Background: General anesthesia is the act of administering a combination of drugs during surgery with the aim of reducing consciousness and eliminating pain in the patient. There are 2.5% of patients experiencing intra-anesthesia complications including hypotension.Objective: To determine the comparison of giving fluid loading and reducing inhaled gas with giving ephedrine to prevent hypotension in patients in central surgery installations.Research Metod: Quantitative method with experimental research type with quasi-experiment. This research used a Pretest-Posttest Two Group Design with 22 respondents.Research Result: Based on test statistics, it is known that the results of the Independent Sample T Test data analysis test show a P value (>0.05), namely H0 is accepted. Based on this test, it was concluded that there was no significant relationship between administering fluid loading and reducing inhaled gas with administering ephedrine to prevent hypotension.Conclusion: Based on this research, researchers are more effective in using fluid loading and reducing inhaled gas than administering ephedrine in the ETT technique for general anesthesia.

Background: Spinal anesthesia is a type of extensive nerve block by inserting a local anesthetic drug into the subarachnoid space at the lumbar level (Rehatta, 2019). This method produces anesthesia by blocking transmission, deactivating motor and sensory abilities, thus creating loss of pain sensation in the perineum. lower abdomen to lower extremities. Among the effects of spinal anesthesia that often occur, post-spinal anesthesia headache is a complication that often does not receive special attention. Due to the minor effects of spinal anesthesia. Management of headaches after spinal anesthesia, one of which is using non-pharmacological techniques with Head Up therapy, namely providing a 30 degree head elevation position with the aim of increasing blood flow to the brain in an effort to maximize oxygen flow to the brain which is believed to reduce the sensation of headaches. Objective: To analyze the effect of giving Head up therapy on headaches in post-operative patients with spinal anesthesia. Method: This research is a quantitative research using a type of experiment with a pre-experimental design, post-test control group design. Sampling technique: accidental sampling Population: The population of this study was 300 people. Sample: This study involved 76 respondents with a division of 38 respondents who were given Head Up therapy and 38 respondents who were not given Head Up therapy. Data analysis: Data were analyzed using the Mann Whitney test. The research instruments were Head Up SOP and Posttest Numeric Rating Scale (NRS) questionnaire. This research was carried out in February-April 2024 at Dr Hardjono Ponorogo Regional Hospital. Results: Based on research and after data analysis, the significance value (P value) in the Mann Whitney test was 0.001 (p>0.05). Conclusion: It was found that there was an effect of giving Head up therapy to post-operative patients with spinal anesthesia

Hypertension is said to be the silent killer because it often has no symptoms and tends to be left uncontrolled. Management of hypertension such as dietary compliance, adherence to taking hypertension medication, and environmental modification are important things that can control hypertension which in turn can prevent complications. The purpose of this study was to determine the description of medication adherence and changes in blood pressure in patients with hypertension. This study used a descriptive survey design, a sample of 57 patients with hypertension, the sampling technique used the total population. Statistical analysis using frequency distribution. The results showed that the middle age of hypertensive patients was 54 years, male 57.9%, graduated from elementary school 50.9%, and worked as laborers 38.6%, adhered to hypertension medication 54.4%, the mean initial blood pressure systole 190.00 mmHg and diastolic 90.00 mmHg while the final systolic blood pressure is 140.00 mmHg and diastole is 80.00 mmHg. It is recommended that nurses provide continuous information and motivation to respondents about the importance of being obedient or obedient in taking hypertension drugs.

Labor is the occurrence of the opening and thinning of the cervix and fetus into the birth canal which this process can cause anxiety. This study aims to apply husband assistance to reduce the anxiety felt by mothers with the length of the labor process at Kardinah Hospital, Tegal City. This research is a descriptive research with a case study approach. Research subjects are clients with primigravida or pregnancy for the first time who experience anxiety during the delivery process. The results of the case study research with husband assistance to reduce anxiety using the HARS Scale in patient 1 with a score of 34 after husband assistance experienced a decrease in anxiety to 20 and in patient 2 who previously experienced it with a score of 33 after husband mentoring experienced a decrease in anxiety to 21. Patients with Anxiety when facing the length of the labor process requires assistance from the husband which aims to reduce the anxiety felt by the mother during the delivery process.

