Background: Comprehensive midwifery care is essential for early detection of complications and reducing maternal and infant mortality rates. In Sleman Regency, despite high access to health services, continuous monitoring remains crucial for ensuring maternal and fetal safety. Objective: To provide comprehensive midwifery care for Mrs. M at Klinik Pratama Shaqi, Sleman, from the third trimester of pregnancy through the postpartum and neonatal periods. Methods: This is a descriptive qualitative study using a case study approach based on Varney's seven-step midwifery management and SOAP documentation. Findings: During pregnancy (37 weeks 6 days), the mother experienced physiological complaints of back pain and shortness of breath, managed through posture education and pregnancy exercises. Labor proceeded normally and spontaneously on October 6, 2024, utilizing "gentle birth" practices such as endorphin massage to reduce pain. A second-degree perineal tear occurred and was treated according to procedure. The baby was born healthy, weighing 3100 grams, with successful Early Initiation of Breastfeeding (EIB). During the postpartum and neonatal periods, uterine involution was normal, breast milk production was adequate, and the infant's growth met standards with no danger signs. Implications: Continuous assistance improves the mother's physical and psychological readiness and ensures a healthy neonatal transition.

Maternal mortality remains a challenge in Indonesia, and one contributing factor is chronic malnutrition (CED), which impacts pregnancy, childbirth, the postpartum period, and the newborn. The 2022 registration report shows that of the 3,249,203 pregnant women whose MUAC was measured, 283,833 had a MUAC of less than 23.5 cm (at risk of CED), meaning 8.7% of these pregnant women were at risk of CED. In the Brebes area, the 2022 report recorded 3,475 pregnant women with CED. At the Bantarkawung Community Health Center in 2023, there were 36 cases of preeclampsia, 22 cases of transverse lie, 107 cases of CED, and anemia. Objective: Provide comprehensive midwifery care in accordance with standards of care for pregnant women, women giving birth, newborns, postpartum women, and family planning. Research method: Qualitative descriptive method, the type of research approach used in this study is a comprehensive case study approach. Results: Comprehensive care was provided to Mrs. R who suffered from chronic energy deficiency, and was successfully overcome. Delivery was carried out by caesarean section, the baby was born normally, and the postpartum period went smoothly, and the mother chose a contraceptive method, namely a three-monthly birth control injection. Conclusion: Comprehensive obstetric care has been proven effective in overcoming chronic energy deficiency, ensuring a safe delivery, a normal baby born, a normal postpartum period, and compliance with contraceptive methods.

Risk factors for pregnancy that cause complications and maternal death include chronic energy deficiency and anemia. The pregnancy rate for pregnant women with chronic energy deficiency at the Bumiayu Community Health Center in 2022 is 56 cases, while the incidence of anemia at the Bumiayu Community Health Center is 22 cases. Research method: the method used in this research is a qualitative descriptive method with a comprehensive case study approach. Comprehensive midwifery care for Mrs S found early pregnancy problems, namely chronic energy deficiency and anemia. The delivery was carried out normally vaginally and the baby was born normally. During the postpartum and birth control period, no complications were found. Comprehensive midwifery care for Mrs S with chronic energy deficiency and anemia was in accordance with midwifery care and no complications occurred.

MMR (WHO) in 2022 will reach 91.46/100,000 live births. MMR in Indonesia (2021) 7,389/100,000 KH. Brebes MMR in (2021) is 105 cases and IMR is 259 cases in 2022. Data from the Kaliwadas Health Center in (2022) is 1 case and IMR (2022) is 0 cases. In WHO (2021), there are (73.2%) pregnant women with CED. And in Bebes there will be 64 cases of pregnant women with Chronic Energy Deficiency (KEK) in 2022. Objective: To provide comprehensive care for pregnant women, giving birth, newborns, postpartum and family planning. Research method: Qualitative descriptive research method with a case study approach. Results: Midwifery care during Mrs. N from the results of Mrs. N experienced chronic energy deficiency KEK and was given treatment and care according to his needs. Furthermore, on the third visit, the results of Mrs. N no longer experiences chronic energy deficiencies. Birth of Mrs. N was assisted in accordance with the standards for midwifery services for childbirth (APN). Neonatal Visits and Newborn Visits I to III Baby Mrs. N experienced no complications and neither did postpartum visits I to IV. Family planning midwifery care Mrs. N is a 3-month injectable contraceptive. Conclusion: Comprehensive midwifery care has been provided to Mrs. N with KEK is in accordance with obstetric services and there are no complications.

