This study discusses the causes of leakage in cargo oil pumps, which play a crucial role in the discharge operation of tankers carrying oil, chemicals, and gas. Regular maintenance is essential to prevent damage that could hinder the ship's operations. The method used is descriptive qualitative, with data gathered from observations, interviews, documentation, and literature review. The analysis is conducted using Fishbone to identify the root causes of the problem. The results show that leakage occurs due to the misalignment of the pump shaft. This misalignment causes excessive vibration at high speeds, damaging components, especially the carbon seal face of the mechanical seal, leading to leakage of cargo. Preventive measures include regular and scheduled maintenance, proper lubrication, routine inspection while the pump is running, and alignment checks to ensure the shaft is straight. If any issues are found, repair or replacement with new parts is necessary to ensure the pump operates efficiently and the discharge process runs smoothly.

Intisari-Penelitian dilatarbelakangi oleh munculnya ragam kecelakan kerja yang terjadi pada saat melakukan dalam proses bongkar muat pupuk pada MV. Mochtar Prabu Mangkunegara di pelabuhan Pusri Palembang. Hal tersebut disinyalir karena adanya kelalaian dan kurangnya pengawasan terhadap para pekerja dalam pemakaian dan perawatan peralatan yang digunakan untuk proses bungkar muat pupuk. Tujuan dalam penelitian ini adalah mendeskripsikan faktor penyebab terjadinya kecelakaan kerja dalam proses bongkar muat pupuk dan mendeskripikan upaya pencegahan terjadinya kecelakaan kerja pada MV. Mochtar Prabu Mangkunegara. Metode yang digunakan dalam penelitian ini adalah deskriptif kualitatif dengan teknik analisis data diagram fishbone untuk menjelaskan secara rinci mengenai faktor- faktor penyebab serta upaya pencegahan kecelakan kerja. Hasil analisis menunjukkan bahwa faktor penyebab terjadinya kecelakan kerja meliputi manusia, material atau mesin, lingkungan, serta dan prosedur pelaksanaan kegiatan kerja. Setiap faktor tersebut dapat diatasi melalui beberapa upaya yaitu dibuatnya prosedur kerja yang efektif, pengawasan kerja, inspeksi secara rutin, pemeliharaan peralatan kerja, penyuluhan keselamatan, serta persuasi.

XYZ is a manufacturing company that produces Polypropylene Woven Bags (plastic sacks). This company uses a make to order production system. The aim of this research is to analyze the consequences of damaged products for plain white sacks and printed sacks in the cutting sawing division. The method used in this research is the seven tools method. From the seven tools analysis, namely the plain white BS sack type at 52% and the printed BS sack type at 48% and the most dominant was the plain white BS sack type with a number of defects of 2,333 pcs or 256.63 kg of sacks for 27 days in the cutting division. sawing. And from the results of the fishbone diagram, there are 5 main factors, namely: humans, machines, materials, methods and environment. The most dominant cause is the human factor. Operators who are less thorough and dexterous, operators who are less competent result in sacks being made that are not perfect, therefore quality control must be carried out on the product.

CV Harico is a manufacturing company that produces briquetted charcoal final products. In this research, the company had problems with the quality of charcoal briquettes which did not meet standards, amounting to 12% of the production process, including slanted shapes, holes and cracks. In this research, problem solving is done by combining the Six Sigma and TRIZ methods to get suggestions for improvement. The Six Sigma method is an organized and systematic method, Six Sgma has 5 stages, which can be known as the DMAIC phase. Based on the calculation of the DPMO value and the calculation of the sigma value at the measurement stage, the average DPMO value for October 2023-November 2023 was 27219 with a sigma value of 3.43. Based on the identification results using the Fishbone Diagram method and the FMEA method, the largest defects were found in the slanted shape of the charcoal briquettes, these results were obtained from the factor with the highest RPN value that caused defects. Proposed improvements based on the TRIZ method analysis include providing training, carrying out periodic actions, making maintenance schedules, evaluating work processes, changing tools to reduce the occurrence of defects. The aim of this research is to analyze the problems that occur as well as efforts to control quality and reduce product defects in briquetted charcoal products.

