Operational form reports which were recorded manually, of course, the results were less effective in terms of saving time and costs. The used of electronic operating reports provides convenience in terms of operation. Digital archiving to a computer makes it easier in terms of data management and distribution. The purpose of this study was to recommend for the Kediri District General Hospital (RSKK) by designing an electronic operation report design by strengthening the interface design, color and input. This study used observation and interviews with two medical record officers. The results of this study were the use of the operating report form in RSKK the recording was still done manually. Then the researchers designed an interface design that was tailored to the wishes of the officers who emphasized the basic color of gray, a simple layout and easy input methods. It can be concluded that it has resulted in the design of an electronic operation report form design that was integrated in the operating report system. This study has suggested implementing the use of an electronic operating reporting system that makes it easier for officers to operate it in the RSKK environment.

Doctors at the Tanjunganom Health Center UPTD use a medical action approval form before taking medical action. Baseded on the findings, the implementation of the medical action approval form at the Tanjunganom Health Center UPTD was difficult to read and the writing was not in accordance with the rules for documenting medical records. Whereas approval of medical action as evidence of legality in court and avoiding malpractice actions that harm patients. The purpose of the study was to design an electronic medical action approval form according to the needed of the officer, based on color, layout and control. This study used a qualitative descriptive, for the population and a sample of 2 inpatient registration officers who use informed consent. The result of this research is the interface design of the electronic medical action consent form. The results of interviews with 2 inpatient registration officers obtained that the colour uses light green as a colour symbol from the Tanjunganom Health Center, the layout was neatly arranged according to the comfort of the officers when using the application, and the controls used are icons and explanatory text adjusted to their functions so that the use of buttons was easy. understood. These results are adjusted to the needed of officers to make it easier for officers to operate the application. It should be realized immediately by organizing training, outreach to officers so that services are more effective and efficient.

Outpatient registration at Tanjunganom Health Centre is done online and manually. Patients who already have BPJS can follow the online registration method, while those who use the manual method can enter Puskesmas. From the results of random interviews with several patients for manual registration, it takes a long time of around 15 minutes, from the resulted of observations, one of the causes is the registration time at the counter and the place is not wide enough, from these problems, an outpatient registration application needed online via mobile with layout, color, and control to made it easier for officers to register patients and also make it easier for patients to carry out examinations and shorten the time. This type of research is descriptive with the object being studied, namely the registration unit, for the population in the registration study and patients a number of 2 officers and 2 patients using saturated sampling. For data collection was using observation and interviews. The results of this study are an online outpatient registration application via mobile based on layout, color, and control at the Tanjunganom Health Center. The informed consent form that not properly recorded, difficult to read, became a problem that occurs in the inpatient registration unit, so it was better to do it by typing to made it clearer and easier to read. the use of electronic informed consent forms provides easy distribution so that it can save time and costs. the use of electronic informed consent forms made it easier for officers in digital archiving on computers so they didn't save a lot of files. The aim of the study was to recommended that UPTD Puskesmas Baron made an informed consent information system interface design accorded to user needs, based on interface design, color, and input. This study used observations at the UPTD Puskesmas Baron, by observing 2 inpatient registration officers, conducting interviews with 2 inpatient registration officers. The result of this study was the informed consent form at the UPTD Puskesmas Baron was manual which was used as an approval for action in inpatient registration, that approval of action must be carried out by Permenkes No. which it must be exist with the suitability of the officer. This research has suggested implementation, socialization and training for the application of electronic informed consent forms.
