Background: Destruction is a process of physical destruction of medical record archives that have ended their function and use value. The knowledge of medical record officers greatly influences the ongoing process of shrinking and destroying medical record files. The main problem is the problem of inpatient medical records that are too many, resulting in storage of medical records not on available shelves, which are finally stored in cardboard boxes.Methods: Data collection techniques used are observation, interviews, and questionnaires. The sampling technique used a total saturation sampling technique of 12 PMIK. Research Results: The results of the Chi square test showed that there was a significant relationship between PMIK's knowledge regarding the eradication of Inactive RMs and the effectiveness of inpatient services, with a p-value of 0.003. A p-value of less than 0.05 indicates a significant relationship between PMIK's knowledge regarding the eradication of Inactive RMs and the effectiveness of inpatient services.Conclusion: Extermination is still unscheduled and has not been carried out because there are obstacles, namely medical record officers often work with double jobs where there is no division of tasks so that the extermination has not been carried out.

Introduction: Children's and mothers' health determines the level of family and community health. The health empowerment success relies on the maternal mortality rate and neonatal mortality rate. The delivery prevalence with the SC method increases around the world by more than 10%-15% as recommended. In Latin America and the Caribbean, the prevalence of section caesarea procedures is high, 40.5%. The data of delivery mothers with Sectio Caesarea at HA Zaky Djunaid Pekalongan reached 100 mothers. Objective: This research revealed the characteristics of delivery mothers with Sectio Caesarea from the perspectives of age, parity, gravida, gestation interval, birth history, and height. Method: This qualitative research is a descriptive retrospective type of research by reviewing the medical records of delivery mothers with section Caesarea.Population: This research population consisted of all delivery mothers with section Caesarea at HA Zaky Djunaid Hospital, Pekalongan City, from July 2023 to August 2023.Sample: The research samples were 100 delivery mothers with Sectio Caesarea taken with total sampling. Results: From the data analysis, the characteristics of delivery mothers with SC, based on age, were mostly 29 years old. Most mothers were multiparity and multigravida. The mothers had SC history with 2-5 years of gestational interval. Most mothers were 148cm high.

The background of this study is still high Maternal Mortality Rate (MMR), MMR is one indicator to see efforts to succeed in maternal health, mortality rates can occur during pregnancy, childbirth and postpartum. Miscarriage during pregnancy and incomprehensive post-miscarriage care is one of the causes of MMR. The purpose of this study was to find out how the implementation of post-miscarriage care in the working area of the South Paringin Health Center, Balangan Regency. This research method is descriptive quantitative, using secondary data derived from medical records, the population of this study is all post-miscarriage mothers from January 2021 to October 2022, the sample in this study uses total sampling techniques where all populations are sampled, namely 37 post-miscarriage mothers. The results showed that post-miscarriage care has not been said to be comprehensive because it still focuses on medical management and is only based on patient complaints.

Diarrhea is a condition where there is increased defecation due to an infection. Children can be said to have diarrhea if the volume of stool is measured at 10ml/kg per day. According to the World Health Organization (WHO) 2020, the benefit of washing hands is to keep hands clean, not only as an effective defense against the spread of infections and serious diseases. The aim of the research was to determine the relationship between hand washing and the incidence of diarrhea in children in Cibinong District. This type of research is quantitative research. The research design used is correlational research with a controlling approach. The sampling technique uses total sampling. The number of samples in this research is 30 medical records of children suffering from diarrhea at the Cirimekar Community Health Center. In this research, the Chi-Square statistical test is used. . The results show that the relationship between hand washing behavior and the incidence of diarrhea shows that the P-Value is 0.001 <0.05, so it can be concluded that there is a significant relationship between hand washing behavior and the incidence of diarrhea in children in the Cirimekar Health Center Working Area. The conclusion of this research is that there is a relationship between hand washing behavior and the incidence of diarrhea in children in the Cirimekar Health Center Working Area in 2023.

