Psoriasis is a chronic immune disease of the skin characterized by the accelerated exchange of epidermal cells and a faster-than-normal keratinization process. Psoriation has a variety of clinical manifestations, one of which is gutata psoriasis which has a distinctive image in the form of red spots rising like water droplets with a small diameter that usually does not exceed 1 cm, arises suddenly and disseminates. The case report is about psoriasis being administered with methotrexate. The patient comes with a major complaint in the form of a full-body itch that the patient has been feeling for more than 20 years. On dermatological status in the anterior et posterior thoracalis region, the brachii et antebrachii dextra et sinistra, the femoral region et crusis dextra et sinitra found hypopigmentation plaques with firm, multiple, milier-numular sizes. Patients are treated with methotrexate taken 3 tablets 1x a week, and folic acid 1x1 tablets.

Human Immunodeficiency Virus/Acquired Immune Deficiency Syndrome (HIV/AIDS) can attack the human immune system. HIV/AIDS patients experience symptoms both physically, psychologically, socially. This condition is related to the occurrence of self-concept of HIV/AIDS patients who have a negative view of themselves. HIV/AIDS patients have difficulty managing themselves because they lose hope so that it can affect their psychological well-being. The purpose of this study was to analyze the relationship between self-concept and psychological well-being in HIV/AIDS patients This research method uses correlational analytic with cross sectional approach. The population was 50 respondents and the research sample was 50 respondents HIV/AIDS patients at the Cahaya Prolink Institute, Probolinggo regency using total sampling. The research instrument used a Robson Self Concept Questionnaire to measure self-concept, Riff Psychological well Being to measure psychological well-being, both questionnaires have been tested for validity and reliability. Then analyzed using the Spearman Rank test. The results of this study showed that 26 respondents (52.0%) or the majority of HIV/AIDS patients had low self-concept, and 25 respondents (50.0%) of HIV/AIDS patients had low psychological well-being. Based on the results of the Spearman Rank statistical test, it can be said that there is a correlation between self-concept and psychological well-being, pvalue=0.001 with a significant level of 0.05 (p≤0.05). HIV/AIDS patients experience low self-concept problems such as feeling humiliated, worried, having no purpose in life. HIV/AIDS patients experience low psychological well-being such as feeling no future, down, frustrated, worried, disappointed. It can be concluded that there is a relationship between self-concept and psychological well-being in HIV/AIDS patients. It is hoped that HIV/AIDS patients have a good picture so that they can accept themselves, have hope to continue life.

Pemphigus vulgaris (PV) is a chronic intraepidermal bullous disease that causes extensive skin lesions. Pemphigus vulgaris initially appears in the form of vesicles and loose-walled bullae that break easily on normal skin, which then develop into bullae with an erythematous base, then rupture, causing erosion, excoriation, which is usually accompanied by crusting and pain. Bullae occur due to damage or damage to intercellular adhesion due to IgG autoantibodies to desmoglein 3, causing epithelial cells called acantholysis (epidermal cell separation reaction). The hallmark of pemphigus is erosion that extends into normal skin by pulling on remnants of the blister wall or rubbing on the periphery of an active lesion. Erosion can be induced on normal-appearing skin away from the active lesion by mechanical stress or shear, a phenomenon known as the Nikolsky sign. Prognosis before corticosteroids are used, death occurs in 50% of patients within the first year. Treatment with corticosteroids makes the prognosis better. Pemphigus vulgaris in the early stages will be easier to control than those with widespread manifestations, the mortality rate will increase if there is a delay in therapy.

Implementing family-based discharge planning, which relies on the participation of family members, can help stroke victims feel better. The purpose of this study was to identify the effect of family-based discharge planning on the quality of life of stroke patients based on journal literature reviews. Methode uses systematic literature reviews of journals taken from research in 15 journals, which were discovered through screening results of a number of journals from Google Scholar, Cochrane, Science Direct, Pubmed, and Wile using the keywords "Discharge Planning" AND "Family" AND "Quality Of Life" AND "Stroke OR Post Stroke". Writing research journals from the 2017-2021 time frame  and those that complied with inclusion and exclusion standards, in PICOTS format, both national and international journals. Results from 15 design journals used quasy experimental (33.5%), Systematic Review (13.4%), Randomized controlled (13.4%), Cross sectional survey (6.7%), Longitudinal Survey (6.7%), True Experimental (6.7%), Parallel group (13.4%), Retrospective Study (6.7%). Conclusion Discharge planning provides family caregiver education about stroke. The support of family members is very important in improving the patient's health by contributing to quality care and creating a pleasant atmosphere.