On August 11, 2020, the World Health Organization (WHO) recorded 19,936,210 people infected with the corona virus and 732,499 people who died, the death rate reached 3.7% 4. In Indonesia, the number is still actively increasing every day, ranging from 1,000 people. every day in August 2020 and the total number of COVID-19 sufferers on August 11, 2020 was 128,776 people and 5,824 people died, an increase of 4.5% from the previous number 5. Central Java Province itself was ranked 4th out of all provinces in Indonesia with the number of patients with Cobid 19 as many as 11,423 people and those who died as many as 1,049 people. Diabetes Mellitus is known as a silent killer disease because it is often not known by the sufferer and when it is known that complications have occurred to the eye (Diabetic Retinopathy), heart (diabetic chediomyopathy), repeated infections can occur, ulcers that do not heal and even amputation of fingers/toes. The design of this research is descriptive analytical with a cross sectional approach. The sample in this study were 30 people using accidental sampling. Data was collected by distributing questionnaires. Furthermore, the data were collected and tested using the chi-square test with a significance level of Asymp.ig.(p-value) = 0.000, where the p-value is smaller than the value of = 0.05. The results of the study where there was good family support with good management of 4 pillars as many as 17 respondents and good family support with poor management of 4 pillars as many as 1 respondent. While for poor family support with good management as many as 1 respondent and poor family support with poor management of 4 pillars as many as 11 respondents. The conclusion in this study is that there is a description of family support for type 2 DM patients in carrying out the 4 pillars of managing DM in the poly room at Bhayangkara Hospital, Manado

The case fatality rate caused by Covid-19 in Indonesia is around 2.9%. The death rate by age group is from all Covid-19 patients who died, including 0.5% aged 0-5 years, 0.5% aged 6-18 years, 2.8% aged 19-30 years, 12.7 % were aged 31–45 years, 36.8% were aged 46–59 years, and 46.7% were aged 60 years and over. With the large number of cases that occurred in Indonesia, it turned out to be due to the lack of proper handling of the early symptoms of the Covid-19 virus. To overcome this, it is needed as a tool for early detection of the Covid-19 variant by using the CBR-AHP expert system with the Sorgenfrei algorithm. The results of the consultation for the early detection of the Covid-19 variant were obtained from the highest similarity value of Sorgenfrei. Of all the variants found above, the highest similarity value of Sorgenfrei is Omicron with a similarity of 1,000. Omicron has the highest value because it has a value of a, which is the same symptom between old cases and large new cases.

Starting from the development of medical technology that is increasingly sophisticated and rapidly growing,researchers conduct medical research, namely about patient infusion handling services. In handling patient infusion, currently it is still manual which is carried out by nurses / medical personnel. Infusion handling services for patients still have shortcomings, namely the process of monitoring and replacing infusion fluids which are often late. If the problem is not treated quickly, it can lead to problems, namely the presence of air embolism in the blood vessels (the entry of foreign objects into the blood vessels, for example air). From that problem, the researchers made a new innovation in medical technology in handling infusions automatically and based on IoT. In this study, the smart online infusion device that has been made has good features and is very effective in handling infusions. This device has 3 main functions, namely: it can monitor the remaining infusion, it can change the infusion fluid automatically and it can indicate a blocked patient's infusion. This device already has a method for processing data with fuzzy logic. Media monitoring has been supported by a website that can be controlled remotely and in real time. Tests have been carried out and the effectiveness of the system is found to have an error rate of 0.2% - 0.7% and has an accuracy of 98%. Thus this tool can be used in terms of handling patient infusion automatically.