Dyspepsia is still the most common problem at every age. It causes discomfort in the stomach, nausea, vomiting and other complaints. Dyspepsia can also interfere with a person's life, causing the sufferer to have to rest or be hospitalized.This research was conducted at the Harapan Sehat private hospital in Meulaboh, West Aceh.This study uses a descriptive method, where data is taken based on the medical records of patients hospitalized at Harapan Sehat hospital throughout 2022. The data taken were patients diagnosed with dyspepsia and treated by an internal medicine specialist.In this study, 256 patients were admitted with a diagnosis of dyspepsia by an internal medicine doctor.The study found that dyspepsia cases were mostly experienced by women, with ages above 40 to 60 years old being the most common age. Meanwhile, patients underwent the most treatment period of 3 days.

Premature Rupture of Membranes (PROM) is a condition where the amniotic membranes rupture before signs of labor appear. Cases of premature rupture of membranes at Brebes District Hospital are cases that are often encountered; this case reached 273. The impact of PROM has a high risk to the continued health of the mother and fetus. The aim of this research is to analyze midwifery care comprehensively using Varney's 7 steps. This research uses a qualitative descriptive method with a case study approach. Data collection used interview methods, observation, physical examinations, supporting examinations, medical records, KIA books, and other literature. The main informant was Mrs. “J”, while the additional informants were the coordinating midwife, general practitioner, obstetrician, laboratory officer, husband, and family. Based on the results, Mrs. “J” experienced problems during labor with premature rupture of membranes. Based on the results, Mrs. “J” at the time of childbirth had problems with the cranium being broken prematurely after anamnesies and continuous examination by midwife by conducting an in-depth examination with thin portio results, opening 10 cm, UUK point, cranial membrane (-), presentation of the head, and head-down hodge IV. The baby is born spontaneously, with strong crying and normal reflexes

The rapid spread of infectious diseases remains a major global health threat, and early detection is vital to minimize their impact. This research investigates the role of predictive modeling using big data in the early detection of infectious disease outbreaks. The primary objective of this study is to assess the effectiveness of big data systems in forecasting potential outbreaks and the implications of these forecasts for public health systems. The study employs machine learning-based predictive models to process large health datasets, including electronic health records, sensor data, and social media information. The results demonstrate that the predictive model achieved an accuracy rate of 87%, significantly surpassing traditional methods in terms of early detection. By integrating various data sources such as medical records, sensor networks, and real-time digital traces, the system is capable of providing more accurate, timely predictions, which can greatly improve the ability of public health authorities to respond effectively to emerging health threats. Furthermore, the application of big data in public health not only improves the speed of response but also enhances the allocation of resources, allowing for more targeted and efficient interventions. Despite these successes, challenges remain, particularly in relation to data quality, privacy, and regulatory issues, which could hinder the broader implementation of such systems. Thus, collaboration between government agencies, healthcare institutions, and technology developers is essential to overcome these obstacles and ensure the sustainable integration of big data into public health infrastructures. This research highlights the significant potential of big data to transform public health responses, offering valuable insights for future epidemic management strategies.

Electronic Medical Records as a form of renewal of health services, which has emerged as a response to and challenges arising from advances in technology. The Minister of Health Regulation No. 269/Menkes concerning Medical Records is no longer in accordance with developments in science and technology, health service needs and the legal needs of society. At this time the government has determined the regulations to be used, namely Minister of Health Regulation no. 24 of 2022 concerning Medical Records. With this regulation, hospitals must improve their preparation so that they can be integrated with the government system. The aim of this PKM is to increase the knowledge of medical personnel in administering electronic medical records in hospitals. The PKM implementation method includes material presentation, discussion and evaluation. The target of PKM consists of 14 health workers with the results of PKM being an increase in health workers' knowledge in maintaining medical records.

Background: Errors in inputting medical record data are still found in health services. The phenomenon that occurs in the use of electronic medical records is still an obstacle for health workers due to the complexity of the system. So research to reduce errors in inputting electronic medical record data needs to be done. Aims: This study was conducted to examine errors in inputting medical record data. The error of medical record data is influenced by technological stress factors. In addition, technological stress which is a mediator variable is also influenced by three factors, namely technology that is realistically more advanced, compatibility and complexity. Methods: . Information collected through questionnaires distributed to health workers who meet certain criteria and using electronic medical records. A total of 200 individuals in the sample were selected, selected using nonprobability sampling techniques. To test the validity of the Strucktural Equation Model (SEM) Analysis used with the Amos method Results: The results of data analysis in this study show that technological stress has a positive impact on electronic medical record data errors. In addition, relatively more advanced technology and complexity also positively affect technology stress and compatibility negatively affect technology stress Conclusion: The application of technology for hospital administration must be easy to use without the need for more learning and compatibility between work, technology used, and people who use it to reduce stress on technology in order to facilitate the input of medical records.