According to the Indonesian Ministry of Health in 2020, the number of maternal mortality rates (MMR) in Indonesia is still quite high, totaling 4,627 cases of maternal death. This number shows an increase compared to 2019 of 4,221 cases of maternal death (Indonesian Health Profile, 2021). Objective: To apply the theory that has been obtained at the academy with comprehensive midwifery care for pregnant women, laboring women, postpartum women, newborns, and family planning in practice using Varney and SOAP documentation, is there a gap between theory and practice. Research method: In this study the authors used a qualitative research method with a case study approach which was carried out by examining a problem through a case consisting of a single unit. Results: At the first pregnancy obstetric care visit, there was a problem of Chronic Energy Deficiency (CHD) where LiLA was 22.5 cm and also breech location. After being given counseling about high calorie balanced nutrition there was an increase in the size of LiLA by 2, 5 cm so that the mother did not experience SEZ. In the process of childbirth was done by Sectio Caesarea. In the postpartum period there was no complication in the postpartum period. Newborn was normal, no problems. In family planning, Mrs. M chose to use a 3-year family planning implant. Conclusion: During the assistance to Mrs. M, the health problems experienced by Mrs. M, especially the incidence of SEZ in pregnant women can be resolved, and there is a gap between theory and practice related to the size of LiLA and the height of the mother.

Background: The global maternal mortality rate (MMR) in 2022 is 91.46 per 100,000 live births with the cause of death being bleeding while the indirect causes of maternal mortality are during pregnancy wheere many pregnant women experience nutritional problems such as Chornic Energi Deficiency ( CHD) and Nutritional Anemia at 8.43% (WHO, 2022). The maternal mortality rate in ASEAN is 235 per 100,000 live births with the majority of causes of death being bleeding (ASEAN Secretariat, 2022). Until now, the maternal mortality rate in Indonesia alone in 2020 was 91.45/100,000 live births. In 2021, maternal mortality will increase to 305/100,000 live births. Meanwhile, in 2022, the maternal mortality rate will decrease by 230/100,000 live births. Within a period of 3 years, the causes of maternal death included Covid 19, bleeding, hypertension in pregnancy, and heart disease (Ministry of Health of the Republic of Indonesia, 2022). The maternal mortality rate in Central Java in 2020 was 84.6/100,000 live births, in 2021 it will be 199/100,000 live births. In 2022 there will be a decrease to 98.6/100,000 live births, hypertension in pregnancy and bleeding are still the cases of death in that year (Central Health Office, 2022). Brebes Regency shows that the maternal mortality rate in 2020 was 62 cases. In 2021 there were 105 cases and in 2022 there was a decrease to 50 cases with the causes of death being bleeding, hypertension, infection and heart disorders (Brebes District Health Office, 2021) Objective: Reduce maternal and infant morbidity and mortality rates in Brebes Regency, especially the Bumiayu Community Health Center area Conclusion: After providing comprehensive midwifery care to Mrs. S aged 32 years with Chronic Energy Deficiency (KEK) at BPM Mrs. The R working area of the Bumiayu Community Health Center from the third trimester of pregnancy, childbirth, newborns, postpartum and family planning is in good condition.

This research is a comprehensive midwifery care case study on Mrs. K from pregnancy to postpartum family planning conducted at the PMB Okta Saprina, S.Tr.Keb and Mrs. K's home from October-December 2023. The assessment during pregnancy found complaints of frequent urination which were addressed by counseling on reducing drinking at night. During labor, Mrs. K experienced severe pain on a scale of 8, which was reduced to moderate pain on a scale of 5 after being given lavender aromatherapy. Labor care stages I-IV were normal according to theory and without complications. The baby was born spontaneously, full-term, without complications. During the postpartum period, Mrs. K experienced breast engorgement which was treated with aloe vera compresses for 7 days until the complaint was resolved. Care for the newborn and postpartum period was according to standards. At 42 days postpartum, Mrs. K chose to use 3-month injectable contraception after being screened with the KLOP wheel. The management provided to Mrs. K was in accordance with evidence-based practice in providing comprehensive midwifery care, namely counseling for frequent urination, lavender aromatherapy for labor pain, and aloe vera compresses for breast engorgement with satisfactory results.

