Nusantara: Jurnal Pengabdian kepada Masyarakat 2026 Pusat Riset dan Inovasi Nasional

The postpartum period is a crucial phase for mothers to recover after childbirth. One of the common problems is perineal wounds, caused either by episiotomy or spontaneous rupture, which require proper care to prevent infection and accelerate healing. This case study aimed to provide midwifery care for Mrs. M, 28 years old, P3A0H3, with a second-degree perineal wound at PMB Bd. Silvi Ayu, S.Keb. The SOAP approach was applied through assessment, diagnosis, intervention, implementation, and evaluation over five days (June 12–16, 2025). Interventions included education on perineal hygiene, encouragement of nutritious food intake, light mobilization, and perineal wound care using boiled binahong leaves. The results showed decreased pain, reduced edema, a dry wound, and complete healing on the fifth day, with the REEDA score decreasing from 11 to 0. The discussion emphasized that flavonoids, saponins, and ascorbic acid in binahong leaves contribute to tissue regeneration and faster healing. Limitations of this study included the short monitoring period, limited sample, and reliance on maternal compliance. In conclusion, binahong leaf decoction was found to be effective, practical, and able to enhance maternal independence in wound care. This study is expected to serve as a reference for midwifery practice and as a basis for further research with a wider scope.

Remaja sebagai generasi penerus bangsa memiliki peran penting dalam melanjutkan pembangunan Negara, oleh karena itu dapat dikatakan bahwa remaja merupakan aset atau modal utama sumber daya manusia bagi pembangunan bangsa di masa depan. Oleh karena itu, kesehatan remaja perlu dijaga agar dapat menjadi generasi penerus bangsa yang sehat, produktif dan berkualitas. Metode penelitian ini menggunakan metode Participatory Action Research (PAR). Metode PAR menggunakan beberapa teknik, antara lain; pertama, observasi, focus group discussion (FGD), Sharing Knowladge, wawancara mendalam, pelatihan dan dokumentasi. Berdasarkan hasil penelitian, ditemukan bahwa pertama, kegiatan bakti sosial yang dilakukan oleh Himpunan Mahasiswa Kebidanan (HIMADAN) tahun 2026 di Panti Asuhan Ma'had Tahfidz Al Anwar, Desa Tegalandong, Kecamatan Lebaksiu, Kabupaten Tegal, berhasil memberikan edukasi kesehatan reproduksi, Perilaku Hidup Bersih dan Sehat (PHBS), dan gizi kepada 63 peserta anak dan remaja. Kedua, diskusi menekankan bahwa kesehatan reproduksi, PHBS, dan gizi merupakan aspek krusial bagi remaja, terutama dalam mencegah masalah seperti anemia, stunting, dan penyakit menular. Ketiga, secara keseluruhan, kegiatan ini berhasil menjadi pemicu bagi remaja untuk menerapkan pola hidup sehat, meskipun diperlukan evaluasi lebih lanjut dan penguatan bahan gizi untuk dampak yang maksimal.

Maternal mortality remains a major global health concern and an important indicator of health system performance. The World Health Organization reported in 2023 that approximately 290,000 maternal deaths occurred worldwide, highlighting persistent challenges in maternal health services. In the ASEAN region, the maternal mortality rate in 2023 was recorded at 16.85 per 1,000 live births. In Indonesia, the Maternal Mortality Rate in 2023 reached 189 per 100,000 live births according to the Ministry of Health of the Republic of Indonesia. Significant progress has been observed in Central Java Province, where the rate decreased from the United Nations Sustainable Development Goals target of 226 per 100,000 live births to 88.11 per 100,000 live births in 2023. At the district level, Brebes Regency recorded 54 cases per 100,000 live births in 2023 and reported 54 maternal deaths in 2024. Meanwhile, the Kaliwadas Community Health Center reported no maternal deaths during 2023 and 2024, indicating relatively effective maternal health services at the local level.