There are a number of factors that can impact the value of uric acid levels, which can result in increased blood uric acid levels.This study aims to determine variables influencing uric acid levels. This research method used correlational analytic with cross sectional approach. The sampling technique used  total sampling of gout respondents with a total sample of 51 responents. The results of this study found that there was a relationship between all the  factors stuied, both from diet, age, physical activity  and BMI with p value ≥ 0.05, while the dominant factor releted to the level of uric acid levels is the dietary factor with p value = 0.000, a ≥ 0.05 and OR value = 30.664. It can be concluded that diet, age, physical activity  and BMI can affect uric acid levels but the most dominan factor in creasing uric acid levels is diet, so that future researchers are expected to provide can examine dietary variables in gout patients and increase the number of samples so that the results of the study are maximized .

Surgery is a treatment action with invasive techniques, namely opening the body part to be treated through an incision and ending with closure and suturing of the wound. Anxiety occurs as an emotional response process when the family feels fear, which is followed by several signs and symptoms such as tension, fear, and alertness. The purpose of this study was to determine the relationship between family knowledge about surgery and family anxiety levels in pre-operative patients at TK II Robert Wolter Mongisidi Hospital, Manado. This research uses descriptive analytic method with cross sectional approach. The statistical test is as many as 33 respondents using accidental sampling. Data was collected by using a questionnaire to each respondent and using the HARS (Hamilton Anxiety Rating Scale). Furthermore, the data that has been collected is processed using the SPSS version 16.0 computer program to be analyzed with the Chi-Square test with a significance value (α) of 0.05. The result of this study is that there is a relationship between family knowledge about surgery and the level of family anxiety in pre-operative patients at TK II Robert Wolter Mongisidi Hospital, Manado. p = 0.000 where the p value is smaller than = 0.05.

Background; Stroke is a blood circulation disorder in the brain which results in the death of brain tissue. Stroke patients who experience hemiparesis can result in functional impairment, impaired mobility, impaired daily activities and irreversible disability. Purpose: This study aims to determine the results of implementing rubber ball ROM exercise on muscle strength in non-hemorrhagic stroke patients. Methods: The application was carried out using a descriptive method with a case study design in non-hemorrhagic stroke patients who are at risk of experiencing muscle weakness. The application of the rubber ball rom exercise was carried out for 4 consecutive days by holding the rubber ball for 5 seconds and repeated 7 times. Results: There was an increase in muscle strength of 1 degree in both respondents after the rubber ball ROM exercise. Conclusion: The results of the application to the two respondents between before and after being given the rubber ball ROM exercise showed an increase in muscle strength. So that rubber ball ROM exercise can be used as a non-pharmacological technique to increase muscle strength in stroke patients.

Background:Nearly 70-94% of acute stroke patients experience increased blood pressure (hypertension). Hypertension is the main triggering factor for the occurrence of strokes, both hemorrhagic and ischemic strokes, one way is to provide non-pharmacological therapy with dhikr relaxation (remembrance of Allah). Objective: to find out the application of dhikr therapy to reduce blood pressure in acute stroke patients at HCU Neuro Anggrek 2 RSUD Dr. Moewardi Surakarta. Method: the application of this journal uses a descriptive method with a case study approach to 2 respondents according to the inclusion and exclusion criteria, the independent variable of the research is dhikr therapy, namely therapy that is carried out by reminding respondents to Allah SWT through recordings of reading recitation sentences, by adjusting the position of the respondent first first, namely supine position with head elevation of 30o, then listen to the recorded dhikr sentences using hands-free to the respondent 3 times a day for 10 minutes for 3 days. Results: Based on the results of the application that has been carried out, there is a decrease in blood pressure before and after the dhikr relaxation therapy is performed. Conclusion: There is a decrease in blood pressure before and after the application of dhikr from first degree hypertension to prehypertension.