PT. Diamondfit Garment Indonesia is a company operating in the apparel industry, especially intimates and swimware. One of the products is the Intimates fused bra. In the production process, many defects are still found. This research aims to propose quality improvements using the six sigma method starting from the define, measure, analyze, improve and control stages. At the define stage, the production process is identified using the SIPOC diagram and identification of the type of defect. At the Measure stage, the calculation stage is carried out: fusing delamination DPMO value 366405.6 with a sigma value 1.84, Wavy At Neckline & Armhole DPMO value 65274.15 with a sigma value 3.01, Molding Yellowish DPMO value 350739.8 with a sigma value 1, 88, Dirty Glue has a DPMO value of 26109.66 with a sigma value of 3.44, and Heat Seal Not Sticking has a DPMO value of 191470.8 with a sigma value of 2.37. As a result of the analysis, it was found that factors causing defects occurred using FMEA tools and fishbone diagrams, namely machine, manpower, material and method factors. In the improvement stage, improvements are made to the factors causing defects and in the control stage, supervision is carried out over the proposed improvements that have been made by creating SOPs.

Arthantra Batu Bata, produsen batu bata expose di Klaci III rt 02 rw 12, Margoluwih, Sleman, DIY, menghadapi permasalahan material handling karena tata letak pabrik yang tidak efektif. Total jarak mingguan mencapai 13.848 meter dengan ongkos material handling sebesar Rp1.423.210,24. Dalam tugas akhir ini, dilakukan evaluasi dan perancangan tata letak baru menggunakan SLP, Blocplan, dan Fishbone diagram. Layout Usulan 1, hasil dari evaluasi, memiliki jarak 9.610,44 meter, mengurangi 31% dari layout awal. Ongkos Material Handling (OMH) sebesar Rp1.374.426,47, menghemat 3%. Alat material handling yang diusulkan adalah Handtruck, memungkinkan penghematan OMH sebesar Rp1.175.153,58 atau 14,5% dari layout usulan 1. Perubahan ini bertujuan untuk meningkatkan efisiensi operasional dan mengurangi biaya material handling, menghadirkan solusi konkret bagi Arthantra Batu Bata.

Quality is an important factor in production, so companies must maintain the quality of their products in order to be able to compete in the market. One of them is PT. Swadaya Graha operates in the construction and fabrication services sector, working on several projects, one of which is pipe support. In pipe support fabrication there are problems during the production process. This problem is the presence of "scratch" defects on the surface of the material. The aim of the observations carried out is to minimize the occurrence of production process defects in pipe support fabrication. From the results of the analysis using a cause-and-effect diagram, there are 3 factors that cause defects. Warehouse is an important part of a company. Warehousing activities must have a good storage system to support the smooth production process and warehousing activities. PT. XYZ is a manufacturing company in the field of paper production that uses 100% recycled paper as its raw material. PT. XYZ has many warehouses needed for production. Stock taking is an activity of physically counting the inventory of goods in the warehouse or production to determine the real or actual quantity owned by the company. Stock taking is important to carry out control, so that the company can monitor and know the assets it owns. By carrying out comprehensive stock taking activities, real stock reports will be obtained which will be used as material for consideration for further development. One of the problems that often occurs in spare part warehouses is a mismatch in the final balance of spare part components between the stock card and the inventory system which occurs continuously. This problem can cause errors in stock taking reports which can result in losses for the company. This research uses the fishbone diagram method. This diagram is often called a cause and effect diagram. This fishbone diagram is used to analyze and look for factors causing problems that have a significant impact on differences in inventory. Human, environmental, material and method factors are factors that cause differences in existing inventory. Human factors such as drowsiness and lack of concentration can also be caused by environmental factors such as lack of air circulation, causing the temperature in the spare parts warehouse to increase. With the increasing era of globalization today, competition in the industrial sector is becoming increasingly fierce and competitive. With the development of modern technology for the better but on the other hand raises problems for the environment. Every business actor who wants to win the competition must pay full attention to the quality of the products or services produced, at a low cost, the right time and quantity, and safe conditions. This study aims to analyze and reduce waste of packaging waste at PT XYZ, a manufacturing company engaged in the manufacture of medicines and vaccines. In the last 7 months, there were 1,906 pcs of defective packaging out of a total of 204,940 pcs used. The defects were caused by mold, misprinted information, faded colors, and lack of glue on the packaging. The methods used in this research are Pareto diagram and fishbone diagram. Pareto diagrams are used to identify dominant product defects, while fishbone diagrams are used to identify the causes of defects. It is expected that the results of this research can help PT XYZ reduce packaging waste and improve the quality of their products. By reducing packaging waste, PT XYZ can also contribute to reducing negative impacts on the environment.