The highest mortality cases during pregnancy until the postpartum period are caused by several factors such as hypertension, infection, bleeding, anemia and KEK. Data obtained from pregnant women at the Bumiayu Community Health Center in 2023 were 30 cases, in 2024 there was an increase of 183 cases and in 2025 from January to February there were 30 KEK cases (Bumiayu Community Health Center profile, 2025). The purpose of the study was to provide comprehensive midwifery care to pregnant women, childbirth, newborns, postpartum and family planning (KB) using the Varney and SOAP management approaches. The method used in this study is a qualitative descriptive method with a comprehensive case study approach. The results of the study of comprehensive midwifery care to Mrs. S found problems in pregnancy with KEK have been given management according to the mother's needs. In labor, newborns, postpartum to KB Mrs. S there were no complications. Conclusion Comprehensive midwifery care to Mrs. S with KEK has been carried out in accordance with Standard Operating Procedures with the condition of the mother and baby being good.

Teenage pregnancy remains a reproductive health issue in Indonesia, including in Batam City. According to data from the Batam City Health Office in 2024, there were 18 cases of teenage pregnancy (0.06%) with the Lubuk Baja Health Center being one of the highest areas. Pregnant teenagers are at high risk of complications such as anemia, malnutrition, and low birth weight (LBW), which contribute to high rates of maternal and infant morbidity. This study used a descriptive method with a case study approach on Ny. S, an 18-year-old teenager with a pregnancy in the Lubuk Baja Health Center area of Batam City in 2025. Data collection was conducted through interviews, observations, physical examinations, and SOAP documentation following the 7-step Varney obstetric management. The findings revealed that Ny. S experienced mild complaints such as nausea, reduced appetite, and anxiety about her pregnancy. After three visits, the mother's complaints decreased, appetite improved, anxiety decreased, and nutritional status was within normal limits. Comprehensive midwifery care with a biopsychosocial approach was proven effective in improving both the physical and mental health of pregnant teenagers and preventing pregnancy complications.

Background: Comprehensive midwifery care is essential for early detection of complications and reducing maternal and infant mortality rates. In Sleman Regency, despite high access to health services, continuous monitoring remains crucial for ensuring maternal and fetal safety. Objective: To provide comprehensive midwifery care for Mrs. M at Klinik Pratama Shaqi, Sleman, from the third trimester of pregnancy through the postpartum and neonatal periods. Methods: This is a descriptive qualitative study using a case study approach based on Varney's seven-step midwifery management and SOAP documentation. Findings: During pregnancy (37 weeks 6 days), the mother experienced physiological complaints of back pain and shortness of breath, managed through posture education and pregnancy exercises. Labor proceeded normally and spontaneously on October 6, 2024, utilizing "gentle birth" practices such as endorphin massage to reduce pain. A second-degree perineal tear occurred and was treated according to procedure. The baby was born healthy, weighing 3100 grams, with successful Early Initiation of Breastfeeding (EIB). During the postpartum and neonatal periods, uterine involution was normal, breast milk production was adequate, and the infant's growth met standards with no danger signs. Implications: Continuous assistance improves the mother's physical and psychological readiness and ensures a healthy neonatal transition.

The goal of continuous midwifery care is to use the SOAP documentation technique to provide comprehensive care to pregnant women up to birth control according to care standards. From May 2 - July 14, 2025, Mrs.C, G2P1A0, 32 years old received continuous midwifery care from pregnancy until get contraception methods, starting at 34 weeks of gestation. The mother complained of back pain during prenatal check-ups. The labor process went smoothly. With the assistance of a midwife, the baby was born on June 10, 2025. Birth weight: 3000 grams; length: 50 cm; head circumference: 34 cm; chest circumference: 33 cm; received Hb0 vaccine two hours after birth, eye ointment, and vitamin K shortly thereafter. The baby did not experience any problems or complaints during the first visit. Mrs. C received four visits during the postpartum period. The mother did not feel anything during the first visit up to the following visits. She continued to breastfeed exclusively, and her vital signs and fundal height returned to normal. The mother used a three-month injectable contraceptive so as not to interfere with the exclusive breastfeeding program. Mrs.C has received continuous midwifery care during pregnancy and contraceptive use, and her health is normal. It is expected that the midwife will be able to provide continuous midwifery care so that in the event of an emergency, it can be immediately identified and promptly handled.

