Background: Patients with Chronic Renal Failure undergoing Hemodialysis (HD) have various health problems, one of which is skin manifestations. Dry skin is described in the majority of patients with Renal Failure and is suspected to be a significant pathogenic factor in pruritus and may increase itching intensity in CRF patients. It was found that 52% of patients who underwent HD had dry skin (xerosis). Dry skin that appears in CRF patients with hemodialysis is usually caused by atrophy of the sebaceous glands, impaired external secretory function, and impaired hydration of the stratum corneum. Purpose: to increase skin moisture in patients with chronic renal failure undergoing hemodialysis Methods: This community service is carried out by providing VCO and teaching massage stroking techniques. VCO is applied to the skin twice a day for 4 weeks followed by massage. Results: Of the 53 patients, 60.37% of patients experienced an increase in skin moisture, 16.98% of patients had a decrease in skin moisture levels and 22.64% could not be evaluated Conclusion: Routinely apply VCO twice a day for 4 weeks, experience an increase in the value of skin moisture. Meanwhile, in patients who did not apply regularly, there was a slight increase compared to patients who did not apply VCO at all. Oily skin and the smell of coconut oil are the reasons for patients not to routinely give VCO.

The adherence with therapy in hypertensive patients is important to note because hypertension is a disease that cannot be cured but can only be controlled. The success of controlling blood pressure in hypertensive patients influenced by compliance factors in taking antihypertensive drugs. The purpose of this study is to evaluate the relationship between the level of adherence of antihypertensive drugsuse  in first and advanced health facilities. The study design was an observational cross sectional using subjects with hypertension patients older than or equal to 18 years old at Kotabunan Bolaang Mongondow Health Center (first level health facility) and outpatient installation of Robert Wolter Monginsidi Hospital in Manado (advanced health facilities) during the period from October to December 2019. The level of compliance with the use of antihypertensive drugs was assessed using the MMAS-8 questionnaire (Morisky 8-Item Medication Adherence Scale). The total patients in this study were 171 patients, as many as 73 patients in the first level health facilities and 98 patients in the advanced health facilities. The data of patient adherence in the use of antihypertensive drugs in the first level of health facilities showed 83.56% with low levels of adherence (MMAS score <6), and only 16.44% with moderate to high levels of adherence (MMAS score 6-8). In the advanced level of health facilities were 85.71% of patients with low adherence and 14.29% with moderate to high adherence. Comparative statistical of adherence on the two health facilities was not significantly different, with p = 0.767 (95% confidence interval). The data shows that the average patient has a routine history of control as much as 91.8% in health facilities. Based on these results, recommended that each hospital and health center must require its pharmacists to do pharmaceutical practices, and each pharmacy must not provide drug services without the presence of a pharmacist.

Monitoring realizes efforts to improve the quality of health services. To obtain information on patient condition data during treatment. The monitoring process is done manually. So that it has an impact on the service and condition of the patient during treatment. The design of monitoring the patient's body temperature and heart during treatment with IoT can be controlled through the NodeMCU sensor ESP8266, MLX9014, MAX30100 sensor and Arduino IDE software program. Furthermore, it can detect the patient's temperature and the patient's heart rate during treatment. And processed by the NodeMCU ESP8266, the data from the two sensors is displayed on the SSD1306 OLED LCD and also to the smartphone of the medical officer on duty via Blynk. In order for the tool to work properly and optimally, it is necessary to adjust the pin placement so that it can work optimally. The problem of internet connection interference causes delays, resulting in a mismatch between the measurement of the test equipment and the standard tool. Data values are taken with an accuracy of 70%-93% for the MAX30100 sensor, for the temperature sensor it is close to optimal with a value that is read on a standard tool with an accuracy of 97%-99%.