Geographically, Kadirejo village is located to the south of the Karanganom sub-district capital, which is still an integral part of the Klaten Regency area with a distance of 2 km from the sub-district capital and 7 km from the district capital, while from the provincial capital it is around 100 km. The Kadirejo village government has several parts of land that will have no access due to the closure of the Solo – Jogja toll road. To get around this, the Kadirejo Village Head wants to create a new tourist spot in the form of a Tiban market and fishing pond so that the village remains busy and increases the village's cash fund income through Bumdes. For this reason, the village head collaborated with academics to participate in planning the idea. The purpose of this community service is to help the community provide a brief description of partner problems and the goals of community service. The methods used are field surveys, data processing and reports. The final result is suggestions or recommendations regarding parking patterns and types of access roads to the tourist location in question. The recommended parking pattern is an island parking pattern with a type A fishbone shape (60° angle), and the road type is paving.

Quality control is crucial for the company to minimize the existence of defective products. The research focuses on the quality control section. The purpose of this study is to find out how the implementation of quality control occurs at PT Andalan Mandiri Busana, identify the factors that cause defects in garment products, and determine proposed improvements as an effort to suppress defective products. The results showed that there were factors that caused production defects and the biggest cause was broken stitches, and the sigma value obtained was 4.15, which means that the company has carried out pretty good quality control. Based on this research, the recommendations that can be given are in the improve stage, in the form of a table of proposed improvements using the Five M-checklist approach and Kaizen based on fishbone diagrams. The results of this study are expected to be one of the inputs for companies in implementing quality control and can increase the company's sigma level.

AUTO2000 Basuki Rahmat is one of the branches dealer Toyota, which is located in the city of Surabaya, is engaged in services such as sales, maintenance, repair and supply of Toyota spare parts whose management is fully handled by the Astra Group. The company always strives to provide fast and accurate service, but customers spend quite a long time waiting because the car service process is relatively long. The long service time is caused by an unorganized operational system that causes waste (waste). Based on these problems, it is necessary to analyze Basuki Rahmat's car service services at AUTO2000 using the concept lean service by using the method value stream mapping, fishbone, and 5 whys. The research results obtained weight waste motion or transportation of 0.1481; waiting or delay of 0.1325; incorrect inventory of 0.1273; lack of customer’s focus of 0.1195; defect of 0.1065; duplication of 0.0935; overproduction of 0.0779; unclear communication of 0.0675; underutilized employees of 0.0675; And variation of 0.0597. Research recommendations that can be proposed to reduce waste including repairs layout, adding monitors on the service floor for matters of sending and receiving information, increasing the number of mechanics, making plans booking with different hours, making and applying the system tracking of the presence of the tool. Therefore, lead time the initial service service process of 170 minutes becomes lead time 128 minutes.

PT KMPI is an industrial company that produces textiles and plastics, including plastic products made of polypropylene (PP). The problem that occurs in PT Kusuma Mulia Plasindo Infitex is the quality of the products that still have defects, based on the results of the analysis in February 2023 with a total production of 76,596 kg there are rejects data of 4,695 kg and the percentage of product defects that occur is 6.1%. The objective of this study is to determine the predominant types of defects that influence the factors that cause defects. This study uses the seven tools method to find the root cause of the problem to be solved, as well as the Kaizen Five-M checklist and the Kaizen Five Step Plan to suggest improvements to the company. Based on the results of data processing, there are three types of defects, namely wrinkle defects with a percentage of 43% for a total defect of 2039 kg, medium wrinkle defects with a percentage of 45% for a total defect of 2094 kg, and thick-thin defects with a percentage of 12% for a total defect of 562 kg. From the fishbone diagram, it can be seen that the occurrence of defects is caused by several factors, namely people, materials, methods and machines. Suggested improvements using the Kaizen Five-M checklist analysis are training and monitoring of production, implementation of SOPs (Standard Operating Procedures) established by the company, such as dosing the mixture of plastic seeds with addictive substances at a ratio of 25 Kg. 1 oz. and Kaizen management control: 1 Ons and Kaizen five-step plan management controls, namely Seiri, Seiton, Seiso, Seiketsu, Shitsuke. This includes pre-job briefings and periodic assessments, and the establishment of standard operating procedures for the supervision of single-table melting machines with a melting temperature of 160-166oC.