Leukemia is a progressive malignant disease of the blood-forming tissues. Leukemia is caused by abnormal growth of white blood cells (leukocytes) in the bone marrow. These cells then spread in the blood circulation. This disease is still a public health problem in the country and in the world, which affects children to adults, leukemia is a cancer with the highest death cases in the world. One of the complementary therapies that can be used to improve the psychological response of cancer patients is music therapy. Music therapy can increase relaxation and reduce anxiety in leukemia patients during the chemotherapy process. Objective : Knowing the results of the application of music therapy to hemodynamics in children with leukemia in the room Flamboyan 9 RSUD Dr. Moewardi surakarta. Method : Methods of data collection in this application is to use the method of interviews, and observation. Findings : After performing music therapy for 1 day, the results were the same for both respondents, that is, children can lower blood pressure and pulse frequency within normal limits. Implication : Music therapy can affect the decrease in hemodynamics in children with leukemia because there are significant changes in hemodynamics from height to decrease.

Background; Effleurage massage is a form of massage using the palms of the hands that apply gentle pressure to the surface of the body in a circular direction repeatedly. The number of mothers giving birth in the Ponek room (Maternity Room) with January 01, 2021 - July 31, 2023 amounted to 1047 patients and 15 patients gave birth normally. Objective; To determine the results of implementation in the application of Effleurage Massage techniques to reduce the pain scale in postpartum mothers in the Ponek Room of Dr. Moewardi Surakarta Hospital. Method; This type of research is descriptive in the form of a case study. The approach taken uses a nursing care approach, planning, implementation and evaluation. The application instrument uses the Numeric Rating Scale (NRS) and checklist sheet as a measuring tool to determine the level of pain scale. Results; The results of the application using Effleurage Massage techniques found that respondents had a pain scale of 4 (moderate pain) down to 0 (none pain). Conclusion; There is a difference in pain scale before and after Effleurage Massage at Dr. Moewardi Surakarta Hospital.

Background: Hallucinations are false perceptual distortions that occur in maladaptive neurobiological responses, sufferers actually experience sensory distortions as real things and respond accordingly. Hallucination sufferers will find it difficult to distinguish between stimuli that arise from internal sources such as thoughts, feelings, somatic sensations and external impulses and stimuli. According to the World Health Organization 2019 mental disorders around the world, according to data, there are 264 million people experiencing depression, 45 million people suffering from bipolar disorder, 50 million people experiencing dementia, and 20 million people experiencing schizophrenia. In this study, how to see an increase in the ability to control hallucinations by using the AHRS scale used to measure the score of auditory hallucinations Objective: Determine the results of the application of generalist hallucination therapy with increased ability to control hallucinations with hallucinatory sensory perception disorders Method: The application of generalist therapy was carried out using a case study descriptive method to 2 respondents for 2 consecutive days. The instrument used is the Auditory Hallucinations Rating Scale (AHRS). Results: based on the results of the application that has been carried out, there is a decrease in the symptoms of auditory hallucinations in patients, with a comparison of the final results between the two respondents for the level of auditory hallucinations, namely 29:34. Conclusion: Generalist therapy can be used as an intervention to improve the ability to control hallucinations in patients with auditory hallucination psychiatric disorders.

In 2019 the total number of tuberculosis cases in North Sumatra was recorded at 99,398 cases, while in 2020 there were 78,698 cases and in 2021 there were 57,247 cases. This study aims to look at the relationship between knowledge, attitudes and family support with the risk of pulmonary TB transmission. This type of research is survey research which is analytic in nature. Cross sectional research design. The sample is pulmonary TB patients in the Working Area of the Kartini Public Health Center, Pematang Siantar City for the period of October 2022 as many as 44 people. Data analysis using chi square. The results of the study concluded that there is a relationship between knowledge, attitudes and family support with the risk of pulmonary TB transmission in the family. The role of health workers and Community Health Centers is as a source of correct information through applicative counseling by giving leaflets to pulmonary TB patients.

