the course of labor, particularly during the second stage, and increase the risk of complications such as perineal rupture. Along with obstetric factors, the quality of labor guidance is crucial in preventing perineal trauma. This study aimed to analyze the relationship between maternal anxiety, labor guidance, and the incidence of perineal rupture at Obi Hospital, South Halmahera. Using an observational analytical design with a cross-sectional approach, the study involved 50 laboring mothers undergoing vaginal delivery, selected through purposive sampling. Data collection took place from February to April 2024, with maternal anxiety measured using a standardized anxiety questionnaire. Labor guidance and perineal rupture were assessed through observation sheets and clinical examinations. Data analysis was conducted univariately and bivariately using the Chi-Square test with a significance level of p < 0.05. The results showed a significant relationship between maternal anxiety and labor guidance (p < 0.05), as well as between maternal anxiety and perineal rupture (p < 0.05). Additionally, labor guidance was significantly associated with the incidence of perineal rupture (p < 0.05). Mothers with higher anxiety levels tended to receive poor labor guidance and had a higher risk of perineal rupture. The study concludes that maternal anxiety significantly impacts labor guidance and perineal rupture, emphasizing the need for holistic care that addresses both psychological and physical aspects of childbirth.

Background: Perineal rupture is a common complication in vaginal delivery that can increase the risk of postpartum hemorrhage, infection, and maternal death. One of the factors that contribute to the occurrence of perineal rupture is the baby's birth weight. Babies with a birth weight ≥4000 grams can cause greater pressure on the mother's birth canal, increasing the possibility of perineal tissue tears. In Indonesia, 75% of mothers who give birth vaginally experience perineal lacerations, and postpartum hemorrhage is still the main cause of 40% of maternal deaths. Purpose: This study aims to determine the relationship between birth weight and the incidence of perineal rupture in normal deliveries at the Independent Midwife Practice of Lusi Marbun, Simalungun Regency in 2022. Method: This study used an observational analytical design with a case-control approach. A sample of 30 mothers giving birth was selected using the total sampling method. Data were analyzed using the Chi-square test to determine the relationship between birth weight and perineal rupture. Results: The study showed that the majority of infants had a birth weight <4000 grams (63.3%), and the majority of mothers experienced second-degree perineal rupture (70%). Bivariate analysis showed a significant relationship between birth weight and the incidence of perineal rupture (p-value = 0.000). Conclusion: There is a significant relationship between birth weight and the incidence of perineal rupture. The greater the infant's birth weight, the higher the risk of perineal rupture in the mother during labor. It is important for health workers to detect fetal weight early, provide nutritional education during pregnancy, and optimally prepare for labor to minimize the risk of complications.

The results of the Population Census data carried out in 2020 showed that the maternal mortality rate reached 189 per 100 thousand live births. And based on these figures, Indonesia is ranked second highest in ASEAN, which is further than other countries such as Thailand, Malaysia, Vietnam and Brunei, which have achievements below 100 per 100 thousand live births. The maternal mortality rate is one of the important programs for the Ministry of Health (Kemenkes) to pay attention to. Therefore, through this, the Ministry of Health carries out a care program from before pregnancy, pregnancy, childbirth, babies. And a number of cases of pregnant women giving birth include anemia 48.9%, hypertension 12.7%, KEK 17.3% and complications as much as 28%. The impact of childbirth on maternal mortality is also something that cannot be separated from the occurrence of maternal death. As a health worker with high integrity, it is very necessary to implement Mother and Baby Care. Indicators of quality service are in the process of pregnancy care, postpartum delivery carried out by health workers to the mother. One of them is carried out in the delivery process by explaining to the mother the technique of pushing when the opening is complete or 10cm. Do not do forced pushing (Valsalva) or tell the mother to push forcibly before it is certain that the opening is complete. This is one of the strict prohibitions conveyed by Bergastrom in his book. The process of delivering a baby can generally cause tears in the vagina and perineum. So when leading pushing in a mother in labor, it is necessary to do so as not to cause irregular tears in the birth canal. However, the process of delivering a baby through the birth canal or through normal delivery requires deliberate tearing to widen the birth canal. However, for further action, it is necessary to do hecting or stitches on the episiotomy wound which aims to restore the original shape of the perineal incision scar and stop bleeding as well as healing.

This study is a Systematic Literature Review (SLR) which aims to analyze the relationship between risk factors and the incidence of birth canal laceration and its impact on postpartum hemorrhage. The review of various studies showed that postpartum hemorrhage and perineal rupture have complex and interrelated risk factors, with placental retention, uterine atony, and birth canal laceration as the main causes (p-value <0.05). Maternal characteristics such as age, parity (especially in multiparous with a prevalence of 32.9%), history of previous bleeding (OR=7.725), and pregnancy spacing less than two years (OR=6.943) contributed significantly to the risk of complications. Birth weight had a significant association with the incidence of perineal rupture, while the innovative use of warmpad proved effective with 58.8% of cases showing an intact perineum post-delivery. The study also identified anemia in pregnancy as an important risk factor, with the distribution of causes of postpartum hemorrhage including placental retention (44%), uterine atony (28%), uterine rupture (18.6%), and uterine inversion (9.4%), confirming the importance of comprehensive monitoring during pregnancy to prevent complications.

The postpartum period (puerperium) is the period that begins after 2 hours postpartum and ends when the uterus returns to its pre-pregnancy state, lasting 6 weeks. The postpartum period is an important thing to pay attention to in order to reduce maternal mortality. Perineal rupture is a tear caused by spontaneous birth of a baby, either by using a tool or action. Perineal wound care is important to prevent infection of the reproductive organs, one method of perineal wound care is non-conventional therapy using red betel leaf (Piper crocatum) boiled water which contains flavonoids, alkaloids, polyphenolic compounds, tannins and essential oils which are known to has antibacterial activity. The purpose of this study was to determine the effectiveness of red betel (Piper crocatum) cooking water for perineal wound healing in postpartum mothers. This research is a quantitative study with a population of all postpartum mothers with perineal wounds at PMB Rica Merdekawaty with a total sample of 30 respondents consisting of the intervention group and the control group. The data that was collected was then analyzed using the chi square statistical test (p value = 0.029). Based on the chi square analysis, it showed that red betel leaf boiled water was effective in healing perineal wounds in postpartum mothers.
