Patients who are admitted to the Intensive Care Unit (ICU) often require mechanical ventilation due to respiratory failure. One of the common problems that arises in patients with mechanical ventilation is the buildup of sputum, which can interfere with the ventilation process as well as lower the tidal volume. To overcome this, a combination of clapping and suction interventions are used as a therapeutic method with the aim of helping to clear the airways and improve ventilation function. This study aims to determine the effect of the combination of clapping and suction on tidal volume in patients with respiratory failure using mechanical ventilation. The research design used a pre-experiment with a one-group pretest-posttest approach on 43 respondents in the ICU room. Interventions in the form of clapping and suction are given in a structured manner according to standard nursing procedures. The results showed that the majority of respondents were female (53.5%), with the most medical diagnosis being SNH (23.3%). Most of the respondents were in the late adult age category (32.6%) and used PCV ventilation mode (51.2%). Before the intervention, most respondents had a low tidal volume (72.1%), while after the intervention the majority had an increase to the normal category (90.7%). The Wilcoxon test showed a value of p = 0.001, which means that there is a significant effect of the combination of clapping and suction on the increase in tidal volume. Thus, it can be concluded that the combination of clapping and suction is an effective intervention in increasing tidal volume in patients with respiratory failure with mechanical ventilation at the ICU of RSI Sultan Agung Semarang.

This research aims to develop and evaluate the performance of a steam plant prototype designed as an alternative source of electrical energy to support the Vessel to Grid (V2G) concept. Utilization of backup energy on ships is becoming important as electricity demand increases and demands for a more sustainable electrical system. This system relies on ESP32 microcontroller technology as a control center that functions to monitor and control several key parameters, including steam pressure, combustion temperature, boiler water level, and the generated electrical voltage. The research method used is an experiment with a static and dynamic testing approach. Static testing is carried out to measure the performance of main components such as the boiler, turbine, and generator separately, while dynamic testing focuses on evaluating the overall system by involving the integration of sensors and supporting actuators. The test data is then analyzed quantitatively to determine the system's response to variations in steam pressure, temperature, and other operational conditions. The results show that the steam produced by the boiler is able to rotate the turbine, thereby driving the generator to produce electricity. The maximum voltage achieved is 25.7 volts at a steam pressure of 50 psi. The highest energy conversion efficiency was recorded at 4%, while the lowest efficiency was 0.9%. These findings demonstrate that, despite its relatively low efficiency, the prototype can function as an alternative energy source and emergency backup solution. Thus, this research provides an initial contribution to supporting the implementation of the V2G concept through the development of a small-scale steam plant-based energy conversion system.

This article examines the legal consequences of a delayed notification of a share acquisition under Indonesian competition law, focusing on the case of PT X's takeover of PT Y. The central issue is PT X's failure to report the acquisition to the Commission for the Supervision of Business Competition (KPPU) within the mandated 30-day period, as stipulated by Law Number 5 of 1999 and its implementing regulation, Government Regulation Number 57 of 2010. The notification was submitted with a significant delay of 2,023 days, a clear violation of the statutory requirements. As a result of this non-compliance, the KPPU, through its Decision No. 31/KPPU-M/2020, imposed an administrative fine on PT X amounting to   IDR 1,050,000,000.00 (one billion fifty million rupiah). Beyond the direct financial penalty, the violation led to severe indirect repercussions, including significant    reputational damage, which threatened to erode investor confidence and disrupt existing business relationships. The KPPU's firm and consistent enforcement in this case highlights the critical function of the post-merger notification system in Indonesia as a preventative mechanism to assess transactions that could lead to excessive market concentration. The decision underscores the legal principles of     transparency, fairness, and accountability that underpin Indonesian competition law. This case serves as a powerful deterrent, signaling to all business actors that non-compliance will not be tolerated. Ultimately, such rigorous enforcement is crucial for fostering a culture of legal adherence, maintaining market integrity, and building a more transparent and competitive business ecosystem that supports sustainable economic growth in Indonesia.