The State of Indonesia in providing protection for patients who receive medical services, doctors and dentists in carrying out medical practices are required to provide medical services according to professional standards and standard operating procedures in accordance with Indonesian law no. 29 of 2004 article 51 letter a,.Informed consent is one of the requirements that must be met before doctors perform medical procedures on their patients.  However, even though there is Indonesian Law No. 29 of 2004, there are still doctors who commit negligence by providing services that are not in accordance with procedures by not asking for approval for medical actions that lead to malpractice claims from their patients.. The research conducted is classified as normative juridical research, using primary legal materials and secondary materials from laws and regulations, court decisions, legal theory, and existing journals as a basis.. The doctor's error by taking actions that are not in accordance with the procedure in the form of without asking for informed consent is an administrative violation so it is categorized as administrative malpractice. Indonesian Supreme Court Decision No. 233 K/PID.SUS/2021 which imposes a prison sentence in an administrative malpractice case on a legal basis that has been declared non-binding by the Indonesian Constitutional Court is an act of carelessness. However, the guilty verdict for the defendant was already correct, because the defendant did not give informed consent before carrying out a risky procedure for his patient

Background: Coronary artery disease (CAD) is the main cause of death in the world. In this study we assessed the ankle-brachial index (ABI) as a screening tool for CAD. Methods: Between 2020 and 2023, a large population-based cross-sectional study was conducted on 4,207 new patients referred to Harapan Kita Hospital. The patients underwent selective coronary angiography via a radial artery approach. ABI was calculated for all patients. Researchers compared ABI with coronary angiography results to determine the specificity and sensitivity of ABI as a screening tool. Results: Abnormal ABI was significantly more common in patients with proven severe CAD (893, 54.8%) than in patients with proven mild CAD (33, 4.7%) or no CAD (94, 5.3% ). The specificity of the ABI was 95.3% and the sensitivity was 54.8%. ABI is associated with risk factors such as smoking, male gender, hypertension, diabetes mellitus and dyslipidemia. Conclusion: ABI can be used as a screening test to prevent CAD with a specificity of 95.3%. We need to consider risk factors other than ABI to improve screening sensitivity. Multidimensional scoring systems should consider risk factors and other noninvasive tests in addition to ABI to develop an ideal screening system for CAD

Pulmonary tuberculosis is an infectious infectious disease caused by bacteria, which can attack the respiratory system, namely the lungs. This disease annually contributes to the highest mortality in the world, including one of developing countries. To achieve successful treatment, patients must patiently take TB drugs thoroughly. There are several factors that influence the failure of treatment, namely from the patient himself, education, knowledge, and other social support.  The purpose of this study was to determine the relationship of knowledge and motivation to adherence to taking medication for pulmonary tuberculosis patients in hospitals. H.Adam Malik in 2020.This type of research is descriptive correlation with cross sectional research design. The number of samples was 31 from April to June 2020. The research was conducted at RSUP. H. Adam Malik  at TB Polyclinic. Bivariate analysis using the chi-square test to determine the relationship of Knowledge and Motivation to medication adherence in pulmonary TB patients.  Based on a hypothesis test using chi-square with a confidence level of 95%, there was no significant relationship between knowledge and medication adherence (p = 0.473). There was a significant relationship between motivation and medication adherence (p = 0.001). The conclusion of this study is that there is no relationship of knowledge with adherence to taking medication and there is a relationship of motivation with adherence to taking medication in TB patients.