A high-risk pregnancy is a pregnancy that can affect both the pregnancy, delivery, newborn, and postpartum period. There are several high-risk pregnancies, including pregnant women who are <145cm tall, have low birth weight, are too young, and give birth too closely spaced, or what is known as the 4T. Prevention is carried out by conducting comprehensive supervision, this is a way to reduce MMR and IMR. The Central Java government has developed the One Student One Client (OSOC) program, it is hoped that the implementation of this program can reduce MMR in Central Java. MMR according to WHO in 2023 reached 189/100,000 live births, MMR based on ASEAN reached 235/100,000 live births, MMR according to the Indonesian Ministry of Health reached 205/100,000 live births, MMR in Central Java in 2023 has reached 485/100,000 live births, MMR in Brebes in 2023 54/100,000 live births and at the Bantarkawung Community Health Center MMR in 2023 amounted to 2 cases of maternal death. In this study, researchers used Varney and SOAP to document midwifery care comprehensively, with direct qualitative descriptive approach methods such as observation, interviews, and documentation.

Background : The Maternal Mortality Rate (MMR) worldwide reaches 183,000 per 100,000 live births (WHO, 2024). In Indonesia, there were 4,305 reported cases (Ministry of Health RI, 2024), while Central Java Province recorded 315 cases (Central Java Health Office, 2024). In Brebes Regency, there were 54 maternal deaths in 2024 (Brebes Health Office, 2024), and in the Bumiayu Public Health Center area, 2 maternal deaths were recorded (Bumiayu PHC Data, 2024). Objective : To analyze, implement, and provide comprehensive midwifery care for Mrs. A, 25 years old, with anemia in the Bumiayu Public Health Center area in 2025, using the Varney midwifery management approach and the SOAP method. Research Methods : This study used a qualitative descriptive design in the form of a case study. Results : Midwifery care during pregnancy for Mrs.A with anemia was carried out bassed on the mother’s needs. During labor, newborn care, postpartum period, and family planning services, no complications were found. Conclusion: The comprehensive midwifery care provided to Mrs. A with anemia was carried out in accordance with the Standard Operating Procedures (SOP), with both the mother and baby in good condition.

Background: Maternal mortality according to WHO (2024) is 287,000, in Indonesia (2024) 183/100,000 live births, in Central Java (2024) 3,149 live births, in Brebes (2024) there were 54 cases of maternal mortality. At Bantarkawung Community Health Center (2024), there were no maternal deaths. One of the causes of maternal mortality is postpartum hemorrhage. Postpartum hemorrhage can be caused by anemia in mothers during pregnancy. Objective: It is hoped that the researcher can provide comprehensive midwifery care for Mrs. L in accordance with midwifery service standards, covering pregnancy, childbirth, newborns, neonates, postpartum, and family planning, using the midwifery management approach according to Varney and SOAP. Research method: The method used is qualitative descriptive with a comprehensive case study approach. Findings: During the researcher's assistance with anemia cases, the researcher did not find issues that required further in-depth investigation, but there were some occurrences resulting from anemia, one of which was the mother experiencing retained placenta. From the assistance process conducted by the researcher, the midwifery findings have been applied comprehensively, from pregnancy stages to family planning, in accordance with midwifery service standards.