Corruption is still a scourge and a sore that destroys the economic joints of a country or nation. Problem formulation: 1) What is the normative basis of the authority of the District Attorney's Office in overcoming corruption; 2). What is the role of the District Attorney's Office in overcoming corruption? This research will be compiled using the normative legal research type. The data sources used are primary and secondary data. Secondary data in this study include: Primary legal materials, consisting of: Law Number 48 of 2009 concerning Judicial Power, Law Number 16 of 2004 concerning the Attorney General's Office of the Republic of Indonesia, Law Number 31 of 1999 which has been amended to Law Number 21 of 2001, Law Number 30 of 2002 concerning the Corruption Eradication Commission, Law Number 46 of 2009 concerning the Corruption Court. Research results: 1). Protection of patient rights as consumers in medical records is the existence of regulations that The normative basis for the authority of the District Attorney's Office in dealing with corruption crimes is divided into two, namely preventive handling and repressive handling. Preventive handling of corruption crimes by the prosecutor's office is an action taken within the framework before the crime or criminal act occurs, the legal basis for which is Article 30A and Article 30B letter d of Law Number 11 of 2021 concerning Amendments to Law Number 16 of 2004 concerning the Attorney General's Office of the Republic of Indonesia. The main umbrella is Law Number 8 of 1981 concerning the Criminal Procedure Code, especially those stated in Article 1 number 6 letters (a) and (b) of the Criminal Procedure Code. The prosecutor as a criminal investigator also serves as a public prosecutor in handling corruption crimes. So to complete his obligations, the prosecutor must cooperate with other related parties. Cooperation with other parties is called a legal relationship, because in carrying out cooperation in a rule or law that is certain in nature. Legal relations with other parties can be individuals, legal entities and other government agencies.

Alignment is the process of aligning medical records in the storage room according to the system used. The medical record storage system at Kembangan Hospital uses centralization, the medical record numbering system uses the Unit Numbering System (UNS) and the alignment system uses Terminal Digit Filing (TDF). The aim of this research is to determine the accuracy of medical record alignment. This research used a descriptive method with a quantitative approach, the population was 293 medical records using a saturated sample. From the results of research on the primary digit 07 storage shelf, it was found that there were inaccuracies in the alignment of 162 medical records (55%). This alignment inaccuracy was divided into three digits, namely 34 medical records (21%) had the 1st digit incorrect, 85 medical records (52%) had the 2nd digit incorrect and 43 medical records (27%) had the 3rd digit incorrect. . It was found that the cause of the alignment error was due to human error, the unavailability of a tracer and the map not using color coding. Medical records that cannot be found can be traced by looking at the patient's history of treatment at Simrs Khanza, then looking for the first digit number that is being searched for. When a medical record is not found, a search for the accuracy of the alignment of the medical record is carried out using the medical record tracking method. If a medical record cannot be found, a new medical record will be used temporarily.

Babies with Low Birth Weight (LBW) are a health problem for babies and can affect the number of Infant Mortality Rates (IMR). Another factor that can cause LBW is the hemoglobin level of pregnant women or anemia in pregnant women. Anemia in pregnant women can cause mortality and morbidity in both mother and baby. Research objective: to determine the relationship between anemia in pregnancy and the incidence of LBW babies in RSUD Kabupaten Tangerang. Research method: this study used an analytic observational design with a retrospective cross-sectional approach. The sample for this study were all babies born with a birth weight < 2500 grams for the September-December 2022 period of 123 babies. The sampling method used in this study is the total sampling method. Data was obtained by filling out a note sheet from the search results of the respondent's medical records. Univariate and bivariate data analysis using the chi-square test. The results of the study: of the 123 respondents, the majority were 75 respondents (61%) with mild anemia and 97 respondents (61%) with LBW. There was a relationship between anemia in pregnancy and the incidence of LBW babies with a p value of 0.000 <α (0.05). Conclusions and suggestions: There was a relationship between anemia in pregnancy and the incidence of LBW babies in RSUD Kabupaten Tangerang. Hospitals are advised to improve nursing services, especially health education to increase pregnant women's knowledge about the importance of iron during pregnancy in an effort to prevent anemia during pregnancy, one of which can cause low birth weight babies.