The combination of low-dose intrathecal morphine with spinal anesthesia provides an effective and safe postoperative analgesia effect and may enhance the analgesia effect of bupivacaine. At Vita Insani Hospital, almost all patients who will undergo S.C surgery are generally carried out with spinal anesthesia by an anesthesiologist with habitual drug use and the dose used is bupivacaine 0.5% hyperbaric 12.5 mg plus adjuvant morphine 0.2 mg. Seeing the phenomenon of this case, the researcher wanted to see how the description of the effect of giving bupivacaine 0.5% hyperbaric 12.5 mg plus adjuvant morphine 0.2 mg which is commonly used in spinal anesthesia on the duration of analgesia and pain response time that will be felt by patients after sectio caesarea surgery at RS Vita Insani. The research design used is a quantitative descriptive method with a cross sectional approach, with the method used in this research is a prospective descriptive conducted in the post-surgical treatment room (recovery room) Central Surgical Installation (IBS) Vita Insani Hospital from May 2021 to July 2021. This study only uses one group because it only describes the results of a condition. The number of samples as many as 63 samples with purposive sampling technique in accordance with the inclusion and exclusion criteria that have been determined The respondents felt the most pain starting at 3 hours of postoperative assessment with respondents who felt mild pain as many as 31 cases (49.2%) and at least respondents who felt severe pain as many as 5 cases (7.9% ). and pain increased at 4 hours postoperative assessment, namely moderate pain. However, it can also be seen at 0 to 2 hours of postoperative pain assessment that more respondents still do not feel pain. That the duration of analgesia began to decrease in the first hour and then the most significant postoperative pain was felt at 3 hours postoperatively. So that other pain management is needed to reduce the pain experienced by patients such as relaxation techniques, distraction and collaboration of pharmacological therapy in dealing with postoperative pain.

Background: Post spinal anesthesia complications, one of which is urinary retention, it is an inabity of the bladder to partially of completely empty urine during urination. In performing injections on the subarachnoid can be done on the patient in a sitting position on the surgery table or in a tilted/lateral decubitus position. Objective: To analyze the corellation between spinal position an the occurrence of post-anesthesia urinary retention, with the inclusion criteria of patients undergoing spinal block anesthesia in sitting position. Research Methods: The design of this research is analytical observation with a cross sectional approach, sampling using pusposive sampling technique. The number of samples as many as 42 people, the sample was taken in the Central Surgical Installation Room of RSUD dr. H. Jusuf SK. Collecting data using observation sheets that have been prepared and given instructions on how to fill them, regarding the patient’s sitting position in spinal surgery, that is the sitting position with both legs straight or sitting sitting position with both knees flexed, as well as the patient’s ability to urinate after spinal surgery, which is less than 8 hours or more than 8 hours after spinal surgery. From the study, the were 16 respondents who experienced post-spinal urinary retention, 13 respondents who received a spinal sitting position with both legs straight and 3 respondents who received a spinal sitting position with both knees flexed. Result: Statistical test using chi square, obtained P value = 0.715, thus P value > alpha (0.05), so Ho is accepted, it means that it can be concluded that there is no correllation between spinal anesthesia block position and the ccurance of post-anesthesia urinary retention.