Continuity of Care (CoC) is a midwifery care model that provides continuous care from pregnancy, childbirth, the postpartum period, and newborn care to family planning services. This approach aims to improve the quality of care, enable early detection of complications, and enhance maternal and neonatal health outcomes. This study aimed to describe the implementation of Continuity of Care midwifery services for Mrs. D, aged 25 years, at PMB Siti Nur Azizah, Wonoayu, Sidoarjo, East Java.This study employed a descriptive method with a case study approach. The subject was Mrs. D, a primigravida woman who received midwifery care from the first trimester of pregnancy through the postpartum period, newborn care, and family planning services. Data were collected through interviews, observation, physical examinations, and medical record review.The results showed that throughout pregnancy, both maternal and fetal conditions remained within physiological limits with routine monitoring and adequate health education. Labor occurred spontaneously and normally with the application of respectful maternity care and interventions based on clinical indications. The postpartum period progressed normally with good uterine involution and adequate breast milk production. The newborn demonstrated optimal physiological adaptation and received essential neonatal care. In the family planning phase, Mrs. D chose a three-month injectable contraceptive, which was appropriate for her breastfeeding condition and pregnancy spacing needs.

Emesis gravidarum experienced by pregnant women can affect both maternal and fetal health, particularly in terms of nutrition and quality of life. One non-pharmacological approach that can be applied is peppermint aromatherapy, which should be implemented through safe and comprehensive midwifery care tailored to the mother’s needs. This case report aims to evaluate the effectiveness of peppermint aromatherapy in reducing symptoms of emesis gravidarum. The subject is Mrs. S, a 25-year-old pregnant woman at 7–8 weeks of gestation who received care at Melayu Kota Piring Public Health Center, Tanjungpinang, from February to March 2025. Data were collected through interviews, observations, physical and laboratory examinations, and documentation using the SOAP format. The results showed that peppermint aromatherapy effectively reduced the frequency and severity of nausea and vomiting, helping the mother maintain better food intake. In conclusion, peppermint aromatherapy can serve as an effective non-pharmacological alternative for managing emesis gravidarum in primary healthcare settings.

Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) can be a measure of a country's welfare in determining the optimal degree of health. The maternal mortality rate in the world is 303 thousand people (WHO 2023). MMR in Indonesia was 4,129 people in 2023. Meanwhile, the Infant Mortality Rate (IMR) in the world is 29,945 people (WHO, 2023). In Indonesia, there were 16.85 infant mortality rates in 2023. SEZ is a nutritional deficiency in pregnant women that lasts for several months or years. SEZ conditions can indirectly cause MMR and IMR. SEZ in pregnant women can cause complications such as IUGR, Abortion, LBW, Circulation, Anemia, Bleeding, and Childbirth complications. Research Objective: Providing Comprehensive Midwifery Care in accordance with midwifery care standards starting from pregnancy, childbirth, postpartum, newborn, and family planning in Mrs. A 22 years old with Chronic Energy Deficiency (SEZ) and Chepalo Pelvic Disproportion (CPD) with a midwifery management approach according to Varney and SOAP. Research Methods: Descriptive Qualitative with Case Study approach Results: Comprehensive Midwifery Care on Mrs. A aged 22 years with SEZ can be overcome by an increase in LILA, there is a CPD so that SC delivery is performed and appropriate case management is carried out so that labor, newborn, postpartum period, and family planning goes well. Conclusion: The results of the comprehensive care given to Mrs. A 22 years old with Chronic Energy Deficiency (SEZ) can be resolved marked by an increase in LILA, SC delivery on indications of CPD, normal LBW, normal postpartum, and mini pill birth control.

According to the World Health Organization (WHO), in 2023, around 305,000 per 100,000 live births of women will die due to prevalence due to CED related to pregnancy throughout the world, MMR in ASEAN in 2023 will be 100 per 100,000 live births with prevalence due to CED 40%, MMR in Indonesia in 2023 will be 4,129 with a prevalence due to KEK of 17.3%, MMR in Central Java will be 183 per live birth with a prevalence due to KEK of 6.7%. The MMR in Brebes is 1,083 with a prevalence due to CED of 5%, in Paguyangan women with a prevalence of CED from 2023-2024 is 119 cases. To find out about cases of pregnancy, childbirth, childbirth, postpartum and birth control Mrs. A with CED and history of TB. Qualitative descriptive research method with a case study approach. Comprehensive Care for Mrs. A was found to have problems with pregnancy with CED and a history of TB. Childbirth was carried out by induction based on KPD indications, there were no problems or abnormalities in the postpartum period, the postpartum period was still CED and had mild anemia, there were no complications during the postpartum period. Mrs. A chose the KB implant and there were no complaints after installation. It can be concluded that comprehensive midwifery care for Mrs. At 26 years of pregnancy, childbirth and postpartum there is a gap between theory and practice.