Pregnancy with breech presentation is where the fetus lies longitudinally in the uterus while the head is at the fundus uteri and in the buttocks position below the uterine cavity (Putri and Ningrum, 2019). Cases of breech presentation both mother and fetus experience a large increase in risk and for labor with breech location have high mortality and morbidity (Putra 2015). The maternal mortality rate (MMR) according to WHO 2021 is 32.9/100,000 KH. ASEAN 2021 MMR is 235/100,000. Indonesian Ministry of Health 2021 AKI 7,389 people, Central Java Health Office 2023 AKI as much as 485. Data from the Brebes Health Office 2022 as many as 50 cases while from the kaliwadas puskesmas area in 2022 there was 1 case of death. Researchers implement comprehensive midwifery care using Varney and SOAP documentation. This study uses descriptive qualitative methods through direct approaches, interviews, observation and documentation. The main informant is Mrs.H while for additional informants there are village midwives, puskesmas midwives, husbands and Mrs.H's family. Based on the results of research obtained from the care of pregnancy, childbirth, postpartum, LBW and family planning, problems were found in the form of pregnancy with breech presentation and cesarean delivery for indications of failed induction.

Data from Kaliwadas Community Health Center (2022) MMR 0 case. The aim of this research was to reduce maternal and infant morbidity and mortality rates in Brebes district, especially the Kaliwadas Community Health Center area. This research method is qualitative research, namely a research method carried out by exploring, describing and explaining and predicting a phenomenon that occurs on the basis of it in the field. The results of this research are after carrying out comprehensive midwifery care in Mrs. S is 42 years old with risk factors: age, anemia and CED at the Community Health Center Kaliwadas from the third trimester of pregnancy, childbirth, newborns, postpartum and family planning using Varney and midwifery approaches SOAP found gaps between theory and practice.

The aim of this research is to reduce maternal and infant morbidity and mortality rates in Brebes Regency, especially the Bumiayu Community Health Center area. This research method uses descriptive research with a qualitative descriptive approach. The results of this research show that in the results of Comprehensive Care there was a gap between theory and practice after comprehensive midwifery care was carried out on Mrs. E is 39 years old with risk factors for CED age and anemia in Mrs. BPM. R working area of Bumiayu Health Center from third trimester pregnancy, childbirth, newborns, postpartum and family planning using Varney midwifery and SOAP approaches.

Background: World Health Organization (WHO) in 2020 the maternal mortality rate (MMR) in the world is 152 deaths per 100,000 live births. In ASEAN, the highest MMR in 2020 is in Myanmar, which is 235 per 100,000 live births. The maternal mortality rate in Brebes Regency in 2020 was 62 cases. In the Bantarkawung Health Center working area in 2020 there was 1 case, in 2021 there were 5 cases. Objective: To provide comprehensive midwifery care services to Mrs. N Age 19 Years G1P0A0 with Risk Factors Age <20 Years at the Bantarkawung Health Center, Brebes Regency with a midwifery management approach according to Varney's seven steps and SOAP.  Research method: This study uses descriptive qualitative research with a case study design. The Scientific Writing assignment report is written based on a case report of comprehensive midwifery care in pregnant women, childbirth, newborns, and postpartum using the type of case review study research method. Results: The results of comprehensive midwifery care are the absence of gaps between theory and practice where according to the theory pregnant women with a young age <20 years will experience several risks such as in any.N who experienced a short TFU of 28 cm. Conclusion: After conducting an assessment and collecting data on comprehensive midwifery care on Mrs. N with risk factors for age <20 years at the puskesmas from third trimester pregnancy.

In Indonesia, the MMR in 2022 is 230/100,000 live births (Indonesian Ministry of Health, 2022). The MMR in Central Java in 2022 was 98.6/100,000 live births (Central Java Health Office, 2022) and the MMR at the Bumiayu health center in 2022 was 2 cases (Bumiayu Health Center, 2020-2022). Objective : To reduce morbidity and mortality rates of mothers and infants in Brebes district, especially the working area of Puskesmas Bumiayu. this research is a qualitative descriptive study, the type of research used in this study is using a comprehensive case study approach. Comprehensive case study research is a method for understanding individuals that is carried out interactively (Notoadmojo, 2019). Research Results : This comprehensive care found a gap in theory and practice in pregnancy and childbirth care. Conclusion: Comprehensive Midwifery Care for Mrs. S aged 43 years with preeclampsia and high risk factors at BPM Mrs. R in the working area of the bumiayu health center from third trimester pregnancy, childbirth, newborns, postpartum and family planning with the Varney and soap midwifery approach.