Surgery still often causes various problems, one of which is pain, but the problem of pain during and after surgery can be overcome by anesthesia technique. One of them is spinal anesthesia technique. However, spinal anesthesia still has drawbacks where the duration of sensory and motor blockade is still very short. Various methods are used to prolong the duration of sensory and motor blockade of spinal anesthesia, one of which is the addition of adjuvants such as fentanyl and clonidine. This study was conducted to compare the speed of onset of action of sensory-motor blockade and the lengthening of duration of action of sensory-motor blockade between the addition of 25 g fentanyl compared to 50 g conidine in 10 mg hyperbaric 0.5% bipivacaine administered under spinal anesthesia. This study used a Randomized Control Trial (RCT) design with a double-blind randomized control trial and was conducted at Mitra Plumbon Hospital Cirebon from March-April 2021. There were 42 patients who met the inclusion criteria who underwent surgery with spinal anesthesia. Patients were divided into 2 groups, group 1 used 10mg hyperbaric 0.5% bupivacaine plus 25µg fentanyl, while group 2 used 10mg hyperbaric 0.5% bupivacaine plus 50µg clonidine. We investigated the onset of action and duration of action of sensory-motor blockade, the effect of hemodynamic shock, the effect of sedation levels and intraoperative adverse effects (side effects). Data analysis using SPSS 20.0. In the group of 10 mg bupivacaine 0.5% hyperbaric plus clonidine 50 g, the onset of action of sensory-motor blockade was faster and the duration of action was longer than in the group of 10 mg bupivacaine 0.5% hyperbaric plus 25 g fentanyl with a P value <0.05. Likewise, the effect of hemodynamic fluctuations in the 10mg hyperbaric 0.5% bupivacaine group plus 50 g clnidine was more stable than the 10mg hyperbaric 0.5% bupivacaine group plus 25 g fentanyl. A common disease experienced by the elderly is hypertension. Cases of hypertension in Demak are quite high, one of them is in Mranggen village. This reality proves that many people still ignore hypertension. Because there are sufferers who feel no symptoms and there are symptoms such as headaches, weakness, fatigue, shortness of breath and blurred vision. If not treated immediately it will cause complications such as kidney failure, stroke, coronary heart disease and retinopathy. It is of course important to make efforts to control blood pressure. A combination of non-pharmacological management of deep breathing exercise and religious music is one of the efforts that can be made. The purpose of this study was to describe the administration of a combination of deep breathing exercise and religious music therapy to the blood pressure of elderly hypertensive patients. This research uses descriptive case study research with a nursing care process approach.The number of samples is 2 respondents, with the criteria of elderly clients who have hypertension, clients who do not take medication during hypertension in order to determine the effectiveness of the action, clients who can communicate well, and clients who are willing to be respondents. Deep breathing exercises and religious music are carried out once a day for 3 days in 1 week with a duration of 15 minutes. Blood pressure measurement using a sphygmomanometer and stethoscope. Blood pressure measurements were taken before and after the procedure. The results showed that the blood pressure of respondent 1 before and after being given deep breathing exercise therapy and religious music for 3 consecutive days with a duration of 15 minutes, the blood pressure on the first day before receiving therapy was 160/70 mmHg and after being given therapy for 3 days on the last day the blood pressure became 150/60 mm Hg. In respondent 2 before and after being given deep breathing exercise therapy and religious music for 3 consecutive days with a duration of 15 minutes the blood pressure on the first day before receiving therapy was 179/80 mmHg and after being given therapy for 3 days on the last day the blood pressure was 156/70 mmHg .

Application of generalist therapy in Schizophrenic Patients with Nursing Problems Hearing Hallucinations   Auditory hallucinations are hearing voices or noises that are less clear or clear, where sometimes these sounds are like talking to the patient and sometimes ordering the patient to do something. If auditory hallucinations cannot be controlled, it can result or impact injuring oneself, others and the environment. The purpose of this case study is to determine the implementation of generalist therapy in schizophrenia patients with auditory hallucinations nursing problems at the Avicenna Clinic Makassar. This scientific paper uses a case study method design with a nursing process approach consisting of assessment, diagnosis, intervention, implementation and evaluation of nursing. The data collection technique was carried out through interviews and observations at the Avicenna Clinic in Makassar which was carried out on 27-30 December 2021.

: Family support is one of the factors that may affect relapse in patients with epilepsy. Epilepsy is one a illness of brains and nerve’s with the highest incidence to four from ten diagnose in the Mental Hospital of West Java Province in  January-march 2022, with the incidence rate of as much as 500 patients. This study aims to determine the relationship of social support of families with epilepsy patients relapse frequency in the out patient Mental Hospital of West Java Province. Design of quantitative research with cross sectional approach and the sample amounted to 62 patients with epilepsy. Results of that study (40,3%) less family support, (9,7%) were family social support, and (50%) good family support. Results for the frequency of relapse in patients with epilepsy (38,7%) high recurrence rate (32,3%) recurrence rate was, and (29%) recurrence rate is low. The results using spearman rank test was obtained ρ value of 0,000 < 0,05. Means Ho is rejected, this shows there is a relationship of social support for families with epilepsy patients relapse frequency in the out patient Mental Hospital of West Java Province. Spearman correlation coefficients obtained a value of 0,603 which means that there is a significant relationship between social support of families with the frequency of relapse in patients with epilepsy. So it is suggested that the education in the family in order to continue to provide support to patients with epilepsy and is expected to nurse a mental hospital to provide information to families about the importance of the meaning of family social support for patients with epilepsy.