High-risk pregnancy is a pregnancy with a number of risk factors that can cause more serious risks or complications and disrupt the pregnancy process until the postpartum period which endangers the life of the mother and fetus, and occurs due to several factors including pregnancies that are too close together, pregnancies with a mother's age that is too young or old, or pregnancies with too frequent births. Too close pregnancy spacing is a pregnancy gap of <24 months (2 years) between the current pregnancy and the previous pregnancy. Based on the midwifery management method described by VARNEY and SOAP, this study aims to offer complete midwifery care services to Mrs. T, from pregnancy to the use of contraceptives, through a qualitative descriptive method with a case study approach. Research findings from the comprehensive midwifery care assessment that has been provided to Mrs. T aged 31 years, in pregnancy found problems spacing of less than 2 years. Mrs. T gave birth spontaneously at Siti Asiyah Bumiayu Hospital with severe preeclampsia. At By. Mrs. T visits I, II, III no problems were found. Mrs. T's postpartum. From 6 hours postpartum to 42 days postpartum, problems were found at the second visit, including hypertension and lower extremity oedema. Proper care from midwives during pregnancy, childbirth, newborn care, postpartum care, and family planning is crucial for the health of both mother and baby.

This study aims to provide an overview of comprehensive midwifery care in reducing maternal morbidity and mortality at the Paguyangan Health Center, Brebes Regency. According to WHO data in 2023, the maternal mortality rate was recorded at 4,129 per 100,000 live births, with the same rate in Indonesia and 335 per 100,000 in Central Java. In Brebes Regency, there were 54 cases of maternal death caused by various factors. This study used a case study method with a descriptive approach to analyze midwifery care during pregnancy, childbirth, postpartum, newborn care, and family planning programs. The results show that Mrs. Ny. N, aged 35 and with a history of multiparity, had a pregnancy with high-risk factors. Despite this, her delivery proceeded naturally with a spontaneous birth of a female baby, who cried loudly, weighing 3110 grams, and measuring 50 cm in length. During the postpartum period, no complications were found, uterine reflexes were normal, and lochia was physiologically appropriate. The baby was healthy and received hepatitis B immunization and vitamin K1. In the family planning program, the mother chose to use a 3-month injection contraception. The conclusion of this study is that despite the high-risk factors faced by the mother, comprehensive midwifery care can help minimize risks and improve the quality of care during pregnancy and childbirth.

(MMR) refers to the number of deaths of women that occur during the pregnancy period or within 42 days after delivery. According to WHO in (2022) MMR is an indicator that states the number of maternal deaths for 303,000 per 100.000 live births, According to the Brebes health office (2022) MMR 177.94/ 100.000 live birth, IMR 197 cases. According to data obtained from the Sirampog Health Center daring 2022, the maternal mortality rate was recorded at one case, and eleven infant dealths. The study aims to provide comprehensive midwifery care to patients with a history of cesarean section, using a qualitative descriptive case study approach. Based on integrate midwifery care carried out on patients who had a history of cesarean section, the results were obtained, namely Mrs. Saya’s pregnancy with an indication of CPD as well as high blood pressure and ureine protein approaching positive one. Delivery was carried out by secarean section as an effort to prevent complications in the mother anda baby. During the neonatal care period up to 28 days of age, no abdormalities or problems were found. During the postpartum period up to 42 days, on problem or complications were found. Mrs. I  chose to use an IUD as a contraceptive.