Sidewalks are a crucial element of urban infrastructure that support pedestrian mobility. However, on Jalan Dr. Mansyur, Medan Selayang, sidewalk conditions present various problems, such as damaged surfaces, a lack of accessibility facilities, and the presence of street vendors occupying the lanes. This study aims to analyze the impact of sidewalk quality on pedestrian activity by examining three main factors: the physical condition of the sidewalk, environmental safety, and accessibility. Using a mixed methods approach, the sample for the quantitative approach was taken using a stratified random sampling method with a total of 150 respondents. Quantitative data were analyzed using descriptive statistics and SPSS software, while qualitative data were collected through in-depth interviews with key informants. The results showed that sidewalk accessibility had the strongest relationship with pedestrian activity (correlation coefficient 0.72), followed by the physical condition of the sidewalk (0.68) and environmental safety (0.55). Minimal accessibility factors, such as the lack of special lanes for the disabled, proved to be the main obstacle to pedestrian activity. In addition, the damaged physical condition of the sidewalk and the use of sidewalks for activities that are not in accordance with their function further reduce user comfort. Based on these findings, key recommendations include physical improvements to sidewalks with regard to the principles of universal accessibility, including the addition of dedicated lanes for the disabled, repair of damaged surfaces, and reconfiguration of spaces to discourage use by street vendors. The government needs to ensure adequate street lighting to improve user safety. The public is expected to maintain the function of sidewalks by utilizing them according to their intended purpose, while future researchers are advised to explore the impact of sidewalk improvements on broader social and economic aspects, as well as consider the integration of technology in sidewalk design and management. The conclusion of this study is the need for collaborative action between the government, the community, and other relevant parties to create inclusive sidewalk infrastructure that supports active mobility.

Regulatory changes through Law No. 3 of 2024 concerning the Second Amendment to Law No. 6 of 2014 stipulate an extension of the village head's term of office from six to eight years. This policy is intended to strengthen leadership stability and the sustainability of village development, but also raises concerns regarding reduced leadership circulation, weakened community political participation, and increased potential for abuse of power. This research uses empirical legal methods with sociological, conceptual, and case-based approaches, conducted through field observations and in-depth interviews with village officials, community leaders, MSMEs, and residents of Kerato Village. The results show a diverse response from the community. Some support this policy because it is seen as providing the village head with the opportunity to complete development programs sustainably. However, others reject it, arguing that term extension has the potential to reduce the quality of local democracy, limit leadership regeneration, and reduce the frequency of community political participation. The discussion emphasizes the dilemma between village government stability and the principles of participatory democracy. In conclusion, this policy is ambivalent: positive in maintaining development continuity, but risky for accountability and community participation. Therefore, strengthening the function of the Village Consultative Body (BPD), transparency, and effective oversight mechanisms are very necessary to maintain the balance between government stability and village democracy.

Enhanced Recovery After Surgery (ERAS) is a multidisciplinary, evidence-based perioperative care approach designed to minimize the stress response to surgery, preserve organ function, and improve clinical outcomes. A substantial body of evidence has demonstrated that implementing ERAS protocols in elective procedures not only accelerates patient recovery but also reduces healthcare costs. Surgery, one of the most commonly performed medical interventions worldwide particularly major procedures such as abdominal and colorectal surgery carries a high risk of postoperative complications. These complications contribute to increased morbidity, mortality, and economic burden for both patients and healthcare systems. This situation presents a particular challenge in the era of universal health coverage, which demands efficiency in terms of time, cost, and resource utilization. ERAS implementation has been proven to enhance postoperative recovery, shorten hospital stays, and expedite the return of normal physiological function compared to conventional surgical care, especially in lower abdominal surgeries and colorectal resections. A literature review was conducted by searching relevant articles through Google Scholar using inclusion criteria such as publications from 2018 onwards, focused on ERAS in abdominal surgery, full-text availability, and academic journal sources. The data were analyzed using a matrix table comparing research methods, study populations, research locations, and reported outcomes. ERAS protocols have shown to be effective in abdominal surgical procedures for improving patient recovery and reducing postoperative complications.