Bell's palsy is a neurological disorder and disorder of the cranial nerve VII (facial nerve) in the temporal bone area that causes weakness or paralysis of the facial muscles around the stylomastoid foramen. The cause is not known to be vascular ischemia, viral disease (Herpes simplex, Herpes zoster), autoimmune disease, or a combination of these factors. The purpose of this study was to determine the effect of Physiotherapy Management in Bell's Palsy with Infra Red, Electrical Stimulation of Faradic Current and Exercise Therapy as modalities. The study was conducted at Bendan Hospital, Pekalongan City with descriptive analytic method in patients with Bell's Palsy condition. Methods of collecting research data using autoanamnesis and physical examination. The research instrument consisted of examining pain using the Visual Analogue Scale, muscle spasm with palpation, muscle strength using manual muscle testing, and functional activity using the Ugo Fisch scale. After therapy 1 to therapy 4 there was a decrease in pain from T1 = tenderness 4 to T4= tenderness 0. Increased facial muscle strength T1 = m. orbicularis oculi 3 to T4 = m.orbicularis oculi 5. Decreased muscle spasm T1 = m. frontalis 1, m. corrugator supercilli 1, m. zygomaticum 1, m. orbicularis oris 1, m. mentality 1 to T4 = m. frontalis 0, m. corrugator supercilli 0, m. zygomaticum 0, m. orbicularis oris 0, m. mentalis 0. The conclusion is Infra Red, Electrical Stimulation of faradic currents and Exercise Therapy can reduce pain, decrease muscle spasm and increase muscle strength on Bell’s Palsy Sinistra.

Diabetes Mellitus (DM) is a chronic disease that occurs when the pancreas is disable to produce insulin, which isenergy. Good knowledge determines adherence to diet in patients with type 2 diabetes mellitus. The purpose of this study was to determine the relationship between knowledge and dietary compliance of patients with type II diabetes mellitus at WolterMonginsidiTk.IIHospital Manado. This study was conducted in August on patients with diabetes mellitus at WolterMonginsidiTk.IIHospital Manado with population are 141 respondents and 35 respondents was taken as samples of this research with using non-probability sampling technique type which is accidental sampling technique. The type of this research was using descriptive analytic method with a cross sectional approach, and used questionnaire as research instrument. The data was collected and analysed using Chi-square test with a significance (α) ≤0.01. The results of Chi-square statistical test obtained ρ-value (α = 0.01). This means that there is a significant relationship between knowledge and dietary compliance in patients with diabetes mellitus at the WolterMonginsidiTk.II Hospital Manado. The conclusion of this studythere is a relationship between knowledge and dietary compliance in patients with diabetes mellitus. The suggestion from this study is hopefully this study can give benefit in Nursing development.

Hypertension is an abnormal increase in blood pressure in the arteries continuously over a period of time. A person is said to be hypertensive if the systolic blood pressure is equal to or above 140 mmHg and/or the diastolic blood pressure is equal to or above 90 mmHg. The high prevalence of hypertension is one of the problems that affect public health status. The purpose of this study was to determine the effect of giving celery leaf decoction on blood pressure in hypertension patients in Mahawu, Manado This research design is a pre-experimental research design (one-group pre-post test design). The population in this study amounted to 53 patients with the sample used was 17 respondents. The sampling technique used is Accidental Sampling. Data collection using blood pressure observation sheet. The results of the study found that giving boiled celery leaves before giving boiled celery leaves were 12 respondents (70.6%). And after giving boiled celery leaves, 13 respondents (76.5%). The result of p value 0.000 (<0.05) so Ha is accepted. There is an effect of giving boiled celery leaves on blood pressure in hypertensive patients in Mahawu, Manado. The conclusion in this study is that there is an effect of giving celery leaf decoction on blood pressure in hypertension patients in Mahawu, Manado. Suggestions to respondents who often experience hypertension can do their own treatment when high blood pressure by boiling celery leaves.