Maternal mortality remains a challenge in Indonesia, and one contributing factor is chronic malnutrition (CED), which impacts pregnancy, childbirth, the postpartum period, and the newborn. The 2022 registration report shows that of the 3,249,203 pregnant women whose MUAC was measured, 283,833 had a MUAC of less than 23.5 cm (at risk of CED), meaning 8.7% of these pregnant women were at risk of CED. In the Brebes area, the 2022 report recorded 3,475 pregnant women with CED. At the Bantarkawung Community Health Center in 2023, there were 36 cases of preeclampsia, 22 cases of transverse lie, 107 cases of CED, and anemia. Objective: Provide comprehensive midwifery care in accordance with standards of care for pregnant women, women giving birth, newborns, postpartum women, and family planning. Research method: Qualitative descriptive method, the type of research approach used in this study is a comprehensive case study approach. Results: Comprehensive care was provided to Mrs. R who suffered from chronic energy deficiency, and was successfully overcome. Delivery was carried out by caesarean section, the baby was born normally, and the postpartum period went smoothly, and the mother chose a contraceptive method, namely a three-monthly birth control injection. Conclusion: Comprehensive obstetric care has been proven effective in overcoming chronic energy deficiency, ensuring a safe delivery, a normal baby born, a normal postpartum period, and compliance with contraceptive methods.

Anomalous positioning is an abnormal position of the fetal vertex (with the small fontanelle as a marker) relative to the maternal pelvis. Oblique malposition is a cause of maternal mortality rate (MMR) of 3-10%. Obesity in pregnancy is a condition characterized by an imbalance between body weight and height. According to WHO data, the prevalence of obesity in 2021 was 340 million, with data in Central Java province reaching 6.62%. In Brebes Regency, the figure was 20.51% (2019). In the Bumiayu Community Health Center, data on obesity in pregnant women reached 63 in 2022. This study aims to provide comprehensive midwifery care for pregnant women, childbirth, postpartum, newborns, and family planning for Mrs. N, 23, with malposition and obesity. This study used a qualitative descriptive method with a comprehensive case study approach. Based on the comprehensive midwifery care provided to Mrs. N, 23, with malposition and obesity, the pregnancy outcome was found to be grade I. Her gestational age was not commensurate with her gestational age. Delivery was performed by vaginal delivery. No problems were found during newborn care up to 28 days. No problems were found during the postpartum period. Mrs. N chose intrauterine contraception (IUD) during pregnancy, but her husband did not allow it due to religious reasons. Appropriate midwifery care during pregnancy, childbirth, newborn care, postpartum care, and family planning is crucial for the health of both mother and baby. This allows early detection of risk factors, preventing potential complications, and reducing maternal and infant mortality.

Breech presentation is a condition in which the fetus is in an elongated position with the head at the uterine fundus and the buttocks in the lower part of the uterine cavity. This condition can increase the risk of labor complications, especially if accompanied by serotinus (post-term pregnancy). Based on data from the Indonesian Ministry of Health (2021), the maternal mortality rate caused by breech presentation and serotinus in Indonesia was recorded at 1,309 cases. In Central Java, the incidence of breech presentation reached 11.5% (Central Java Health Office, 2021). This study aims to determine the implementation of comprehensive midwifery care for Mrs. D, aged 24 years, with breech presentation and serotinus at Mrs. S's PMB in the Kaliwadas Community Health Center area in 2023. The research method used a qualitative descriptive design with a case study approach. Data collection was carried out through interviews, observations, physical examinations, documentation studies, and medical record searches. The results of midwifery care showed that during pregnancy, Mrs. D had a good quality of life. Mrs. D underwent three antenatal care (ANC) visits, during which signs of pregnancy complications suggestive of serotinus were detected. Delivery was performed via cesarean section due to serotinus and breech presentation. During the neonatal period, three visits were conducted, with the baby's examination results within normal limits. During the postpartum period, four visits were conducted, during which the mother's condition was generally good, but at the third postpartum visit, problems with breastfeeding were discovered. Mrs. D was also known not to be using postpartum contraception. The results of this study emphasize the importance of early detection and appropriate management of breech presentation and serotinus pregnancies to prevent complications that endanger the mother and baby. Comprehensive midwifery care covering pregnancy, labor, postpartum, and newborn care is an integral effort to ensure the safety of mothers and babies.