According to the World Health Organization (WHO), in 2021 around 329,000 women died due to complications related to pregnancy or childbirth throughout the world, MMR in Indonesia until 2022 was 305/100,000 live births, WHO data for 2021 states the Infant Mortality Rate (IMR) in the world amounted to 235 deaths per 100,000 live births. Brebes Health Office (2022) MMR 50 cases, IMR 259 cases. Data from Bantarkawung Community Health Center (2022) MMR 1 case, IMR 5 cases. This researcher is able to carry out comprehensive midwifery care for Mrs. R aged 26 years with Preeclampsia using Varney's seven-step obstetric management and SOAP. Based on the background above, the problem formulation is how Comprehensive Midwifery Care for Mrs. R aged 26 years in the working area of the Bantarkawung Community Health Center, Brebes Regency in 2023. The research design used by the author is qualitative descriptive research. Comprehensive Care for Mrs.R found problems early in pregnancy with preeclampsia. Labor was induced, and the newborn was normal. During the postpartum period, the uterine involution process goes well. Conclusion: Through this scientific paper, it is hoped that health center health workers can maintain and improve the quality of health services in accordance with service standards and the application of theory.

Comprehensive midwifery care is a complete examination provided with simple examinations and counseling for pregnancy, childbirth, postpartum, and newborn care. The goal of continuous midwifery care given to Mrs. "N" at the Special Hospital for Mother and Child Siti Fatimah is to assess the condition of the mother and her fetus. This study was conducted from April 14, 2023, to July 9, 2023. Midwifery care methods use midwifery management based on the 7 steps of Verney from 38 weeks of pregnancy until deciding to use a 3-month contraceptive injection. The results of prenatal care include two antenatal visits during pregnancy. In the third trimester, Mrs. "N" complained of back pain, which was already manageable by the mother, so throughout the pregnancy, Mrs. "N" remained in good and normal condition. In conclusion, Mrs. "N" gave birth normally, assisted by a midwife and following the established care plan. The baby was born spontaneously with the head presenting, female, birth weight: 2,900 grams, birth length: 46 cm. Postpartum care for Mrs. "N" proceeded smoothly, and there were no complications for both the mother and the baby. The midwife provided communication, information, and education (CIE) during the care process, and during the postpartum period, the mother decided to use a 3-month contraceptive injection after 42 days of the postpartum period.

 Mother and Child Health (MCH) requires efforts through a holistic approach known as Continuity of Care (COC). Continuity is essential throughout a woman's life cycle, including during pregnancy, childbirth, and the postpartum period. To achieve COC, it can be implemented through monitoring and services such as ANC, INC, PNC, and Newborn Care, as well as connecting healthcare facilities to ensure prompt and accurate handling of emergencies. The aim of this research is to provide comprehensive midwifery care for pregnant women in the third trimester, during labor, postpartum, care for the newborn, neonates, and family planning. The study was conducted from April to May 2023 at the Special Regional Hospital for Mother and Child, Siti Fatima, Makassar. The care method in this research involves interviews, observations, and management of the subject's care. The subject in this care is Mrs. "H," a woman in her second pregnancy, who has given birth once, is currently at 37 weeks and 4 days gestation. The results of comprehensive midwifery care for Mrs. "H" during the third trimester of pregnancy showed well-preparedness for childbirth. Normal delivery occurred, followed by normal care for the newborn and neonatal care. The conclusion of this research is that comprehensive care allows for data collection, determination of actual and potential diagnoses, and timely referral or collaboration with other healthcare providers in case of issues.

High parity pregnancies increase the risk of complications for both the mother and fetus. In the Bumiayu Community Health Center alone in 2021-2022 there were 45 cases of mother with grande multipara. To provide Comprehensive Midwifery Care service to pregnant, maternity, newborn, postpartum and family planing sevices in a comprehensif manner though a midwifery management approach according to Varney and SOAP. Based on the bacground above, the problem formulation is how to provide comprehensive midwifery care for Mrs. I aged 32 years in the Bumiayu Community Health Center, Brebes Regency in 2022. The author uses a quality descriptive research method with a case study approach. In pregnancy cases of anemia and CED were found, partus precipitatus birth accurred. As well as giving formula milk to babies and lack of personal hygiene in neonatus and the normal postpartum periode. Grande Multipara Comprehensive Midwifery Care, Presipitatus delivery, giving formula milk to babies, and lack of personal hygiene with newborn and normal postpartum bebies.