This study aims to understand the dynamics of children’s communication within families after parental infidelity in Subang Regency, with a focus on conflict, problem-solving strategies, and conformity. The research employed a qualitative approach using a case study method, with data collected through purposive sampling involving four key informants and five supporting informants. The findings indicate that post-affair conflicts often lead to children’s withdrawal from family interactions, while problem-solving efforts are not always successful in restoring trust and repairing family relationships due to unresolved emotional wounds. Children’s conformity tends to become increasingly closed and restrictive, with the family no longer functioning as a safe space to express feelings and share concerns. Parental infidelity significantly disrupts family communication, particularly in parent–child interactions, as children tend to feel insecure, lose trust, and adopt rigid or defensive communication patterns. This condition reflects a broader psychological impact in which children experience emotional distress, confusion, and difficulties in establishing meaningful relationships. Moreover, the study highlights that unresolved family communication breakdowns may contribute to long-term consequences, such as reduced self-esteem and heightened vulnerability to stress. The research underscores the importance of maintaining open, honest, and empathetic communication to support children’s emotional recovery after parental infidelity. It also recommends seeking professional assistance, such as counselors or psychologists, when families are unable to independently rebuild healthy communication patterns. This provides not only emotional relief but also preventive measures for further deterioration of family relationships.

Background: Stroke is a health condition that significantly impacts quality of life and causes physical and mental disability, as well as death, in both productive and elderly individuals. The loss of bodily functions in stroke patients can hinder productivity and reduce the fulfillment of ADL (Activities of Daily Living). Dependence in ADL is the inability to perform all or some activities and the need for assistance from others. The high incidence of stroke in patients and its role as a leading cause of disability makes it difficult for stroke patients to perform daily activities, causing mental and emotional disturbances, reducing productivity and quality of life. The loss of independence and mobility in stroke patients necessitates family support, as they play a crucial role in the care and rehabilitation of their loved ones. Objective: The purpose of this study was to determine the relationship between family support and the fulfillment of daily living (ADL) in stroke patients. Methods: This study employed a correlational analytic method with a cross-sectional approach. A purposive sampling technique was used for the sample, employing 89 respondents. Data analysis used the Spearman rank sum. Results: The Spearman rank sum analysis yielded a ρ value of 0.000, less than 0.05, and an r value of 0.827 (very strong). The role of the Regional People's Representative Council (DPRD) Secretariat is crucial in supporting the effective supervision function of the DPRD as a pillar of good governance. However, at the Cilegon City DPRD Secretariat, this role faces significant constraints due to a policy of assigning staff to assist in work visits that is not competency-based, which risks weakening local government accountability. This research aims to analyze the facilitation role of the Cilegon City DPRD Secretariat, identify the constraints arising from the removal of expert supervision staff, and formulate optimization strategies to strengthen the institution's role. This study uses a qualitative descriptive method, with data collected through in-depth interviews with key informants within the Secretariat, observation, and document analysis. The research findings indicate that (1) the Secretariat's role has been degraded to be administrative rather than substantive; (2) the main constraint stems from a staff assignment system that ignores competence, which leads to a decline in the quality of supervision reports; and (3) there is an urgent need to reform the human resource management (HRM) system. It is concluded that the optimization of the Secretariat's role can only be achieved by reforming the staff assignment policy and implementing structured competency development programs to restore professionalism and effective support for the DPRD's supervision function. Therefore, this research recommends three strategic steps: in the short term, revising the staff assignment policy to prioritize competence; in the medium term, developing and implementing clear Competency-Based HRM Guidelines; and in the long term, organizing continuous functional training programs to enhance staff capacity in professionally and effectively supporting the DPRD's supervision function.

The purpose of this investigation is to analyze the influence of digital transaction methods on the control mechanisms of cash revenue at the Grand Hyatt Bali Hotel. The study applies a qualitative descriptive design, where information is gathered through interviews, field observations, and the review of supporting documents, including transcripts of discussions. The findings reveal that digital payments at the Grand Hyatt Bali provide convenience to guests in conducting transactions such as transaction speed, recording accuracy, and ease in the verification process. However, the use of digital payments also has a negative impact where digital cash receipts are recorded in the accounts receivable section requiring many supporting documents making it appear less optimal and less functional and highly dependent on the availability and stability of the internet network connection. From an internal control perspective, digital payments help improve transparency and accuracy in cash recording processes. The risk of miscalculations or data manipulation is reduced because transaction evidence is readily available digitally, in the form of sales slips, system notifications, and account statements. The mechanism of internal supervision regarding cash inflows at Grand Hyatt Bali Hotel based on COSO consists of several components, namely the organizational climate, monitoring practices, hazard analysis, exchange of insights and knowledge, and supervision. Overall, the implementation of internal controls has been carried out by all staff, however, the implementation is still not optimal related to the collection of documents that are still late, this is caused by staff knowledge about the documents required when digital payments are still not executed according to the procedures outlined in the SOP.