Kualitas merupakan salah satu komponen bagi industri agar dapat bertahan dan bahkan dapat unggul dalam persaingan bisnis yang semakin kompetitif ini, sehingga perlu menjaga kualitas produk agar mematuhi serangkaian kriteria persyaratan kualitas yang ditentukan atau memenuhi persyaratan pelanggan, perlunya dilakukan pengendalian kualitas. Penelitian ini bertujuan untuk melakukan pengendalian kualitas agar dapat mengurangi  cacat produk coran aluminum alloy wheel pada proses casting dengan menggunakan metoda Seven Tools, yang dilanjutkan langkah perbaikannya dengan menggunakan konsep PDCA. Hasil Analisa menggunakan diagram pareto , dan diagram sebab akibat , yang dilanjutkan tindakan perbaikan dengan konsep PDCA, menunjukkan  bahwa cacat shrinkage yang merupakan cacat produk coran terbesar, yang semula 7,17% , turun menjadi 3,95%. Sedangkan secara keseluruhan prosentase  cacat produk coran yang awalnya 12,89% turun menjadi 6,61% . Penurunan prosentase cacat produk coran yang penting ini setelah rekomendasi langkah perbaikan telah dilaksanakan dengan baik, pelaksanaan evaluasi hasil tindakan perbaikan, serta menjadikan langkah perbaikan kerja sebagai pedoman untuk kerja selanjutnya, diantaranya yaitu pelatihan karyawan dan pengawasan kerja karyawan, pengendalian parameter proses casting dan melting yang terutama  pemeriksaan pada perbedaan ketebalan di tiap posisi rim contour,   dan pengawasan berkala temperatur saat   logam cair dituang  ke dalam cetakan.   Keywords: Fishbone diagram: Pareto diagram: PDCA; Quality control; Seven tools

Sebuah ketel uap  harus cukup  kuat  supaya  dapat  bekerja  dengan  aman pada tekanan tertentu, sehingga perlu dilengkapi pesawat-pesawat keamanan. Masinis di atas kapal mempunyai tuntutan agar secara rutin melakukan perawatan pada ketel uap bantu dengan berpegangan pada manual book yang sudah dibuat maker. Berdasarkan pengalaman saat berlayar di salah satu kapal ditemukan suatu kejadian bahwa auxiliary boiler beroperasi serta mengeluarkan uap bertekanan yang berguna untuk kecukupan kebutuhan uap di atas kapal. Bunyi pada alarm yang setelah diperiksa pada panel auxiliary boiler, ternyata alarm muncul pada flame failure dan lowest water level, sedangkan feed pump beroperasi normal serta masih dalam keadaan mengisi air ke dalam auxiliary boiler. Penelitian ini dilakukan untuk mengetahui faktor yang dapat mengakibatkan air masuk ke dalam furnace, mengetahui dampak yang dapat terjadi jika air  masuk ke dalam furnace serta mengetahui langkah yang dapat diambil untuk mengatasi masuknya air ke dalam Furnace. Metode penelitian ini menggunakan metode kualitatif. Sumber data diperoleh dari data primer maupun data sekunder. Teknik pengumpulan data yang  digunakan  adalah  wawancara,  observasi,  maupun  dokumentasi.  Teknik  analisis  data  yang digunakan adalah fishbone diagram dan USG. Hasil penelitian menunjukkan bahwa masuknya air ke dalam auxiliary boiler  disebabkan oleh retaknya dinding  furnace. Dampak  yang  ditimbulkan  yaitu terganggunya proses pembakaran di dalam furnace serta kerusakan pada komponen-komponen burner. Upaya yang dapat dilakukan adalah dengan mengelas bagian dinding furnace yang retak agar air tidak masuk ke dalam furnace.