Anemia in pregnancy is a national problem because it reflects the socio-economic welfare values of society, and has a huge influence on the quality of human resources. The high incidence of anemia in pregnant women is very worrying because it can have a negative impact on mothers during pregnancy, childbirth and postpartum. The factors that influence the incidence of anemia are lack of nutrients, apart from that, education and maternal parity are closely related to the incidence of anemia. The aim of this study was to obtain information about anemia in pregnant women focusing on education, parity, maternal age and gestational age. This type of research is descriptive, namely to find out the general picture of the incidence of anemia in pregnant women, where the information is obtained from the results of medical records at the Kampili Community Health Center. Overall the population was 1090 and a sample of 375 pregnant women who experienced anemia were obtained. The results of the study concluded that the average education level of mothers who experienced anemia was mothers with a low education parity of more than 4, the mother's age was under 20 years and the gestational age was in the second trimester.

The availability of medical record files accurately and quickly when needed is one indicator of quality patient service so that patients feel well served. Correct and valid reports depend on the process of obtaining the data, therefore a management system is needed that is able to monitor the data processing process quickly and in real time. Currently the system running at the Sumbersari Community Health Center is implemented manually. The manual medical record system takes quite a long time plus the risk of being vulnerable to loss, damage and requires a large space to store the documents. Using a website-based medical record system is the right solution to solve this problem.  So that all medical records personnel can quickly master the use of this system, assistance is needed in the form of socialization on the use of a website-based electronic medical records system

Blockchain platforms propagate into every facet, including managing medical services with professional and patient-centered applications. With its sensitive nature, record privacy has become imminent with medical services for patient diagnosis and treatments. The nature of medical records has continued to necessitate their availability, reachability, accessibility, security, mobility, and confidentiality. Challenges to these include authorized transfer of patient records on referral, security across platforms, content diversity, platform interoperability, etc. These, are today – demystified with blockchain-based apps, which proffers platform/application services to achieve data features associated with the nature of the records. We use a permissioned-blockchain for healthcare record management. Our choice of permission mode with a hyper-fabric ledger that uses a world-state on a peer-to-peer chain – is that its smart contracts do not require a complex algorithm to yield controlled transparency for users. Its actors include patients, practitioners, and health-related officers as users to create, retrieve, and store patient medical records and aid interoperability. With a population of 500, the system yields a transaction (query and https) response time of 0.56 seconds and 0.42 seconds, respectively. To cater to platform scalability and accessibility, the system yielded 0.78 seconds and 063 seconds, respectively, for 2500 users.

Background: Medical records at Udayana Denpasar Level II Hospital that exceed capacity are caused by double medical records, misfiles occur during the implementation of retrieval, the retention process has not run optimally and the increasing number of patient visits. The purpose of this study was to determine the impact of medical record storage overload on the response time of medical record provision. Methods: The research design used is a quantitative method with a cross sectional approach. The data analysis used is the Chi Squere test. With a total sample of 16 medical record officers and medical record documents contained in 30 medical record storage shelves where each shelf consists of 5 sections.   Results: The results of the Chi Squere test show that the p value is 0.002, the value is less than 0.05. So it can be concluded that there is an impact of overload of medical record storage on the response time of providing medical records, thus impacting the quality of hospital services, the services provided become long because they wait for medical records to be delivered to the polyclinic. Based on these results, it is necessary to transfer media by scanning medical records then stored in sharing folders and uploaded to the interface system. This media transfer is expected to provide a solution in dealing with the overload of medical record storage so that it can be more efficient in providing medical records.       Conclusion: The conclusion in this study is that there is an impact of overloaded medical record storage on the response time of medical record provision, which has an impact on the quality of hospital services, the services provided will be long because they wait for medical records to be delivered to the polyclinic, so that doctors experience problems in patient examinations because they cannot see the patient's previous examination history.