Background: Prevalence of patients with mild cedar heads recorded from March to July 2023 in the Emergency Room of RSUD Dr. Moewardi as many as 757 cases, and most of them complained of pain in the head, pain in CKR patients by nurses can be treated with pharmacological and non-pharmacological therapy. Non-pharmacological therapy such as behavioral therapy (relaxation, hypnotherapy, biofeedback). One of the relaxation techniques used is slow deep breathing. Objective: Changes in the application of the Slow Deep Breathing technique to reducing pain scale in mild head injury patients at RSUD Dr. Moewardi. Method : the method used in this application is a descriptive method. In this application, pain scale measurements were carried out using the Numeric Rating Scale both before and after slow deep breathing was carried out. Findings : the results of the implementation that has been done, it was found that respondent I experienced a decrease in the pain scale from a scale of 8 to a scale of 7, respondent II experienced a decrease in the pain scale from a scale of 6 to a scale of 4. Implication : slow deep breathing can reduce the pain scale in patients with mild head injuries at Dr. Moewardi.

Old age is one of the risk factors that affect hypertension because the higher the age, the higher the risk of getting hypertension, this is caused by natural changes in the body that affect blood vessels, hormones, heart. by using non-pharmacological treatment, namely the administration of cucumber infused water and acupressure therapy at the point of LI 4 (Hegu) can reduce blood pressure for people with hypertension.  Purpose : This study aims to determine the effectiveness of cucumber infused water and acupressure therapy at the point of LI 4 (hegu) on reducing hypertension in the elderly. Methods: The population in this study was the elderly affected by hypertension with a total of 240 patients. In this study, the sample number was 30 respondents with each intervention group of 15 respondents. The type of this study is quantitative research that uses a two-group pretest-postest experimental model and is analyzed using statistical tests. Results: Based on the results of the study, it can be concluded that the results of the Wilcoxon test data there is an effectiveness between the administration of cucumber infused water and acupressure therapy at the LI4 (Hegu) point against reducing hypertension. With the results of v – palue 0.003 for the results of cucumber infused water against reducing hypertension and p-value 0.002 for the results of giving acupressure therapy at the LI4 point (Hegu) against reducing hypertension. In this study, there was a significant influence before and after intervention and blood pressure measurement was given in the working area of the Karangtengah Health Center. Conclusion : the effectiveness of cucumber infused water intervention and acupressure therapy at the LI4 (Hegu) point for the elderly affected by hypertension in the working area of the Karangtengah Health Center.

Parkinson's disease is a disorder of brain function caused by the process of degeneration of the basal ganglia in the substantia nigra pars compacta (SNc) cells and is characterized by characteristics such as tremor at rest, rigidity of muscles and joints (rigidity), slowness of movement and speech (bradykinesia) and instability upright position (postural instability). Parkinson's Disease occurs in the range between the ages of 55 to 65 years and occurs in 1% -2% of people over the age of 60 years, increasing to 3.5% at the age of 85-89 years. Approximately 0.3% of the general population, the prevalence is higher in males than females with a ratio of 1.5:1.0. Manifestations that can be found in patients with Parkinson's Disease can be assessed through the Hoehn and Yahr Scale (HY). Current treatment for Parkinson's disease aims to reduce motor symptoms and slow the progression of the disease.

Background: Hyperthermia is a condition where the body temperature is higher than normal because the body is unable to dissipate excess heat in the body, which threatens to cause fever. The results of the medical records of the children's inpatient room at Dr Socratno Gemolong Hospital, the number of children treated at the Anggrek ward who experienced fever, namely preschool aged children in the last 4 months, was 116 patients. Efforts to reduce fever can use Water Tepid Sponge. Water Tepid Sponge is a warm compress action using wipes and blocks not only in one place. Objective: The aim is to find out the results of the implementation of giving Water Edged Sponge to body temperature due to fever. Methods: using a case study design, subjects were 2 pre-school aged children who were treated in the orchid children’s room at dr. Soeratno Gemolong hospital. The instrument used to measure body temperature is a thermometer. Application of water tepid sponge was carried out for 3 days. Findings: water tepid sponge is able to reduce body temperature in pre-school aged children (1-3 years) with hypertermia. Implication: there are dfferences in the develoment of decreased body temperature in pediatric patients with hypertermia before and after the application of the water tepid sponge. So that the action of water tepid sponges can be used as a non-pharmacological technique to reduce body temperature in pre-school aged children (1-3 years) with hyperthermia.