The COVID-19 pandemic had a significant impact on UMKM. Social distancing has changed consumer behavior from conventional to digital. UMKM Shapro Official is an original automotive t-shirt vendor that was established in 2017. Shapro Official has difficulty in introducing and marketing products with a clear target consumer and competitive price competition. Sales promotions have not been managed optimally and sales data management is still manual (notes). Owners also have difficulty monitoring sales reports in real time. Website created using CMS Wordpress to help Shapro Official difficulties. The system development method is Rapid Application Development (RAD). System analysis method using fishbone diagram and system modeling using Unified Modelling Language (UML). The results showed that the existence of a website can help the management to manage promotions, sales and sales reports. Having a website makes sales targets focus and increase.

Pengabdian dilakukan dengan wawancara dari berbagai perwakilan untuk menggali permasalahan tentang Tanggung jawab lembaga social dan Perusahaan (TJSLP) atau Corporate social responsibility (CSR). Setelah mendapatkan hasil dari wawancara dilakukan analisa dengan menggunakan fishbone diagram dan hasilnya menunjukan bahwamasalah komunikasi diantara pemangku kepentingan masih kurang, kebudayaan dan sanksi menjadi coding yang harus diperhatikan dalam permasalaan tidak maksimalnya TJSLP yang selama ini telah dilakukan.komponen yang dibutuhkan dalam komunikasi terdiri dari Informasi SCR, Transparansi, Konsisten, Relevansi pribadi, Pesan yang disampaikan. SIMRS is used in the patient registration process, one of which is the registration of the patient in the hospital. The stages of hospital patient registration are pre-admission, elective admission, and payment. The initial registration of patients in the hospital is only done through two doors: the Poliklinik and the UGD . (Unit Gawat Darurat). One of the problems with the registration of hospital patients is that (1) the absence of a hospital patient registration flow from the Poliklinik or IGD causes confusion for the patient or their family. (2) Patients and families complain about hospital registration at the polyclinic because there are frequent errors and long loadings on the SIMRS. (3) The hospital admission space is too small or closed. Methods used in research II are qualitative methods with case study approaches. The instruments used are interviews and observations. The study conducted analysis using the methods of ultrasound and fishbone to determine the priority of the problem, and the implementation of the hospital patient registration process was carried out by applying acrylic walls in the hospital admission section and conducting observations for 2 days that showed that there was a level of awareness or information given to the patient about the process and the course of hospital patient registration. Students can make a design of the patient's registration cycle from a poliklinic, and based on the results of observation over two days, it is known that there are 15–17 patients who are responsible for having observed the registry cycle of hospital patients from the poliklinic.

This study aims to examine and evaluate the results of the implementation of an android-based labor payroll system. The purpose of this research is to find out the description of the system, application implementation, problems, and solutions on android-based payroll at Mitra Indo Agro. The results of the study obtained are used to evaluate the factors and parts of the payroll system for this android-based application that needs to be improved. This study uses a qualitative descriptive research method. Determination of informants using snowball sampling technique where the research subjects as many as 4 employees who are directly related to the android-based payroll process. Data collection techniques are through field studies consisting of interviews, observations, documentation, and literature studies. Data analysis techniques through data reduction steps, data exposure, data interpretation and drawing conclusions. The data validation technique uses source triangulation. The data presentation technique uses a program flow (flowchart) and fishbone diagrams (fish bones). The results of the research have shown that the Android-based GajiQ system helps provide large databases with the CRUD (Create, Read, Update, Delete) method , automate reports such as attendance recap, salary calculations, payroll history reports and make it easier to find labor data. The implementation of the Android-based GajiQ application can be a new medium of choice for companies to manage payroll in real time, quickly, and accurately that does not involve many parties in making payroll, payments, and payroll procedures. Problems found by the system are still bugs or crashes, error messages such as app force close appear.