Background: Based on the results of observations made in September 2022-December 2022 in the medical record storage room of the Tk. IV Dr. R. Ismoyo Kendari, there were 427 inpatient medical records and 254 (59%) inpatient medical records that suffered damage caused by physical, chemical and biological factors. The purpose of this study was to determine the causes and types of damage to the medical records of inpatients due to physical, chemical and biological factors in the medical record storage room of the Tk Army Hospital. IVDr. R. Ismoyo Kendari.Methods: This study used a qualitative analytic descriptive design, especially the cross sectional type. The sample in this study consisted of 4 medical record officers, namely 1 Head of Medical Records, 1 Secretary of Medical Records, 1 registration officer and 1 filling officer in the medical record storage room of the Tk Army Hospital. IV Dr.R.Ismoyo Kendari and 254 medical records of inpatients in September 2022 - December 2022 who were damaged. The research instrument used observation, checklists, and interview guidelines. Results: The results of this study indicate that physical factors, chemical factors, and biological factors cause damage caused by negligence of officers, exposure to sunlight, dust, ink, rust, mold, and insects. Conclusion:The conclusion of this study found forms of damage such as torn, detached/missing medical record forms, and yellow to brown in color as a result of physical factors, chemical factors, and biological factors.

Electronic Medical Records (EMR) is one of the health IT applications that is becoming a trend around the world. EMRs are considered to improve the overall quality of care and have an impact on patient safety. EMRs can also help reduce costs, improve accessibility, and improve the quality of hospital services. Therefore, the preparedness analysis of the implementation of EMR with the DOQ-IT approach (Doctor's Office Quality-Information Technology) is the goal of this research. This research uses a type of quantitative descriptive research with a cross sectional approach. The samples were taken from all the officers who were in direct contact with the medical records at AMC Aisyiyah Clinic. Sampling technique using a total sampling of eight officers. Primary data collection is done with a questionnaire. The method of analysis used is descriptive analysis of frequencies, percentages, graphs and tables. From the results of analysis using the DOQ-IT method obtained a total score of 104.75 belongs to category III which shows that human resources, organizational work culture, governance and leadership, IT infrastructure AMC Aisyiyah Clinic is ready to take advantage of EMR as well as overcome the potential challenges for successful adoption of the EMR.

Diarrhea is a condition in which a person has bowel movements with a soft or liquid consistency, it can even be just water and the frequency is more frequent in one day. Diarrheal disease in toddlers is ranked 2 out of the top 10 childhood diseases at the Sadewa Mother and Children Hospital (RSKIA) in May 2022. Knowing the pattern of prescribing diarrhea drugs in toddlers in outpatient care at the Sadewa Mother and Children Hospital for the period January - June 2022. The research conducted was a non-experimental descriptive research with a retrospective method. The study was conducted by taking prescription data from electronic medical records at the Sadewa RSKIA pharmacy and then recording them on a data collection sheet. Data were analyzed using tabulations in the form of percentages (%). The results showed that out of 130 samples, the profile of diarrhea patients under five was dominated by males, 77 children (59%), aged 2-3 years 65 children (50%), and the degree of dehydration of patients without dehydration, 120 children (92%). ). The most widely used drug class was zinc with 120 prescriptions (34.3%), zinc has a protective effect against diarrhea and can reduce recurrence and probiotics with 119 prescriptions (33.8%), probiotic bacteria will help the process of nutrient absorption and prevent disturbances in water absorption. The dominating combination was a combination of 3 drugs namely electrolytes, zinc and probiotics 69 prescriptions (57%), oralit functioned to replace fluids and electrolytes in the body that are wasted during diarrhea, zinc supplementation and probiotics in diarrhea effectively reduced stool output. The rule for using the treatment of diarrhea in toddlers is mostly 1 time a day with a total of 132 drugs (37.7%).