Background: Hypertension is a health problem in all parts of the world and one of the main risk factors for cardiovascular disease. It is estimated that 1.28 billion adults aged 30-79 years worldwide suffer from hypertension as well as the Karanganyar District Hospital where there have been 889 cases with hypertension since the last 1 year. One of the non-pharmacological therapies that can be used for hypertension is foot massage. Foot massage is one of the complementary therapies currently used for hypertension. Objective: To find out the results of the implementation of giving foot massage to reducing blood pressure in 2 respondents with hypertension at the Karanganyar Regency Hospital. Methods: Using a descriptive method with a case study design approach, respondents used 2 hypertensive patients. Determination of respondents according to the inclusion and exclusion criteria was carried out for 3 days. Foot massage and blood pressure measurement using a sphygmomanometer Results: Providing foot massage was able to reduce blood pressure in 2 respondents at the Karanganyar District Hospital Conclusion: There were changes in 2 respondents after foot massage was carried out for 3 days at the Karanganyar District Hospital.

Electronic-based nursing care in hospitals refers to the application of information technology and electronic systems in the process of providing care for patients. There are various impacts in the use of electronic nursing records that have implications for patient safety. The purpose of this study was to review the impact of using electronic-based nursing documentation on patient safety and quality of nursing care. The method used is literature review by searching articles in three electronic databases: PubMed, ProQuest, and Science Direct. The inclusion criteria used were articles that matched the research objectives, according to the specified keywords, were in English, full text and published until 2022. There were eleven research articles that met the inclusion criteria. The results of a literature review on the eleven articles illustrate that electronic-based nursing documentation has a positive impact on patient safety and the quality of nursing care. The use of electronic nursing documentation provides various benefits, such as efficiency in managing patient data, increasing accuracy and speed in processing information, and improving coordination and communication between health workers. In addition, two articles also convey the impact of using electronic nursing documentation which is considered detrimental in an effort to maintain patient safety. Thus, the information in this literature review can be useful for planning the development of strategies for implementing electronic-based nursing documentation in hospitals.

The non-pharmacological techniques that can be used to reduce postoperative pain is early mobilization techniques. Early mobilization techniques are effective in reducing pain through several mechanisms, including eliminating the patient's concentration at the location of the pain in the operating area, reducing the activity of chemical mediators in the inflammatory process which increases the pain response. and minimize pain nerve transmission to the central nervous system. This study used a quasi-experimental design with the pretest-posttest Nonequivalent Control Group design methodwithcase study descriptive method to 2 respondentsPost Sectio Caesarea. Based on the results of the implementation that has been done there is a decrease in pain intensity in patientspost sectio caesarea after being given the application of early mobilization.

Background: Cervical cancer is a type of deadly malignant cancer that attacks women. So that it can affect the quality of life for sufferers and Self Efficacy is needed. Purpose: Knowing the relationship of self-efficacy to the quality of life of cervical cancer patients. Research method: Quantitative research design with a sample of 50 respondents, the technique used is purposive sampling. Results: Based on age, education, and length of illness, bivariate results obtained using the sperman rank correlation test showed that the p-value was 0.002 (<0.05) meaning that there was a significant relationship between self-efficacy and the quality of life of cervical cancer patients. The correlation coefficient value obtained was 0.429, meaning that the relationship between self-efficacy and the quality of life of cervical cancer patients showed a moderate relationship with a positive direction, meaning that the higher the self-efficacy, the better the quality of life of cervical cancer patients. Suggestion: Future researchers can focus on one respondent who is undergoing chemotherapy or who is currently on an outpatient basis.