The success of maternal and child health can be seen from the indicators of maternal mortality rate (MMR) and infant mortality rate (IMR). One of the causes of maternal mortality is bleeding that occurs due to anemia during pregnancy. Anemia conditions can be prevented by antenatal care (ANC) checks, and can help make labor run physiologically. Objective To provide comprehensive midwifery care to the patient, family, community or closest friends in order to get appropriate care.This study used a qualitative descriptive method with a case study approach. In the first visit pregnancy care found the incidence of Mild Anemia, at the second visit found the incidence of Mild Preeclampsia. In the care of childbirth, there were cases of premature rupture of membranes and chepalopelvic disporpotion so that delivery was carried out by Sectio Caesarea. And in the care of newborns, postpartum and family planning there are no problems and the condition of the mother and baby is normal. Based on the comprehensive care provided to Mrs. K, there were no gaps in the care of pregnancy, puerperium, newborn, and family planning, but in the care of childbirth there were some gaps between theory and practice.

Indicators to measure the quality of health services in an area can be seen from the Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR). One of the ways that can be done to reduce maternal mortality (MMR) and infant mortality (IMR) is by implementing continuous care. The purpose of this continuous midwifery care is to be able to provide comprehensive midwifery care to Mrs. "S" from pregnancy to family planning in accordance with midwifery care standards. Case study research method. The results of the implementation of midwifery care on an ongoing basis (Continuity of Care) on Mrs. "S" G1P0A0 age 24 years starting from the gestational age of 30 weeks 3 days to the age of 38 weeks. At the first pregnancy visit there was a problem in the mother, namely pregnancy with breech location. At the second visit the baby's position was in a normal position. The delivery process ended by cesarean section because the baby had an Early Rupture of Membranes. The baby was born at Budi Kemuliaan Hospital with a weight of 3000 gr Body Length : 48 cm Head Circumference: 33 cm, Chest Circumference: 32 cm. In the postpartum period, physiological complaints were found such as pain in the suture wound but not accompanied by signs of suture wound infection. Mrs. "S" chose to use 3-month injectable birth control. The conclusion of this assessment is that midwives are expected to be able to provide comprehensive care from pregnancy to family planning services and apply care in accordance with midwifery service standards.

There are various complaints experienced by pregnant women during their pregnancy, one of the most common complaints is dizziness. Objective: LTA provides comprehensive midwifery care to pregnant, maternity, postpartum, BBL, neonate and family planning women using a midwifery management approach for Mrs "P" who complained of dizziness. Method: Care in this LTA is through interviews, observation and care management. The subject in this care is Mrs "S" at the Pratama Riyyan Kabanjahe Clinic. Results: results of comprehensive midwifery care for Mrs "S" during pregnancy in the second trimester and third trimester with dizziness, in normal labor, spontaneously without complications, in the postpartum period with normal puerperium, in BBL with BBLN, in patients. The care that mothers receive for family planning counseling is to inform the mother about normal vital signs and her current condition, provide health education about family planning effectiveness, side effects, and how to use it and explain to the mother how to space her pregnancies or to space her pregnancies. After midwifery care was provided to Mrs. S, the results of the evaluation showed that the mother was healthier and able to carry out daily activities. Based on the results of the questionnaire from assessing maternal knowledge about childbirth, it was found to be ADEQUATE with 70% results from 20 questions.

Based on data from the Indonesian Ministry of Health, the number of MMR in 2020 reached 4,627 people. This number increased by 10.25% compared to 2019 of 4,197 people. The number of infant deaths in 2019 was 26,000 cases and increased by almost 40% to 44,000 cases in 2020. The Bumiayu Health Center, one of the auspices of the Brebes Regency in Central Java, contributed 2 cases of AKI and 5 cases of IMR in 2020. by conducting comprehensive midwifery care with a continuity of care approach which is carried out starting from the mother during pregnancy to family planning. Researchers implemented comprehensive midwifery care for pregnant women, childbirth, newborns, postpartum, and family planning using Varney and SOAP documentation. The research method used is descriptive qualitative with a case study approach. Data collection uses interviews, observations, physical examinations, supporting examinations, documentation studies, and literature studies, as well as medical record documentation studies, the main informant is Mrs. W with 2 additional informants namely 1 coordinating midwife and 1 patient's family and 1 triangulation informant namely the head section of the brebes district health office. Based on the results of pregnancy, childbirth, newborns, postpartum, and family planning, there are problems in pregnancy, namely at 30+3 weeks of gestation with an Hb level of 10.4 g/dl in the category of mild anemia. There is no gap between theory and practice. From the results of the study, it can be concluded that comprehensive care for Mrs. W 25 years of gestation, psychological postpartum, physiological delivery, and physiological BBL. There is no gap between theory and practice.
